Amputation alternatives in the lower limb, stressing combined management of the traumatized extremity.

A methods approach for the care of the traumatic lower extremity amputee is presented, emphasizing joint surgical approach, specifics of management, ideal amputation levels, and functional rehabilitation. The role of the plastic surgeon with an interest in free flaps is ever increasing and has elevated the level and quality of traumatic extremity amputation. Therefore, an understanding by the plastic surgeon of the orthopedic, prosthetic, functional, and rehabilitation principles becomes as important as the orthopedic surgeon's appreciation of current microvascular reconstruction potential. Short-term responses across two sequential plant-based activity sessions: an exploratory fixed-order pilot study.

BACKGROUND: Structured plant-based activities combine natural materials, purposeful manual work, sensory engagement, and social interaction, but direct comparisons of practical formats in community adults remain limited. We characterized short-term responses during two sequential sessions and compared within-session changes. METHODS: Twenty-five adults participated in a single-group, fixed-order pilot study. Session 1 involved horticultural crafting; 1 week later, Session 2 involved balm crafting followed by horticultural crafting. Twenty-three attended both sessions. DASS-21 and STAI-S were

BACKGROUND: Mental health—defined as a state of well-being enabling people to realise their potential, cope with daily stress, perform productive work, and contribute positively to their communities—is still perceived by the majority of people in low-income countries, including primary school teachers, as mental ill-health. This study aimed to understand the perceptions and explanatory factors of primary school teachers’ commitment to promoting mental health in schools in the urban Democratic Republic of Congo (DRC). METHOD: This qualitative study was conducted in the urban DRC, especially in

Exploring challenges and opportunities in paediatric bereavement care: a qualitative study from a tertiary care hospital in Pakistan.

BACKGROUND: In Pakistan, most children with life-threatening illnesses present at advanced stages and succumb to death, leaving families emotionally traumatised. Parental bereavement leads to long-term psychosocial and financial hardships for families, thus highlighting the need for bereavement care as a crucial part of paediatric palliative care. This study explores key challenges around lack of bereavement care in the healthcare infrastructure in Pakistan and identifies opportunities to establish this programme for grieving families. METHODOLOGY: This study presents a secondary qualitative a

A Multimodal Lifestyle and Mind-Body Intervention in Rural Primary Care: Implications for Metaflammation in an Appalachian Population.

BACKGROUND: Chronic metabolic diseases such as obesity, hypertension, type 2 diabetes, and mood disorders frequently cluster in under-resourced communities. Increasing evidence suggests these conditions may be influenced by persistent low-grade systemic inflammation (sometimes referred to as metaflammation), as well as cumulative physiologic stress and environmental constraints. Rural Appalachian populations experience a disproportionate burden of these conditions, often compounded by poverty, food deserts, limited healthcare access, and restricted availability of preventive services. OBJECTI

Development and Validation of a Nomogram to Identify and Predict High-Distress Psychosocial Phenotypes in Tinnitus Patients: A Latent Profile Analysis.

OBJECTIVE: Evidence is limited on the multidimensional psychosocial heterogeneity among patients with subjective tinnitus, and practical tools for early risk stratification remain largely unexplored. This study aimed to identify distinct psychosocial phenotypes under the biopsychosocial model to pinpoint the high-risk population and to develop and validate a visualized nomogram predicting severe psychological distress for targeted interventions. METHODS: A total of 534 subjective tinnitus patients were enrolled. Latent profile analysis using seven psychosocial indicators (Self-Rating Anxiety

Effectiveness of yoga therapy as an adjunct on mental health status, quality of life, and medication adherence among people living with HIV on antiretroviral therapy: A study protocol of a randomized controlled trial (ART YOGA).

INTRODUCTION: Human Immunodeficiency Virus, the retrovirus that causes Acquired Immune Deficiency Syndrome, is a major global public health threat. This chronic viral infection diminishes the immune system by attacking CD4 cells. The principal treatment is antiretroviral medication (ART), which significantly increases the life expectancy of HIV patients. However, ART does not address psychological issues, including depression, anxiety, and stress. Psychosocial factors are known to influence HIV disease progression through activation of stress-related biological pathways, including the hypothal

Resilience in Stroke survivor-care-partner Dyads (ReStoreD): a study protocol for a randomized-control trial.

BACKGROUND: The impact of stroke is shared between the stroke survivor and their care-partner. An estimated 30-50% of stroke survivors and their care-partners experience depression or anxiety that negatively affects rehabilitation outcomes and quality of life. Yet, interventions to support couples post-stroke are largely insufficient or inaccessible. A novel remotely delivered dyadic intervention to promote Resilience in Stroke survivor-care-partner Dyads (ReStoreD) was developed to address this need. The intervention includes psychoeducation and weekly modules that help couples learn and prac

The REDUCE Intervention: The Development of a Person-Centred Cognitive Behavioural Intervention to Improve Ulcer Outcomes in People at Risk of Diabetic Foot Ulceration.

INTRODUCTION: Diabetic foot ulcers (DFUs) affect approximately one-quarter of people living with diabetes. They are chronic, recur frequently and are associated with significant psychological distress and behavioural challenges. The REDUCE intervention is a person-centred, cognitive behavioural intervention designed to reduce the risk of DFU recurrence and support ulcer healing. Here, we describe the iterative development and optimisation of REDUCE, from its inception as a group-based intervention to an individually tailored intervention delivered via video call or telephone. We outline key st

Identity development and adaptation in adolescents with genetic conditions: a qualitatively oriented mixed-methods study to develop strategies for optimizing clinical genetics services.

BACKGROUND: Genetic counselling for adolescents necessitates an approach distinct from that used with adults. Developing best practices is crucial, considering the growing number of disabled adolescents worldwide and increasing use of genomic testing early in life. We investigated perceptions of adolescents (10-19 years) who had been diagnosed with a genetic disorder in terms of how they describe receiving, understanding, and living with a genetic condition. We undertook a cross-sectional, qualitatively oriented mixed methods study underpinned by the pragmatic paradigm. Adolescents completed t

Enhancing questioning skills through child avatar chatbot training with feedback.

Training child investigative interviewing skills is a specialized task. Those being trained need opportunities to practice their skills in realistic settings and receive immediate feedback. A key step in ensuring the availability of such opportunities is to develop a dynamic, conversational avatar, using artificial intelligence (AI) technology that can provide implicit and explicit feedback to trainees. In the iterative process, use of a chatbot avatar to test the language and conversation model is crucial. The model is fine-tuned with interview data and realistic scenarios. This study used a
