Integrated bioinformatics and machine learning identify early diagnostic biomarkers for MAFLD with comorbid psoriasis.

BACKGROUND: Psoriasis and metabolic dysfunction-associated fatty liver disease (MAFLD) share pathological features such as chronic inflammation, immune dysregulation, and metabolic disturbance. Increasing evidence suggests biological crosstalk between the two conditions, offering new insights into their shared mechanisms and comanagement. Early-stage MAFLD, characterized by hepatic steatosis without evident inflammation or fibrosis, provides a crucial window for intervention. This study aimed to identify early diagnostic biomarkers linking psoriasis and MAFLD. METHODS: Transcriptomic datasets…

Case Report: Upadacitinib and apremilast combination therapy for guselkumab-induced paradoxical eczema in psoriasis.

Psoriasis is an immune-mediated inflammatory skin disease. Although biologics have markedly improved psoriasis symptoms, they may occasionally trigger paradoxical inflammatory reactions, particularly with IL-17A or TNF-α inhibitors. However, paradoxical inflammatory reactions associated with the IL-23 inhibitor guselkumab is exceptionally rare. We describe the case of a 70-year-old male with a 5-year history of psoriasis who had previously received tofacitinib for one year without achieving complete clearance. Following the initiation of guselkumab, the patient developed paradoxical eczema. Su…

Psychodermatologic Dimensions of Psoriasis: Perceived Burden, Help-Seeking Behavior and Openness to Supportive Care.

Background/Objectives: Psoriasis is a chronic immune-mediated dermatosis with an important psychodermatologic burden, in which psychological stress, perceived disease impact, stigma, and attitudes toward mental health care may influence patient support needs. Although psychological and psychiatric interventions are increasingly discussed in dermatology, patient openness to different forms of supportive care, including music therapy, remains insufficiently characterized. This study aimed to characterize psychodermatologic burden and support-related attitudes among patients with psoriasis, with …

Two-dimensional GeTe nanosheets for psoriasis through modulation of macrophage activation and psoriatic inflammation.

Psoriasis is a chronic inflammatory disease characterized by thickened erythematous skin lesions covered with white and silvery scales and accompanied by macrophage infiltration into the dermis. Although 2-3% of the world's population suffers from psoriasis, there is still a requirement for novel, safe, and effective treatment options. Previously, our group demonstrated the theranostic effects of two-dimensional germanium telluride nanosheets (GeTe-NSs) in the treatment of inflammatory bowel disease. However, the precise mechanisms underlying their therapeutic action remained unclear. In this …

Causal relationships between psoriasis and coronary artery disease: A two-sample Mendelian randomization study.

Previous studies have revealed a potential association between psoriasis and coronary artery disease (CAD). However, the causal relationship between the 2 remains unclear. This study aims to assess the causal link between psoriasis and CAD, which encompasses coronary heart disease, myocardial infarction, and angina pectoris. After obtaining genome-wide association studies data on psoriasis and CAD, we selected appropriate single nucleotide polymorphisms for Mendelian randomization (MR) analysis. The inverse-variance weighted method was used as the main analytical method. The inverse-variance w…

Dissolving microneedle delivery of anemoside B4: a strategy to enhance therapeutic efficacy against psoriasis.

Psoriasis, a chronic immune-mediated skin disease, lacks satisfactory treatment options due to the inherent limitations of conventional drug delivery: oral therapies often induce systemic toxicity, while topical agents fail to penetrate the thickened skin barrier. Anemoside B4 (AB4), a unique triterpenoidal saponin from Pulsatilla chinensis (content up to 9.7%), has previously shown anti-inflammatory efficacy comparable to dexamethasone. However, its unfavorable physicochemical properties severely hinder oral bioavailability and skin permeation, rendering traditional administration routes inef…

Shikonin-Loaded ROS-Responsive Microneedles for Psoriasis Therapy: Formulation, Transdermal Delivery, and Mechanistic Evaluation.

Background/Objectives: Shikonin (SKN) is a potential anti-psoriatic agent, yet its clinical application is hindered by poor water solubility and low stratum corneum permeability. This study aimed to develop a reactive oxygen species (ROS)-responsive hydrogel microneedle system encapsulating SKN-loaded polymeric micelles (SKN-M@MN) to enhance transdermal delivery and evaluate its therapeutic effects in psoriasis. Methods: Shikonin-loaded micelles (SKN-M) were optimised using a thin-film hydration method. SKN-M@MN was fabricated via a two-step casting method using phenylboronic acid-modified hya…

Psychosexual burden in adult genital psoriasis: outcome measurement, construct coverage, and clinical recognition gaps.

Adult genital psoriasis is associated with local symptoms and effects on sexual activity, intimate relationships, body exposure, and patient communication. The associated psychosexual burden is not adequately summarized by lesion severity or sexual function scores alone. This integrative review examines outcome measurement, construct coverage, and clinical recognition of psychosexual burden in adult genital psoriasis. Existing measurement tools and related measurement approaches capture several relevant components, including genital-site severity, local symptoms, sexual activity-related impact…

Intravital Monitoring of Psoriasis Treatment Response and Drug Delivery Using Multiphoton Fluorescence Lifetime Imaging.

BACKGROUND: Multiphoton tomography equipped with fluorescence lifetime imaging (MPT-FLIM) is a novel noninvasive imaging technique for analyzing morphological and metabolic states of skin diseases at a subcellular resolution. The present study is the first to establish MPT-FLIM as an imaging modality to monitor treatment response in psoriasis during topical anti-inflammatory therapy. MATERIALS AND METHODS: Patients with psoriasis treated with topical calcipotriol/ betamethasone dipropionate (C/B) or a calcipotriol derivative (C/-) formulation were recruited and monitored using MPT-FLIM. Imagi…

Determining the Association Between Clear Cell Acanthoma and Psoriasis: A Retrospective Case-Control Study.

Given clear cell acanthoma's overlapping similarities with psoriasis, we compared the presence of psoriasis and its known cardiovascular comorbidities in patients with clear cell acanthoma with patients with a control benign skin lesion (seborrheic keratosis). We found no significant relationship between clear cell acanthoma and psoriasis.

PD-1 engagement and circulating Th17 effector activity during secukinumab treatment in plaque psoriasis.

Plaque psoriasis (PP) is a chronic immune-mediated inflammatory disease driven by dysregulation of the IL-23/Th17/IL-17 axis. Although IL-17A-targeted biologics achieve robust clinical efficacy, the systemic immune remodeling accompanying successful treatment remains incompletely understood. We performed longitudinal immune profiling in seven patients with PP before treatment and after achieving a 90% reduction in the Psoriasis Area and Severity Index (PASI 90) following secukinumab therapy. Circulating T-cell subsets, programmed cell death protein 1 (PD-1)-expressing populations, cutaneous ly…

Xiegan Liangxue Jiedu Decoction ameliorates psychiatric comorbidities in psoriasis via regulation of sphingolipid metabolism.

ETHNOPHARMACOLOGICAL RELEVANCE: Xiegan-Liangxue-Jiedu decoction (XLJD) is an ethnopharmacological empirical prescription inherited from Gu's Surgery, a national intangible cultural heritage of Chinese ethnomedicine. Developed based on the classic TCM theory of Liver governing emotion, XLJD has been clinically used for centuries to treat psoriasis (PsO) via the principle of the TCM principle of "treating PsO from the Liver perspective", and has shown favorable efficacy in alleviating psychiatric comorbidities in psoriasis (PCP) by soothing the Liver and relieving emotional discomfort. AIM OF T…

Macrophage-Driven Bidirectional Exacerbation in Psoriasis-Atherosclerosis Comorbidity: Insights From a Novel Mouse Model.

BACKGROUND: Psoriasis (PSO) and atherosclerosis (AS) are chronic inflammatory diseases that frequently coexist, with each condition exacerbating the other. However, the underlying mechanisms remain poorly understood, partly due to the lack of a suitable animal model that recapitulates this bidirectional comorbidity. METHODS: PSO-AS mouse models were constructed by combining high-fat diet (HFD)-induced AS with imiquimod (IMQ)-triggered psoriatic inflammation in ApoE-/- mice. PSO severity was assessed via skin manifestation, hematoxylin and eosin (H&E) staining, PSO area and severity index (PAS…

Huaiqihuang Alleviates Imiquimod-Induced Psoriasis-Like Skin Lesions and Depressive-Like Behaviors in Mice.

OBJECTIVE: Depression is a significant comorbidity of psoriasis, prompting an exploration of the potential use of traditional Chinese medicine for the concurrent treatment of both conditions. This study aimed to investigate the effects of Huaiqihuang (HQH) on psoriasis and its comorbid depressive-like behavior in a murine model. METHODS: We utilized an imiquimod (IMQ)-induced psoriasis-like model to mimic psoriasis and concurrent depression. The mice received oral HQH extract, and subsequent assessments were performed to monitor changes in skin lesions, inflammatory cells, and depressive beha…

RSV infection-associated pediatric generalized pustular psoriasis with innate immune activation: a case report.

Generalized pustular psoriasis (GPP) in infancy is uncommon and is often precipitated by infection, drugs, or systemic inflammatory responses. Although viral illnesses such as influenza have been reported to trigger GPP, acute exacerbation secondary to respiratory syncytial virus (RSV) is rarely described. We report a case of a 2-year-old girl with a prior history of mild psoriasis who developed widespread sterile, small pustules several days after onset of RSV infection symptoms (cough and low-grade fever), consistent with an RSV-associated acute exacerbation of GPP. Laboratory testing was po…
