Factors affecting the implementation of employee whole health in the veterans health administration: a qualitative evaluation.

BACKGROUND: There is increasing recognition of the need to focus on the health and well-being of healthcare employees given high rates of burnout and turnover. Employee wellness programs are effective at addressing these issues; however, participation in these programs is often a challenge and requires large scale organizational transformation. The Veterans Health Administration (VA) has begun to roll out their own employee wellness program-Employee Whole Health (EWH)-focused on the holistic needs of all employees. This evaluation's goal was to use the Lean Enterprise Transformation (LET) mode

Total Worker Health® Needs Assessment to Identify Workplace Mental Health Interventions in Rural and Urban Jails.

IMPORTANCE: Jail officers are an underserved population of public safety workers at high risk for developing chronic mental health conditions. OBJECTIVE: In response to national calls for the examination of stressors related to the unique work contexts of correctional facilities, we implemented a pilot study informed by the Total Worker Health® (TWH) strategy at two urban and two rural jails. DESIGN: Participatory teams guided areas of interest for a mixed-data needs assessment, including surveys with 320 jail officers to inform focus groups (N = 40). SETTING: Urban and rural jails in the m

[Tuberculosis and healthcare workers: Risk reduction and disease prevention in health care settings].

Tuberculosis is a human disease caused by Mycobacteriumtuberculosis, and transmitted by airborne pathway. Documented cases of tuberculosis infection in healthcare workers have been reported in both developed and developing countries. Early recognition of potentially infectious cases, immediate implementation of airborne precautions and prompt medical treatment of cases, are required to lower the risk of disease transmission. Molecular biology techniques allow earlier diagnosis. In the event of non-compliance with airborne precautions, the investigation will further have to establish exhaustive

Approaches to worker rehabilitation by occupational and physical therapists in the United States: factors impacting practice.

Work-related rehabilitation services have changed in nature and scope since their inception in the late 1970s. A review of the literature reveals a large body of published data concerning the various approaches used by therapists in this practice area, but a limited number of comparison studies documenting the value of one approach over the other. This national survey of physical and occupational therapists in the US was conducted in 2002, and examined the prevailing trends in service provision, and factors that determine the nature of services provided to clients. Results indicate that servic

Prevention of lead poisoning in construction workers: a new public health approach.

BACKGROUND: In 1990, Yale University, the Connecticut Departments of Health Services and of Transportation, the Connecticut Construction Industries Association, and the state's construction trade unions created the Connecticut Road Industry Surveillance Project (CRISP). METHODS: Data from 90 bridge projects from 1991 to 1995 and approximately 2,000 workers were evaluated. The distribution of peak lead concentrations in the blood for CRISP workers classified into five groups were compared to that from workers outside of Connecticut. RESULTS: This demonstration project was instrumental in lowe

A workplace breast cancer screening program. Costs and components.

Screening for breast cancer can result in early detection of malignancies and lives saved. Many employers now offer periodic screening as an employee health benefit, and some have established screening programs in the workplace. This study was performed to identify the employer costs of breast cancer screening in the workplace, referrals for suspicious findings, and initial treatment of malignant disease. Additionally, the costs for these same services, had they been obtained outside of a workplace screening program, were estimated. Data on program components and associated costs for an establ

Post-abortion adjustment of health care professionals in training.

Sixty-three women students of the health care professions were surveyed about their abortion and post-abortion experiences, feelings, and attitudes. Variables predictive of short- and long-term adjustment, belief in the wisdom of the choice, and right to choose were analysed, as were effects on emotional and attitudinal responses of passage of time and pressure to abort. Most reported mild, transient, short-term negative effects, and generally benign long-term effects. Implications of the findings are discussed.

Occupational exposure and lung cancer risk.

The importance of occupation held longest as a risk factor for lung cancer was examined in a prospective study in Norway of 11,995 men, among whom 125 cases occurred in a follow-up from 1966 through 1978. Based on information about occupation held longest, the respondents were classified into 3 groups according to suspected exposure to respiratory carcinogens at the workplace. After stratification for age, place of residence and cigarette smoking, we found a highly significant relative risk of 2.6 for those judged to have experienced definite exposure versus the group with no workplace exposur

Smoking intervention in the workplace: review and recommendations.

Growing interest has been expressed by the business community concerning intervention against smoking in the workplace. As most adult smokers are unlikely to have access to smokers' clinics or other treatment facilities outside the workplace, the workplace itself could be an ideal location for the provision of treatment programs. If the interest of the business community can be translated into action, large populations of smokers would become accessible to workplace treatment programs. Despite this possibility, the potential of the workplace for smoking intervention remains largely unexplored.

Return to Work After Breast Cancer Diagnosis and Treatment in Uruguay: Challenges and Determining Factors.

INTRODUCTION: In Uruguay, breast cancer (BC) is the most common malignancy in women and the leading cause of cancer-related mortality in the female population. It represents a significant public health problem. Improved survival, driven by early diagnosis and therapeutic advances, has brought new challenges related to quality of life, including return to work and domestic activities, which are key to full recovery and social reintegration. OBJECTIVE: To determine the rate of return to work in women with BC at least 24 months after diagnosis and to analyze associated factors. To evaluate the i

Sero surveillance of human brucellosis and leptospirosis among veterinary personnel in Sikkim, India: A descriptive cross -sectional study.

A cross-sectional sero-surveillance study was conducted across the five districts of Sikkim to determine the seroprevalence of Brucella and Leptospira antibodies among veterinary and para-veterinary personnel. Serum samples were tested for Brucella using the Rose Bengal Test, Standard Agglutination Test, and ELISA (IgG/IgM), while Leptospira antibodies were detected using ELISA (IgG/IgM). Brucella seroprevalence was 18.8%, and Leptospira seroprevalence was 20.5%. The findings indicate a considerable level of exposure to both infections in this occupational group, underscoring the need for stre

A Case of Dilated Cardiomyopathy Associated With Chronic Toluene Exposure.

A 56-year-old woman working in a factory for the past 34 years presented with dilated cardiomyopathy of unknown etiology. She was exposed daily to toluene at work, without adequate protective equipment. A public health department investigation reported that toluene exposure among workers at her factory was more than 3 times the established limit. Toluene inhalation is associated with cases of cardiac arrhythmias, vasospasm, and cardiomyopathy. Occupational exposure to cardiotoxic substances should be investigated in patients with idiopathic cardiomyopathy. This case report highlights an associ

A Chlamydia psittaci Outbreak in Psittacine Birds in Sardinia, Italy.

Chlamydia psittaci is an intracellular bacterium belonging to the Chlamydiaceae family. It is the ethiologic agent of psittacosis, an occupational zoonotic disease that mainly concerns people who work in close contact with birds that represent the main infection route for human transmission. In Italy, information about this disease is lacking. This study is the first case of avian chlamydiosis reported from a pet shop in Sardinia, Italy. Chlamydia psittaci detected in psittacine birds by molecular analysis, direct immunofluorescence test together with anatomo-pathological observed lesions, hig

Palliative Care for Polio and Postpolio Syndrome.

Palliative care is a team-based approach focusing on relief of physical, psychosocial, and existential distress and communication about serious illness. Patients with poliomyelitis and postpolio syndrome are at risk for contractures and can benefit from involvement of physical and occupational therapy. Hypersialorrhea can be treated with anticholinergic medications, botox, or radiation. Patients with dyspnea may require noninvasive positive pressure ventilation ± opioids or benzodiazepines. Constipation is often due to autonomic dysfunction and decreased mobility. There is a higher burden of a

Ear and vestibular symptoms in train operators after sudden air pressure changes in trains.

A healthy 31-year-old train operator presented to our occupational health clinic reporting ear aches, headaches, dizziness, unsteadiness and even slight tinnitus. These symptoms first appeared when the patient started operating from a new train cabin. He described a sudden pressure gradient, experienced on some parts of the trajectory, which might have caused these problems. Although the cabins were equipped with a pressure equalising device, this was usually switched off because of the device creating an uncomfortable feeling in the cabin. The literature describes sudden pressure gradients as

Visual diagnosis: 6-month-old boy with leg pain.

Pediatricians play an important role in diagnosing OI as a cause of fracture and may be asked to differentiate this uncommon genetic diagnosis from intentional trauma and other causes of fracture. Early referral to a pediatric endocrinologist, physical therapist, and orthopedic surgeon for the evaluation and treatment of low bone mass and recurrent fractures is important because early medical and surgical intervention may help to minimize the rate of future fractures, even within the first year of life. Continued follow-up with physical and occupational therapy, audiology testing, and regular

[Cholangiocarcinoma among printing workers].

By June 2013, seventeen workers had suffered from intrahepatic or extrahepatic bile duct cancer (cholangiocarcinoma) in an offset proof-printing company in Osaka and nine of the workers had died. Ages at diagnosis were 25 to 45 years old. Known risk factors for cholangiocarcinoma were not found in the patients. All of the patients were exposed to 1,2-dichloropropane at high level for long-term and were diagnosed with cholangiocarcinoma 7 to 20 years after the first exposure. Twelve of the patients were also exposed to dichloromethane. The Japan Ministry of Health, Labour and Welfare recognized
