The role of computed tomography in the diagnosis and management of lung cancer. An overview.

Computed tomography (CT) is now established as the principal radiographic adjunct to plain film examination in the diagnosis and management of lung cancer. It should be used in the evaluation of every pulmonary nodule to determine whether the nodule is solitary and whether mediastinal metastases are present and to evaluate the mass by assessing its density. In general, nodules with Hounsfield numbers greater than +175 can be presumed to be calcified and, hence, benign. CT is of great value in determining the extent of lung cancer and at present the best imaging modality for evaluating mediasti

Human chorionic gonadotropin radioantibodies in the radioimmunodetection of cancer and for disclosure of occult metastases.

Radioimmunodetection (RaID) of tumors containing human chorionic gonadotropin (hCG; choriogonadotropin) was evaluated in 25 patients by injecting 131I-labeled goat antibody IgG against hCG and performing total-body photoscans with a gamma scintillation camera 24 and 48 hr later. All 10 testicular cancer patients with proven tumor sites had positive RaID results, whereas three cases without known tumor were negative. Four patients with hydatidiform mole and one with degenerative products of conception showed positive RaID results consistent with elevated serum hCG titers. Two putatively false-p

Classification (staging) of renal parenchymal tumours.

The different classification systems (clinical, microscopic, descriptive) of renal cell tumours provide useful clues to the evaluation and prognosis of these tumours. The prognostic reliability of clinical staging as recommended by UICC is, however, questionable because the clinical parameters are not clearly defined and even the radiological features are of equivocal prognostic significance. Descriptive classification on the basis of the complete surgical specimen is regarded as fundamental to the understanding of the evolution, thus also of the prognosis, of renal cell tumours. Decision tree models combining Bi-parametric vesical imaging reporting and data system and apparent diffusion coefficient metrics for predicting muscle-invasive bladder cancer.

PURPOSE: To compare whole-lesion apparent diffusion coefficient (ADC) histogram analysis with representative ADC value for assessing muscle invasion in bladder cancer, and to build a decision tree model that combines bi-parametric Vesical Imaging Reporting and Data System (bp VI-RADS) with ADC values. METHODS: This retrospective study included 82 patients with bladder cancer who underwent 3T MRI and transurethral resection. The bp VI-RADS was scored using T2-weighted and diffusion-weighted imaging, and ADC maps. For histogram analysis, tumors were segmented on all ADC slices to obtain ADC per

Matrix Metalloproteinases -2 and -9, Vascular Endothelial Growth Factor, Basic Fibroblast Growth Factor and CD105- Micro-Vessel Density are Predictive Markers of Non-Muscle Invasive Bladder Cancer and Muscle Invasive Bladder Cancer Subtypes.

Matrix metalloproteinases (MMPs) are involved in extracellular matrix (ECM) remodeling during embryogenesis, wound healing and tumor development. Aberrant expressions and activation of MMP-2 and MMP-9 are examined as one of the major attributes acquired by tumor neoangiogenic markers including vascular endothelial growth factor (VEGF), basic growth factor (bFGF) and CD105-microvessel density (CD105-MVD) during bladder tumorigenesis. The present study examined the levels of MMP-2, MMP-9, VEGF, bFGF and CD105 to elucidate the relationship among them and associated clinical features in the given

Minimally invasive surgery versus thoracotomy for locally advanced lung cancers: A systematic review and meta-analysis.

BACKGROUND: Locally advanced lung cancer is generally regarded as a relative contraindication for minimally invasive surgical approaches. The comparative outcomes between minimally invasive techniques and conventional open thoracotomy in this specific patient population have not been adequately investigated through rigorous clinical studies. METHODS: We systematically searched Embase, Cochrane Library, PubMed, MEDLINE, and Web of Science for studies comparing minimally invasive (robotic and laparoscopic) approaches with conventional open thoracotomy. Pooled standard mean differences, relative

Pancreatic ductal adenocarcinoma and pancreatic surgery in the 21st century: triumphs, turning points, and unresolved challenges.

Outcomes in pancreatic cancer surgery have been constrained by high operative risks and by the unfavorable biological behavior of most pancreatic neoplasms. This narrative review summarizes the major developments that have reshaped the surgical management of pancreatic ductal adenocarcinoma over the past twenty-five years and highlights their interaction with advances in oncologic therapies. Although no single innovation has been decisively transformative, the overall quality of care has improved substantially. The introduction of objective scoring systems has standardized the classification o

Basement membrane turnover drivesfilopodial protease-independent invasion.

Basement membranes (BMs) are specialized, nanoporous extracellular matrices (ECMs) mainly composed of collagen IV fibres and laminins. Because collagen IV fibres form covalent cross-links (CCLs), cells cannot freely cross BMs. BM breaching marks the transition from in situ to invasive carcinoma, commonly attributed to protease activity. However, recent data show tumour cells crossing BMs through protease-independent mechanisms. Experimental evidence suggests that filopodia can play an active role in pore enlargement in extracellular matrices by generating plastic mechanical deformation. Moreov

How to safely discontinue antifungal treatment in invasive pulmonary aspergillosis?-clinical considerations in haematology.

BACKGROUND: Treatment duration in invasive pulmonary aspergillosis (IPA) in patients with haematological malignancies is not well defined through clinical trials and is therefore often driven more by experience than evidence, with implications for outcome, patient quality-of-life, drug toxicity, costs, and antifungal stewardship. OBJECTIVES: To review current evidence and provide expert considerations regarding cessation of antifungal therapy for IPA, and to propose pragmatic criteria and follow-up strategies to guide safe discontinuation and early detection of recurrence. SOURCES: Narrative

[Minimally invasive techniques for mastectomy].

Minimally invasive endoscopic and robot-assisted mastectomy are emerging alternatives to conventional open mastectomy. Current evidence indicates comparable oncological safety and potential benefits in scar length, cosmetic outcomes, and selected complication rates, although procedures are associated with longer operating time, greater technical complexity, and higher resource demands. For carefully selected patients, this review finds that these techniques may represent a relevant supplementary option in Danish breast surgery.

[Pancreatic cancer in 2025 : multidisciplinary management].

Pancreatic cancer (PC) is an aggressive disease with a poor prognosis, characterized by a median survival of 6 to 8 months and a 5-year survival rate of approximately 15%. In Switzerland, around 1,700 new cases and 1,400 deaths are reported each year. Late diagnosis- largely due to the absence of specific symptoms and validated biomarkers-limits the availability of curative treatment options. This article summarizes recent advances in the multidisciplinary management of PC, detailing the diagnostic workup and therapeutic approaches: imaging, endoscopic techniques, pathology, minimally invasive

Modified pulley maneuver to guide surgical plane in minimally invasive liver resection.

INTRODUCTION: Achieving an adequate surgical plane through optimal traction is crucial for liver parenchymal transection in minimally invasive liver surgery (MILS). MILS is more technically demanding than open liver surgery because of limited instrument mobility and the inability to use the surgeon's hand, potentially leading to iatrogenic injuries. The Pulley maneuver using barbed sutures has been used for laparoscopic hepatectomy; however, the sutures are single-use and may pass through the liver parenchyma, making it uneconomical and inflexible. To address this, we developed a modified pull

[Robot-assisted Minimally Invasive Surgery for Mediastinal Tumors].

The subxiphoid approach in thymectomy provides better visibility around the left brachiocephalic vein than the lateral thoracic approach. Robot-assisted thoracoscopic surgery is easier to parform than video- assisted thoracoscopic surgery for surgery of the upper mediastinum, because the forceps can be moved with many joints. Robot-assisted thymectomy using the subxiphoid approach may be less traumatic and less invasive than median sternotomy. We must continue to devise surgical procedures to make oncologically curative surgery more minimally invasive.
