Achieving Hepatitis C Elimination By Using Person-Centered, Nurse-Led Models of Care: A Discussion of Four International Case Studies.

Nurse-led models of care are an important strategy in the management of patients with chronic disease because of the person-centered approach that allows the needs of the individual to be prioritized and addressed in accessible settings. Hepatitis C is caused by a blood-borne virus that can cause liver disease and liver cancer; it predominantly affects marginalized populations, including people who inject drugs. Since 2013, all oral, direct-acting antiviral regimens have been available to cure hepatitis C. Nurses are well placed to be involved in the delivery of hepatitis C testing and treatme

The Beneficence of Cuddle Therapy in Hyperekplexia: A Case Report.

BACKGROUND: Benevolent injustice occurs when well-intentioned treatment efforts produce an outcome that limits the potential of a patient. The unintended harm can result in significant moral distress for the family and the healthcare team. CLINICAL FINDINGS: We discussed an ethical dilemma regarding a neonate who had suspected seizure and hypoxic-ischemic encephalopathy after home birth delivery. The healthcare team experienced moral distress about the mother's desire to not use anti-seizure medications and instead trial other interventions such as cuddling. Subsequently, clinical analysis ru

Opioid-prescribing Outcomes of Medicare Beneficiaries Managed by Nurse Practitioners and Physicians.

BACKGROUND: Primary care providers are at the center of the opioid epidemic. Whether nurse practitioners (NPs) have different opioid-prescribing outcomes from physicians is not known. OBJECTIVE: To examine opioid-prescribing outcomes of Medicare beneficiaries receiving care from NPs and physicians in primary care. RESEARCH DESIGN: We used Medicare data from 2009 to 2013 and a propensity score-weighted analysis. SUBJECTS: Beneficiaries residing in states in which NPs are able to prescribe controlled substances without physician oversight and who did not have a cancer diagnosis, hospice care,

A conceptual framework for increasing clinical staff member involvement in general practice: a proposed strategy to improve the management of low back pain.

BACKGROUND: Low back pain affects about 80% of all adults, many of whom consult general practice. Providing management can be challenging, in part due to the scarcity of effective treatment methods. There is broad consensus in international clinical practice guidelines to provide patients with information about the nature of their pain and recommend them to stay active despite discomfort. Delivering this information is time-demanding and challenged by the limited available resources in general practice in many countries. Furthermore, general practice settings are highly variable in size and in

School Nurses on the Front Lines of Medicine: Emergencies Associated With Sport and Physical Activities: Part 1.

Illness and injury associated with sport and physical activities may occur in the school setting. Although most sport-related illness and injury in students are considered minor emergencies, life- and limb-threatening illnesses or injuries may occur, such as sudden cardiac arrest, heat stroke, status asthmaticus, catastrophic brain or cervical spine injuries, hypoglycemia, blunt chest/abdominal injuries, or extremity fractures requiring surgery. It is important for the school nurse to recognize potential life- and limb-threatening emergencies associated with sport and physical activity, to ini

Wrong site surgery: A critical incident analysis of a near miss.

This article is a reflective account of a near miss that happened in clinical practice during a particularly busy operating list. This critical incident highlights issues that can arise from a breakdown of communication between members of a multidisciplinary team and demonstrates how ineffective teamwork can jeopardise a patient's safety and wellbeing. It emphasises the effects of human factors on professional performance and how they can contribute to mistakes and misconducts. It also stresses the importance of reporting patient safety incidents so that the lessons can be learned and future p

Standardising haemodialysis care by restricting nutrition during dialysis: introducing a quality improvement initiative for renal outpatients.

A number of relevant issues are considered which show that it is essential to address the issue of in-centre meals during dialysis. This discussion paper critically explores the potential complications posed to patients who consume a large calorific intake during their dialysis treatment. The mission is to appeal to more dialysis units and outpatient departments to gradually implement a 'no food' policy during regular scheduled dialysis treatment sessions. The authors aim to put forward the significances and challenges and offer some possible solutions when introducing a 'no eating policy' lik

Informed consent among nursing students participating in biomedical research.

For consent in biomedical research, it is essential that research participants understand the need for research, the study protocol, the risk and benefits of participation, the freedom to participate or decline and the right to leave the study at any time. A structured questionnaire was used to assess understanding and knowledge among nursing trainees participating in a cohort study investigating exposure and latent tuberculosis at a tertiary care hospital. Data were collected for 138 participants. While 97% were aware of their enrollment into a research protocol, only 78% could state that it

French national wound management survey: choice criteria of dressings.

Across Europe, wound care management is organized differently, and in some countries such as the UK or Denmark, wound healing centres have been implemented. In France, a large number of health professionals are not sufficiently educated in wound care management during their vocational training. The rapid evolution of dressings has changed wound management practices and has given rise to new professional recommendations. This national survey was carried out in France in 2009, including 465 health professionals, to determine the criteria they use to choose a dressing and their habits of care wit

Antipsychotics and the risk of death in the elderly: an instrumental variable analysis using two preference based instruments.

PURPOSE: Observational studies have investigated the comparative safety of antipsychotics with varying results. Instrumental variable analysis has been suggested as a possible alternative to conventional analyses when there is concern about the effect of unmeasured confounding in observational studies. Using the example of the risk of death with typical compared to atypical antipsychotics, we aimed to explore the performance of two different instruments. We used the doctor prescribing preference instrument, which has been used in previous studies, to investigate further the assumptions of this

Parents' childhood fever management: community survey and instrument development.

AIM: This paper is a report of a study to explore Australian parents' knowledge, beliefs, practices and information sources about fever management and develop a scale to measure parents' fever management practices. BACKGROUND: Parental fever phobia and overuse of antipyretics to reduce fever continue. No scales to measure parents' fever management practices are available. METHOD: A community-based, postal survey was carried out in 2005 with 401 Australian parents of well children aged 6 months-5 years. Respondents were recruited through advertising (48.4%), face-to-face (26.4%) and snowball

Managing streptococcal pharyngitis: a review of clinical decision-managing strategies, diagnostic evaluation, and treatment.

Prompt and accurate diagnosis of GABHS pharyngitis is critical to prevent ARF, tonsillar abscess, toxic-shock-like syndrome, bacteremia, and deep tissue cellulitis which can result in significant morbidity and death. Antibiotic treatment of all patients that have complaints of sore throat incurs unnecessary costs and potential drug resistance. On the other hand, culturing all patients with complaints of sore throat incurs unnecessary costs and false positive cultures in patients. Utilizing a sensitive and specific clinical guideline assists clinicians in deciding who to culture or screen, and

Impact of interprofessional blended learning with low-fidelity simulation on neonatal resuscitation skills among interns: a quasi-experimental study in Tanzania.

BACKGROUND: Despite the national rollout of the Helping Babies Breathe (HBB) program, neonatal mortality due to birth asphyxia remains a major challenge in Tanzania. Evidence shows that there is inadequate competence (including knowledge, skills, and attitudes) in newborn resuscitation among medical and nursing interns as they transition into practice. The traditional method of a one-day instructor-led HBB model provides little hands-on practice. Integrating low-fidelity simulation with blended learning may contribute to better transfer of learning and offer a more sustainable model for skills
