The Use of "Spare Parts" En Bloc Anterior Compartment Myocutaneous Free Flap to Reconstruct the Transmetatarsal Amputation Stump After Contralateral Below Knee Amputation: Report of Two Cases.

The aim of reconstruction after lower extremity amputation is to provide adequate soft tissue coverage that is compatible with prosthetics to optimize functional status. We present two cases where the anterior compartment myocutaneous free flap used in a "spare parts" fashion was valuable in preserving the length of residual limbs for patients needing simultaneous below-knee amputation (BKA) and contralateral transmetatarsal amputation (TMA). The first case involved a 48-year-old woman undergoing TMA and BKA to address bilateral lower extremity necrosis secondary to septic shock. The anterior

Do Pharyngeal Flaps Have a Greater Risk of Obstructive Sleep Apnea Than Other Speech Procedures? A Systematic Review and Meta-Analysis.

BACKGROUND: Velopharyngeal insufficiency (VPI) is common in patients with repaired cleft palate. Speech surgery, which encompasses operative procedures to improve velopharyngeal function and speech resonance, is associated with postoperative obstructive sleep apnea (OSA). However, the relative risk of OSA between different speech surgery procedures remains unclear. PURPOSE: The purpose of this study was to compare the relative risk of OSA following pharyngeal flap versus nonpharyngeal flap procedures, which included dynamic sphincter pharyngoplasty (DSP), Furlow palatoplasty, and buccal myomu

Influence of Cupid's Bow Peak Placement on Nasolabial Formation in Unilateral Cleft Lip Repair.

ObjectiveTo clarify how the preoperatively determined position of the Cupid's bow peak on the lateral lip element affects postoperative nasolabial formation in patients with unilateral cleft lip. We hypothesize that setting the proposed peak of the Cupid's bow close to the nostril floor causes the cleft-side nasal alar base to droop while positioning it near the oral commissure shortens the red lip, causing an elevated cleft-side oral commissure.DesignRetrospective study.SettingSingle institution.PatientsTwenty-seven patients with non-syndromic complete unilateral cleft lip and palate (UCLP) w

The Use of ECMO and Free-Fillet-Leg Flap for Complex Pelvic Reconstruction: A Case Report.

Advanced sarcoma treatment in complex anatomical regions such as the pelvis poses significant surgical challenges. This report details a case involving a 35-year-old man with recurrent osteosarcoma of the left hemipelvis, who underwent a 16 h surgery for hemipelvectomy and reconstruction using a free tibia and fibula fillet leg flap. The procedure, necessitated by an infected, exposed iliac prosthesis, utilized extracorporeal membrane oxygenation (ECMO) for 8 h to maintain flap viability. The flap, incorporating tibia, fibula, and associated musculature was successfully inset and anastomosed t

Subtotal Thigh Flap for Large Abdominal Wall Defect Reconstruction in Patient With Recurrent Dermatofibrosarcoma Protuberans: A Case Report and Literature Review.

BACKGROUND: Harvesting an adequate-sized flap is challenging for reconstructing large defects on the abdominal wall. A subtotal thigh flap would be one of the choices as it provides a well-vascularized large flap with muscle components. Moreover, dermatofibrosarcoma protuberans (DFSP) is a low-grade dermal neoplasm with a high recurrence rate. There is still no consensus on the extent of resection to prevent a recurrence. OBJECTIVES: We present a case of a patient who underwent the reconstruction of a large abdominal wall defect with a subtotal thigh flap after the resection of recurrent DFSP

Combination of secondary intention healing and primary closure to reconstruct large facial defects.

Secondary intention healing has been a well-established method to close wounds for more than 200 years. Indeed, it represents the easiest technique in the ladder of plastic reconstruction. Primary wound closure (side-to-side closure, direct wound closure) is the second easiest method. The combination of these two techniques is already an integral aspect of specific surgical procedures, e.g. the reconstruction of the donor site of a paramedian forehead flap. This minireview will show that the combination is also a suitable alternative to classic flaps in reconstruction of different aesthetic su

Anesthetic management of a patient with closed tracheal transection.

Closed tracheal injury is a rare clinical emergency that is prone to misdiagnosis and missed diagnosis. Improper management can be life-threatening. This report describes the anesthetic management of a patient with closed tracheal transection undergoing airway reconstruction surgery. The patient, a 33-year-old male, presented with head and neck pain and dyspnea 4 hours after a car accident. CT with 3-dimensional airway reconstruction revealed upper tracheal transection. Emergency cervical exploration and end-to-end tracheal anastomosis were performed under general anesthesia. The anesthetic ap

Considerations for Selecting Root Coverage Techniques in the Anterior Mandible.

The anterior mandible is the most challenging anatomical site for performing periodontal plastic surgeries. Increased demands for optimal root coverage and esthetic outcomes contribute to the development of pedicle flap-based surgical solutions, in contrast to the predominantly used free gingival graft. The aims of this study were to (1) summarize the current literature to identify the mostused techniques, indications, and their efficacy, and (2) provide a decision table for surgeons to navigate through the selection of appropriate techniques. Four main approaches were identified: free gingiva

Reconstruction of extensive lower lip defects using vascularized mucosal flap with external skin complex tissue.

BACKGROUND: The complexity of lip anatomy and the roles played by the lip make the reconstruction of lip defects more challenging. Adequate reconstruction of lip defects requires adaptation of mucosa, vermilion, and skin features in lip as well as its specific function. METHODS: A 59-year-old male with left lower lip cancer underwent en-bloc resection and left selective neck dissection (SND), followed by immediate reconstruction using Facial Artery Myomucosal Island Flap (FAMMIF) with external Skin Complex Tissue. RESULTS: The use of chimeric flap based on FAMMIF and its external skin tissue

Clinical application of V-shaped folded submental flap in the repair of oral defect.

OBJECTIVES: The repair of small and medium-sized defects in the oral has always been a challenge, free skin flap and distal pedicled tissue flaps are difficult to meet clinical needs, and the traditional under-chin flap has the risk of donor-area injury. This study aims to investigate the efficacy of V-shaped folded submental flap in the repair of small-sized and medium-sized oral defects. METHODS: The clinical data of 28 patients with oral defect lesions, who were hospitalized in the Department of Stomatology, Third Xiangya Hospital of Central South University from March 2019 to December 202

[Fillet flap transfer as alternative to conventional lower limb amputation].

BACKGROUND: The fitting of a prosthesis after lower limb amputation is associated with several challenges. Skeletal stump-prosthesis interfaces and selective nerve transfer can partially overcome these but are also associated with new impairments that emphasize the necessity of innovative approaches. The concept of so-called spare part surgery with the use of fillet flaps could play an important role in this respect. OBJECTIVE: An overview of the classical prosthesis-associated discomforts, advantages and disadvantages of treatment strategies and presentation of alternative surgical concepts.

Use of De-Epithalialized Flap for Elevating the Alar Base in Patients With Cleft Lip.

Alar base asymmetry is a common problem in cleft lip patients. Caused by a lack of bone support and soft tissue deficiency, it is one of the problems that must be corrected during cleft lip repair. While it is encountered during primary lip repair, it can also be seen as a secondary cleft lip nasal deformity after repair. This study aimed to provide alar base symmetry with a de-epithelialized muscle-skin flap during the primary cleft lip repair of incomplete cleft lip patients and secondary cleft lip deformity repair. In incomplete cleft lip patients, the tissue under the incomplete part is de
