Patient satisfaction following total knee arthroplasty using restricted kinematic alignment.

AIMS: Alternative alignment concepts, including kinematic and restricted kinematic, have been introduced to help improve clinical outcomes following total knee arthroplasty (TKA). The purpose of this study was to evaluate the clinical results, along with patient satisfaction, following TKA using the concept of restricted kinematic alignment. METHODS: A total of 121 consecutive TKAs performed between 11 February 2018 to 11 June 2019 with preoperative varus deformity were reviewed at minimum one-year follow-up. Three knees were excluded due to severe preoperative varus deformity greater than 15

Mid-level constraint may correct coronal plane imbalance without compromising patient function in patients with severe osteoarthritis.

INTRODUCTION: Mid-level constraint (MLC) in Total Knee Arthroplasty (TKA) offer surgeons the opportunity to obtain a well-balanced varus-valgus balance in the setting of slight ligament imbalance. As such, we sought to evaluate patient reported outcome measures (PROMs), alignment correction, and rate of revision between the MLC group and a cohort of posterior stabilized (PS) knees in a cohort of patients with preoperative degenerative arthritis. MATERIALS/METHODS: We performed a retrospective review of 57 MLC knees and 96 PS knees from a single manufacturer that were implanted by a single sur

Cartilage Subsurface Changes to Magnetic Resonance Imaging UTE-T2* 2 Years After Anterior Cruciate Ligament Reconstruction Correlate With Walking Mechanics Associated With Knee Osteoarthritis.

BACKGROUND: Anterior cruciate ligament (ACL) injury increases risk for posttraumatic knee osteoarthritis (OA). Quantitative ultrashort echo time enhanced T2* (UTE-T2*) mapping shows promise for early detection of potentially reversible subsurface cartilage abnormalities after ACL reconstruction (ACLR) but needs further validation against established clinical metrics of OA risk such as knee adduction moment (KAM) and mechanical alignment. HYPOTHESIS: Elevated UTE-T2* values in medial knee cartilage 2 years after ACLR correlate with varus alignment and higher KAM during walking. STUDY DESIGN:

Knee-Extension Torque Variability and Subjective Knee Function in Patients with a History of Anterior Cruciate Ligament Reconstruction.

CONTEXT: When returning to physical activity, patients with a history of anterior cruciate ligament reconstruction (ACL-R) often experience limitations in knee-joint function that may be due to chronic impairments in quadriceps motor control. Assessment of knee-extension torque variability may demonstrate underlying impairments in quadriceps motor control in patients with a history of ACL-R. OBJECTIVE: To identify differences in maximal isometric knee-extension torque variability between knees that have undergone ACL-R and healthy knees and to determine the relationship between knee-extension

[Progress on study of factors influencing the prognosis and complications of surgery for discoid meniscus injury in children].

The discoid meniscus is a common congenital meniscal malformation that is prevalent mainly in Asians and often occurs in the lateral discoid meniscus. Patients with asymptomatic discoid meniscus are usually treated by conservative methods such as observation and injury avoidance, while patients with symptoms and tears need to be treated surgically. Arthroscopic saucerization combined with partial meniscectomy and meniscus repair is the most common surgical approach., and early to mid-term reports are good. The prognostic factors are the patient's age at surgery、follow-up time and type of surge

Bilateral Patellar Tendon Rupture Following Total Knee Arthroplasty in a Patient With Alkaptonuria: A Case Report.

Alkaptonuria (AKU) is a rare hereditary disorder of tyrosine degradation. The disorder is characterized by the accumulation of a pigment called homogentisic acid. Its accumulation can lead to the breakdown of connective tissue, including tendons. This report presents a 46-year-old male with a history of bilateral total knee arthroplasty (TKA) who sustained bilateral patellar tendon rupture after an acute injury. A single-stage bilateral knee revision with direct repair of the extensor mechanism augmented with Achilles allograft was performed. The procedure was successful, and the patient had a

Validity and reliability of inertial measurement units on lower limb joint angles during landing and cutting tasks: A systematic review and meta-analysis.

OBJECTIVE: The use of wearable inertial measurement units (IMUs) to measure kinematic parameters of high-risk movements, such as landing and cutting, for injury prevention is becoming more common. However, the validity and reliability of IMUs in assessing lower limb joint angles still lack comprehensive validation. This systematic review and meta-analysis assessed the validity and reliability of lower limb kinematic data captured by IMUs during landing and cutting tasks. METHODS: Comprehensive searches were conducted in the Web of Science, Scopus, PubMed, and EBSCO databases up to April 10, 2

Lower Extremity Mechanical Energetics During Walking 6 Months Following Anterior Cruciate Ligament Reconstruction.

Aberrant gait biomechanics following anterior cruciate ligament (ACL) injury and ACL reconstruction (ACLR) are critical factors contributing to the development of knee osteoarthritis (KOA). However, it remains unknown how ACLR impacts lower extremity joint energetics and each joint's contribution to total limb energetics during walking. To compare mechanical energetics at the ankle, knee, and hip joints between ACLR limbs, contralateral, and control limbs during walking. Forty-four participants with primary unilateral ACLR (post-ACLR: 6.0 ± 0.2 months) and 44 uninjured controls were studied. A

Patient-reported impairment following TKA is reduced when a computationally simulated predicted ideal alignment is achieved.

PURPOSE: Joint dynamics following Total Knee Arthroplasty (TKA) may influence patient-reported outcome. Simulations allow many knee alignment approaches to a single patient to be considered prior to surgery. The simulated kinematics can be matched to patient-reported outcome to predict kinematic patterns most likely to give the best outcome. This study aims to validate one such previously developed algorithm based on a simulated deep knee bend (the Dynamic Knee Score, DKS). METHODS: 1074 TKA patients with pre- and post-operative Computerised Tomography (CT) scans and 12-month post-operative K

Subvastus Exposure Compared to Parapatellar Approach in Navigated Sequential Bilateral Total Knee Arthroplasty (SBTKA): A Prospective Randomized Study.

Subvastus approach preserves the quadriceps mechanism and may lead to improved early functional outcomes as compared with a parapatellar approach in primary knee arthroplasty. We performed a prospective randomized study to test the hypothesis if subvastus approach improves patient- and physician-reported outcomes in navigated sequential bilateral knee arthroplasty when compared with the standard parapatellar approach. A total of 93 patients were allotted in each group after power analysis and randomization done by computer-generated sequence: group S by subvastus approach and control group P b

Musculoskeletal modeling to predict and reduce Antetrior Cruciate Ligament injury during single leg drop jump activity: Synergistic muscle co-activation approach.

This paper presents a `drop jump' modeling to study the effect of synergistic muscle activation on controlling Anterior Cruciate Ligament (ACL) injury. ACL injuries are mostly caused during high impact loading. A full body musculoskeletal model with knee ligaments have been developed in `OpenSim platform' to simulate ACL injury during drop jump activity. The model is used to quantify the effect of change in muscle activation on different kinetic and kinematic parameters, which are associated with ACL injury. A neuromusculoskeletal controller have been designed which selects optimal muscle acti

Asymmetric Post-Traumatic Knee Arthritis Is Closely Correlated With Both Severity and Time for Lower Limb Coronal Plane Malalignment.

OBJECTIVE: Mechanical alignment of the lower limbs has been suggested to cause abnormal uneven loading across the compartments at the knee, but its contribution to the initiation and progression of arthritis remains controversial. This study aimed to establish whether malalignment of the lower limb after trauma is associated with worsened arthritis scores in the theoretically overloaded compartment, and if arthritis scores continuously correlate with the degree of malalignment and time with deformity. DESIGN: After screening 1160 X-rays, 60 patients were identified with long-leg radiographs >
