Brain states analysis of EEG predicts multiple sclerosis and mirrors disease duration and burden.

BACKGROUND: Any treatment of multiple sclerosis should preserve mental function, considering how cognitive deterioration interferes with quality of life. However, mental assessment is still realized with neuro-psychological tests without monitoring cognition on neurobiological grounds whereas the ongoing neural activity is readily observable and readable. OBJECTIVE: The proposed method deciphers electrical brain states which as multi-dimensional cognetoms quantitatively discriminate normal from pathological patterns in an EEG. METHOD: Baseline recordings from a prior EEG study of 88 subjects…

Directions for advancing prognostic assessments in multiple sclerosis: Qualitative Insights from MS specialist Interviews.

BACKGROUND: Reliable prognostication in multiple sclerosis (MS) is essential for personalized care, yet remains challenging. In the absence of widely implemented prognostic tools, how MS specialists today formulate prognostic judgments and manage related challenges in routine practice is underexplored. OBJECTIVE: To investigate how MS specialists approach prognostication in practice and identify their priorities for improving prognosis-informed care. METHODS: Twelve MS specialists from European and Middle Eastern regions participated in semi-structured interviews. Data were qualitatively ana…

Technology-Assisted Solutions to Enhance Auditory-Motor Synchronization During Walking in Progressive Multiple Sclerosis.

BackgroundStudies show that beat-based cues from music and metronomes benefit gait in persons with progressive multiple sclerosis (PwPMS) during short walking. While useful for controlled assessment, prolonged walking better reflects real-world function. Adaptive beat-alignment algorithms using biofeedback may enhance synchronization and benefit gait dynamics but their effects in PwPMS remain unknown.AimsTo investigate the effect of walking with and without an adaptive beat-alignment algorithm on synchronization consistency during 8 minutes of walking compared to walking in silence, and its ef…

Impaired force control and muscle activation during isometric contractions in people with multiple sclerosis.

Motor impairments affect up to 80% of people living with multiple sclerosis (MS) and are often asymmetrical, resulting in self-reports of more- and less-affected legs. The purpose of our study was to examine asymmetries in the neural control of force applied by lower leg muscles in people with MS. Eighteen people with MS and 17 healthy controls performed two types of submaximal isometric contractions with dorsiflexors and plantar flexors. The outcomes included measures of force control and electromyographic (EMG) activity of lower-leg muscles. Force control was worse in people with MS compared…

Large-scale proteomics across neurological disorders uncovers biomarker panel and targets in multiple sclerosis.

Cerebrospinal fluid (CSF) is central to neurological diagnostics, yet biomarkers are lacking for many clinical needs. To enable its large-scale proteomic characterization, we developed a high-throughput mass spectrometry workflow quantifying approximately 1,500 proteins per CSF sample across 5,000 individuals, covering a spectrum of neurological disorders. This revealed proteomic alterations associated with blood-CSF barrier impairment, age, and sex, enabling deconvolution of shared and disease-specific signatures. We then focused on multiple sclerosis (MS), using an improved analytical techno…

Bile acid-microbiota interactions in multiple sclerosis: From experimental models to early clinical evidence.

This article examines the bidirectional relationship between bile acid metabolism and the intestinal microbiota and explores how disruptions in this interaction may contribute to the pathophysiology of multiple sclerosis. Bile acids are presented not merely as digestive end-products but as bioactive signaling molecules capable of regulating immune responses, maintaining epithelial and neural homeostasis, and influencing neuroinflammatory processes. Experimental work demonstrates that alterations in microbial composition affect bile acid diversity and circulation, while bile acids themselves sh…

Intrathecal synthesis of Epstein-Barr virus antibodies 15 years before onset of multiple sclerosis.

A 48-year-old woman was diagnosed with multiple sclerosis (MS) in 2019. She participated with blood and cerebrospinal fluid samples as a spinal anaesthesia surgery patient for a case-control study on viruses and MS in 2003. The study had already demonstrated intrathecal antibody synthesis (ITS) to Epstein-Barr virus (EBV) but not against measles, rubella and varicella-zoster (MRZ) reaction viruses in this individual at the time of recruitment. A 2025 study review established that she was the only patient in her control subgroup who also developed MS. This case shows that a specific EBV-ITS may…

Increased VH4+JH6+ antibody heavy chain use in plasmablasts from asymptomatic multiple sclerosis patients.

Radiologically isolated syndrome (RIS) is a neurological condition in people with demyelinating lesions on brain and/or spinal cord magnetic resonance imaging (MRI) studies, but without clinical symptoms of disease. Elucidating the immune profile of people with RIS (pwRIS) who will display MRI or clinical features of advancing disease is critical to our understanding of disease pathogenesis. Our lab previously identified features of B cell dysregulation in people with clinically isolated syndrome (pwCIS), who have both demyelinating lesions and a first clinical event of disease. The goal of th

Disease Activity Following Cessation of Dimethyl Fumarate in Patients with Relapsing-Remitting Multiple Sclerosis.

BACKGROUND: The optimal time between the cessation of one disease-modifying therapy (DMT) and initiation of another in relapsing-remitting multiple sclerosis (RRMS) is incompletely understood. Lymphopenia, an adverse effect of dimethyl fumarate (DMF), may prompt a washout period prior to initiating an alternate DMT to allow absolute lymphocyte count (ALC) recovery. We hypothesized that during the DMF-DMT treatment interval, there would be disease activity, either by MRI or clinical relapses, and that lymphopenia might be a risk factor. METHODS: A retrospective chart review was conducted, coll…

From Demyelination to Intervention: Natural Compounds as Emerging Therapeutic Targets in Multiple Sclerosis Neuroinflammation.

Multiple sclerosis (MS) is a chronic immune-mediated inflammatory demyelinating disease of the central nervous system (CNS), characterized by multifocal lesions, axonal damage, and progressive neurological dysfunction, imposing a heavy burden on patients and healthcare systems. While T lymphocytes have long been regarded as central to MS pathogenesis, accumulating evidence underscores the pivotal role of B lymphocytes and dysregulated cytokine networks (e.g., Th17/IL-17 and NF-κB pathways) in driving disease initiation and progression. Despite advances in symptomatic management and disease-mod…

Multidimensional Determinants of Sexual Dysfunction in Multiple Sclerosis: Incremental Insights from a Hierarchical Model.

Background/Objectives: Sexual dysfunction (SD) is a common yet underrecognized consequence of multiple sclerosis (MS) with multidimensional determinants spanning neurological, physical, and psychosocial domains. However, the relative and incremental contribution of fatigue compared with disability and psychological factors remains insufficiently clarified. This study aimed to evaluate the prevalence and severity of sexual dysfunction in MS and to identify independent and incremental predictors using a hierarchical analytical framework. Methods: In this cross-sectional case-control study, 140 p…

Clinical Prediction of Functional Decline in Multiple Sclerosis Using Volumetry-Based Synthetic Brain Networks.

BACKGROUND: Disability progression in multiple sclerosis (MS) is increasingly recognized as a consequence of large-scale brain network disruption rather than isolated regional damage. Although diffusion tensor imaging (DTI) is the reference method for assessing structural connectivity, its limited availability restricts widespread clinical application. There is therefore a critical need for alternative approaches capable of capturing network-level alterations using routinely acquired MRI data. OBJECTIVE: This study aimed to determine whether synthetic structural connectivity matrices derived …

Dynamics of Spinal Fluid Immune Cell Alterations Following Cladribine Tablet Treatment in Multiple Sclerosis.

BACKGROUND AND OBJECTIVES: Oral cladribine tablet (CladT) therapy is efficacious for relapsing multiple sclerosis (MS). However, the mechanisms by which cladribine exerts benefit in MS remain unclear, particularly regarding its effects on compartmentalized inflammation within the CSF. METHODS: Transcriptional profiles along with T and B lymphocyte receptor repertoires from CSF and blood were obtained by single-cell sequencing methods from a single site participating in a phase IV clinical trial investigating the impact of cladribine treatment for MS. Blood and CSF samples from patients were o…

Depression severity and discordance between fatigue patient-reported outcomes in people with multiple sclerosis.

OBJECTIVE: Fatigue and depression are common and disabling symptoms in people with Multiple Sclerosis (PwMS). This study aimed to examine the relationship between discordant fatigue measures and depression severity in PwMS. METHODS: A retrospective analysis was conducted on 712 PwMS evaluated over 14 years at a comprehensive MS center. All participants completed the Fatigue Severity Scale (FSS), Modified Fatigue Impact Scale (MFIS), and Beck Depression Inventory (BDI) as part of routine clinical care. Fatigue score concordance was defined as both FSS and MFIS being above or below the threshol…

Adjunctive Non-Disease-Modifying Therapies in Multiple Sclerosis: Immunometabolic, Neuroprotective and Remyelination-Oriented Approaches.

Disease-modifying therapies (DMTs) are the standard treatment for multiple sclerosis (MS) and effectively reduce inflammatory disease activity; however, their effects on neurodegeneration, remyelination failure, and long-term symptom burden vary across disease stages and patient populations. Adjunctive non-DMT approaches have therefore been investigated to target biological processes not primarily addressed by conventional immunomodulatory treatment. These strategies often involve agents originally developed for non-neurological indications, whose mechanisms of action extend beyond their prima…

Association between choroid plexus enlargement and impaired glymphatic function in relapsing-remitting multiple sclerosis.

BACKGROUND AND OBJECTIVES: Although the glymphatic system and choroid plexus (CP) have been studied in MS, their longitudinal evolution remains poorly understood. Therefore, we aim to characterize glymphatic system function and CP alterations and their longitudinal evolution in relapsing-remitting multiple sclerosis (RRMS), and to examine the relationship with other clinical variables. METHODS: Ninety-two patients with RRMS and forty healthy controls (HCs) were recruited. The diffusion tensor imaging analysis along the perivascular space (DTI-ALPS) index and the normalized volume of the choro…

DNA methylation mediates the multiple sclerosis onset risk associated with HHV-6 DNA positivity.

In Ausimmune, an Australian multicenter incident case-control study, Epstein-Barr virus (EBV)-related measures, including anti-EBNA antibodies and infectious mononucleosis, show multiple sclerosis (MS) associations mediated by DNA methylation (DNAm). Human herpesvirus-6 (HHV-6) DNA has also been linked to increased MS onset risk, though its mechanisms remain unknown. Therefore, we examined an expanded set of human herpesvirus indices including HHV-6 indices. We first tested associations with MS-associated DNAm modules, then assessed whether HHV-6 DNA contributes to MS onset through DNAm pathwa…
