CAG repeat size correlates to electrophysiological motor and sensory phenotypes in SBMA.

Spinal and bulbar muscular atrophy (SBMA) is an adult-onset, lower motor neuron disease caused by an aberrant elongation of a CAG repeat in the androgen receptor (AR) gene. The main symptoms are weakness and atrophy of bulbar, facial and limb muscles, but sensory disturbances are frequently found in SBMA patients. Motor symptoms have been attributed to the accumulation of mutant AR in the nucleus of lower motor neurons, which is more profound in patients with a longer CAG repeat. We examined nerve conduction properties including F-waves in a total of 106 patients with genetically confirmed SBM

Vestibular-evoked extraocular potentials by air-conducted sound: another clinical test for vestibular function.

OBJECTIVE: The purpose of this study was to identify an appropriate way to detect vestibular-evoked extraocular potentials (oVEMPs) produced by air-conducted sound stimulation in healthy subjects and to apply this test clinically in patients with various vestibular disorders. METHODS: Ten healthy subjects were included in this study. Surface electromyographic (EMG) activity was recorded from active electrodes placed on the face just inferior to each eye. Stimulation with 0.1 ms clicks and 500 Hz short tone bursts was used to activate the vestibular end-organs in healthy subjects. We also test

Long-term depression of a dopamine IPSC.

Two determinants of dopamine release from terminals in striatal and limbic structures are the pattern and rate of dopamine neuron firing in the ventral midbrain. This activity is regulated in part by somatodendritic release of dopamine and subsequent feedback inhibition through activation of D2 receptors on dopamine neuron cell bodies and dendrites. This study describes stimulus-dependent long-term depression (LTD) of IPSCs mediated by dopamine. This LTD was blocked by chelation of postsynaptic intracellular calcium, was dependent on the activation of D2 receptors and was independent of glutam

[Severe Guillain-Barré syndrome and pregnancy: two cases with rapid improvement post-partum].

INTRODUCTION: Guillain-Barré syndrome can occur at any time of pregnancy with the same incidence as in the general population. Observations. We report two cases of patients who developed a progressive ascending paralysis during the second trimester of pregnancy. The worsening of the respiratory insufficiency for one of them required a transfer to an intensive care unit for artificial ventilation lasting 102 days. In the two cases, cerebrospinal fluid examination revealed albumin-cytological dissociation and repeated electrophysiological studies showed typical features of demyelinating neuropat

Anti-ganglioside antibodies and elevated CSF IgG levels in Guillain-Barré syndrome.

Anti-ganglioside antibody production and dysfunction of blood-cerebrospinal fluid (CSF) barrier (BCB) are frequent findings in dysimmune neuropathy patients, whereas intrathecal synthesis of immunoglobulins is still a matter of debate. We examined the CSF, immunological and electrophysiological characteristics from a cohort of patients with Guillain-Barré syndrome (GBS) and chronic inflammatory demyelinating polyneuropathy (CIDP), and from patients with other neurological diseases as control. Thirty-eight percent of GBS patients and 28% of CIDP patients had detectable serum titers of anti-gang

Hypertriglyceridemia and peripheral neuropathy in neurologically asymptomatic patients.

OBJECTIVES: In this study we intended to find a correlation between hypertriglyceridemia and peripheral neuropathy (PN) in patients with a high triglyceride level and no neurological complaints. METHODS: We recruited 24 patients (21 males and 3 females) having a triglyceride level above 300 mg/dl with no neurologic complaints and none of the other common causes of PN and they underwent an electroneurographic study. The distal motor or sensory latencies (DL), motor or sensory conduction velocities (CV), and motor or sensory amplitudes (AMP) were collected for the peroneal, posterior tibial, su

Phenotypic spectrum of disorders associated with glycyl-tRNA synthetase mutations.

We describe clinical, electrophysiological, histopathological and molecular features of a unique disease caused by mutations in the glycyl-tRNA synthetase (GARS) gene. Sixty patients from five multigenerational families have been evaluated. The disease is characterized by adolescent onset of weakness, and atrophy of thenar and first dorsal interosseus muscles progressing to involve foot and peroneal muscles in most but not all cases. Mild to moderate sensory deficits develop in a minority of patients. Neurophysiologically confirmed chronic denervation in distal muscles with reduced compound mo

[Retinaculum flexors: clinical and experimental anatomic bases in favor of reconstruction in carpal tunnel surgery].

The goal of this anatomical study is to prove that reconstruction of the flexor retinaculum in carpal tunnel syndrome surgery is a reliable procedure and can improve the results of this procedure. In the first static study, we noted that when left to heal spontaneously, after 3 months the gap between the two edges of the cut retinaculum remained equal to or even larger than the gap created at the time of the surgery. A special Canaletto implant was created to achieve this reconstruction. It is technically possible to do this reconstruction by a 2 to 3 cm surgical approach in the palm. In anoth

Intraoperative electromyography.

Intraoperative electromyography (EMG) provides useful diagnostic and prognostic information during spine and peripheral nerve surgeries. The basic techniques include free-running EMG, stimulus-triggered EMG, and intraoperative nerve conduction studies. These techniques can be used to monitor nerve roots during spine surgeries, the facial nerve during cerebellopontine angle surgeries, and peripheral nerves during brachial plexus exploration and repair. However, there are a number of technical limitations that can cause false-positive or false-negative results, and these must be recognized and a

Length dependence of variables associated with temporal dispersion in human motor nerves.

Temporal dispersion in motor nerves is associated with changes of amplitude, area, duration, and Fourier spectra of compound muscle action potentials (CMAPs) when comparing responses to proximal and distal stimulation. These changes depend on the length of the nerve segment. To quantitatively assess this dependence, motor conduction studies of nerve segments of various lengths were performed in the median, ulnar, and tibial nerves of 86 test subjects, aged 4 to 73 years. Amplitude, area, duration, and spectral energy above 49 Hz of CMAPs were measured. Values after distal and proximal stimulat

Brachial plexus involvement as the only expression of hereditary neuropathy with liability to pressure palsies.

Hereditary neuropathy with liability to pressure palsies (HNPP) is an autosomal-dominant focal neuropathy characterized by recurrent entrapment neuropathies. Single members of families with HNPP and brachial plexus involvement have been reported previously. We describe a family with three affected members and in which the only symptom of entrapment was recurrent brachial plexopathy. The diagnosis of HNPP in this family was confirmed by both neurophysiological methods and DNA analysis. The distinction between HNPP and hereditary neuralgic amyotrophy is discussed.

[Puffer fish poisoning].

We report three cases of a collective tetrodotoxin poisoning, after ingestion of puffer fish eggs. This neurotoxin is the most potent membrane stabilizer, blocking the nervous conduction and resulting in death from respiratory paralysis in case of massive ingestion. The father died at admission, the mother and her daughter presented an acute respiratory failure and a flaccid tetraplegia, with favourable outcome after 24 hours.

Hypertriglyceridemia may cause a subclinical peripheral neuropathy.

Recently few patients with a painful neuropathy, attributed to extremely high triglyceride levels, were reported. In a prospective study, we evaluated 16 patients with marked hypertriglyceridemia without other causes of neuropathy, using nerve conduction and autonomic function tests. Six subjects (37%) showed mild signs of an asymptomatic motor and/or sensory and/or autonomic axonal polyneuropathy. The study demonstrates, that hypertriglyceridemia may be associated with a mild axonal polyneuropathy, usually subclinical, in significantly more patients than previously considered.

Surgical treatment for patients with cervical flexion myelopathy.

OBJECT: Cervical flexion myelopathy is a rare condition that mainly affects adolescent boys. In recent years, avoidance of neck flexion has been advocated as the treatment for cervical flexion myelopathy, and treatment with a cervical collar and surgery in which fusion of the cervical spine is performed have been found to be effective. However, previously reported series contained only a limited number of patients. The authors report their experience with treating 10 male patients in whom surgery was performed to correct cervical flexion myelopathy, and they evaluate the patients' surgical out

Transcranial magnetic stimulation in patients with cerebellar stroke.

Conduction time of the central motor pathways (CMCT) by transcranial magnetic stimulation (TMS) was performed within the first two weeks in 7 patients with isolated hemicerebellar lesions after stroke. Cerebellar infarcts were small (< 2 cm in diameter) in 5 patients and no brainstem structure was involved in CT studies. The threshold (3 cases) and CMCT (4 cases) were abnormal or asymmetric by stimulation of the motor cortex contralateral to the impaired hemicerebellum. The follow-up study in 2 patients revealed electrophysiological improvement closely related to clinical cerebellar recovery r
