Deep vein thrombosis in pregnancy and the puerperium: a comprehensive review.

Although uncommon during pregnancy and the postpartum period, deep vein thrombosis (DVT) and its sequela, pulmonary thromboembolism, remain leading sources of maternal mortality and morbidity. The majority of literature concerning DVT in pregnancy and the postpartum period exists in the medical journals, with only a small amount in nursing journals. This article provides the nursing community a comprehensive review of literature concerning the incidence of both DVT and pulmonary embolism in pregnancy and the postpartum period, identification of risk factors, the effectiveness of available scre…

[Intestinal intussusception in pregnancy with obstruction, caused by benign neurilemmoma. Role of abdominal echocardiography in the preoperative diagnosis].

A rare case of intestinalm obstruction caused by intussusception of the small bowel due to an intraparietal benign neurilemmoma (synonym: benign schwannoma) in a patient 35th weeks pregnant is reported. It is assumed that intestinal intussusception during pregnancy is a rare complication and the diagnosis is made difficult due to the pregnancy itself, delaying the onset of symptoms and treatment. The echography was the determining factor for the diagnosis. The conclusion is drawn that surgical operation, associated or not with the cesarean section, should be carried out as quickly as possible

[Clinico-morphological characteristics of fetuses and newborn infants of mothers with hemorrhagic shock].

A clinical and morphological analysis has been done using 24 obstetric histories of mothers with hemorrhagic shock (HS) and autopsy protocols of fetuses and newborns who died ante-, intra- or postpartum. Maternal HS was related to placenta previa and presented as a poor general status and microcirculatory disorders, respiratory and adrenal insufficiency. Histologic and electron microscopic studies showed fetal and neonatal lesions in organs and tissues which might be interpreted as irreversible shock. Reduction of neonatal mortality dictates further efforts in the prevention of hemorrhagic com

Perinatal mortality in rural Tanzania.

Prolonged labour was the most frequent cause of perinatal death in a rural hospital in the south western highlands of Tanzania. After the introduction of an obstetric policy aiming to prevent prolonged labour by making use of the guidelines of the partogram, perinatal mortality was reduced from 71 to 39 per 1000 births. Baird's clinico-pathological classification is still considered a useful instrument for the discovery of avoidable factors in perinatal deaths. The concept of the partogram should be an integral part of the training of medical auxiliaries in the field of maternal and child heal

Oxytocin during labor after previous cesarean section: results of a multicenter study.

The use of oxytocin for labor induction or augmentation in patients with previous cesarean delivery is controversial. This paper presents a series of 1776 patients allowed to labor after a previous cesarean section, of whom 485 (27%) were treated with oxytocin. When the patients who received oxytocin were compared with those who did not, no significant differences were found with respect to uterine rupture, maternal morbidity, fetal morbidity, or fetal mortality. We conclude that the judicious use of oxytocin is safe in the patient with a previous low transverse cesarean section.

Joint-ill and other bacterial infections on thoroughbred studs.

From a perinatal survey carried out on Thoroughbred studs, information was obtained on the incidence, mortality and long-term effects of joint-ill and other bacterial infections. Many of these infections occurred in foals with a history of other perinatal abnormalities, and in foals in which receipt of colostrum was delayed. There was no direct relationship between the incidence of infection and maternal age, but prenatal lactation and loss of colostrum were more frequent in older mares. There was a significant reduction in the incidence of infections in foals given neomycin and framomycin dur

Use of evidence based practices to improve survival without severe morbidity for very preterm infants: results from the EPICE population based cohort.

OBJECTIVES: To evaluate the implementation of four high evidence practices for the care of very preterm infants to assess their use and impact in routine clinical practice and whether they constitute a driver for reducing mortality and neonatal morbidity. DESIGN: Prospective multinational population based observational study. SETTING: 19 regions from 11 European countries covering 850 000 annual births participating in the EPICE (Effective Perinatal Intensive Care in Europe for very preterm births) project. PARTICIPANTS: 7336 infants born between 24+0 and 31+6 weeks' gestation in 2011/12 wi

Prevalence of Congenital Malformations in the Oriental Region of Morocco.

This retrospective study conducted at the Mohammed VI University Hospital Center in Oujda analyzed clinically visible congenital malformations among neonates admitted over a one-year period. The maternal age profile predominantly included young adults, with a significant proportion of cases linked to consanguinity, highlighting a potential genetic contribution to the occurrence of these anomalies. Most malformations were diagnosed after birth, with congenital heart defects representing the most frequent anomaly, followed by major digestive malformations such as anal atresia and omphalocele. Uterotonics for prevention of postpartum haemorrhage: EN-BIRTH multi-country validation study.

BACKGROUND: Postpartum haemorrhage (PPH) is a leading cause of preventable maternal mortality worldwide. The World Health Organization (WHO) recommends uterotonic administration for every woman after birth to prevent PPH. There are no standardised data collected in large-scale measurement platforms. The Every Newborn Birth Indicators Research Tracking in Hospitals (EN-BIRTH) is an observational study to assess the validity of measurement of maternal and newborn indicators, and this paper reports findings regarding measurement of coverage and quality for uterotonics. METHODS: The EN-BIRTH stud…

Don't Let Sexual and Reproductive Health become Collateral Damage in the Face of the COVID-19 Pandemic: A Public Health Perspective.

South Africa, similar to many other countries in the African continent is still experiencing challenges in its efforts to provide sexual and reproductive health (SRH) care to women and adolescent girls, and it has become clear that the COVID-19 pandemic is the latest threat to universal access to SRH. In the face of this threat, the Sustainable Developmental Goals that call on the global community to -leave no one behind‖ may become a blurred vision unless we adopt a wider lens away from the tunnel vision that currently plagues health systems around the globe. This paper therefore exposes how

Association of Unexpected Newborn Deaths With Changes in Obstetric and Neonatal Process of Care.

IMPORTANCE: The death of a healthy term infant may signal patient safety and quality issues. Various initiatives aim to encourage clinicians to learn from these events, but little evidence exists regarding how exposure to an unexpected newborn death may alter clinician practice. OBJECTIVE: To examine the association between an unexpected newborn death and changes in obstetric and newborn procedures that may be used in response to potential fetal distress or newborn complications. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study used difference-in-differences analysis of 2011 to

Promoting progress in child survival across four African countries: the role of strong health governance and leadership in maternal, neonatal and child health.

Despite numerous international and national efforts, only 12 countries in the World Health Organization's African Region met the Millennium Development Goal #4 (MDG#4) to reduce under-five mortality by two-thirds by 2015. Given the variability across sub-Saharan Africa, a four-country study was undertaken to examine barriers and facilitators of child survival prior to 2015. Liberia and Zambia were chosen to represent countries making substantial progress towards MDG#4, while Kenya and Zimbabwe represented countries making less progress. Our individual case studies suggested that strong health

Trends, determinants, and newborn mortality related to thermal care and umbilical cord care practices in South Asia.

BACKGROUND: Although child mortality has decreased over the last several decades, neonatal mortality has declined less substantially. In South Asia, neonatal deaths account for the majority of all under-five deaths, calling for further study on newborn care practices. We assessed five key practices: immediate drying and wrapping, delayed bathing, immediate skin-to-skin contact after birth, cutting the umbilical cord with a clean instrument, and substances placed on the cord. METHODS: Using data from Demographic and Health Surveys conducted in Bangladesh, India, and Nepal between 2005 and 2016

Maternal age potentiates the impact of operative birth on serious neonatal outcomes.

Background: Pregnancies at the extremes of maternal age (<20 and >35) are associated with adverse obstetric and perinatal outcomes. There is limited evidence regarding the influence of maternal age on serious neonatal outcomes by mode of delivery. The aim of this study thus was to assess the relationship between maternal age and mode of delivery and its impact on a composite of serious neonatal outcomes. Methods: This was a retrospective cohort study of low risk women birthing at term in Australia's largest maternity hospital over a 7-year period. Intrapartum and perinatal outcomes were collate

Approach to an obstetric prognosis scale: The modified SOFA scale.

BACKGROUND: Severe obstetric morbidity constitutes a serious problem worldwide; however, an effective obstetrical prognosis scale is still m … curve (AUC) for the modified SOFA (0.868; p<0.001) than SOFA (0.796; p=0.003), in the prediction of maternal mortality. CONCLUSIONS: The SOFA score, taking into account a lower value for the Kirby index and a threshold time of 12-h with sepsis before getting specialized medical attention, shows a good predictive value for maternal death and could be considered for …

Delivering post-abortion care through a community-based reproductive health volunteer programme in Pakistan.

This qualitative study was conducted in May-June 2010 with women using post-abortion care (PAC) services provided by the Marie Stopes Society in Pakistan during the six month period preceding the study, more than 70% of whom had been referred to the clinics by reproductive health volunteers (RHVs). The aim of the study was to establish the socio-demographic profile of clients, determine their preferred method of treatment, explore their perceptions of the barriers to accessing post-abortion services and to understand the challenges faced by RHVs. The sample women were selected from six randoml
