Estimation of the planning organ at risk volume for the lenses during radiation therapy for nasal cavity and paranasal sinus cancer.

INTRODUCTION: To establish the optimal planning risk volume (PRV) of the lenses for patients who are receiving radiotherapy to the nasal cavity and paranasal sinus (NCPSC). METHODS: Geometrical uncertainties of the lenses were evaluated for a prospective group of patients (G1). Differences between planned and delivered maximum doses to the lenses were evaluated for every fraction. The relationship between dose differences and geometrical uncertainties were analysed to establish an optimal PRV for the lenses. Obtained results were verified in the second group (G2) of patients for whom calculat

A self-sustaining repair circuit drives the progression of pulmonary fibrosis.

Pulmonary fibrosis is a progressive and often fatal lung disorder whose clinical behavior is not fully explained by conventional injury-driven models. Fibrosis frequently progresses after the initiating insult has resolved, anti-inflammatory therapies have shown limited benefit in idiopathic pulmonary fibrosis, and current antifibrotic drugs slow but rarely reverse disease progression. Moreover, diverse conditions including idiopathic pulmonary fibrosis, post-ARDS or post-infectious fibrosis, radiation-induced injury, drug toxicity, and bronchopulmonary dysplasia arise from distinct causes yet

The Value of Platelet-to-Lymphocyte Ratio (PLR) in Identifying Intracranial Injury in Patients with Mild Head Trauma: A Prospective Study.

Background: Head trauma is a major public health concern. Computed tomography (CT) is frequently used to evaluate these patients but may expose them to unnecessary radiation exposure. Various biomarkers have been investigated to predict prognosis and reduce the need for unnecessary imaging. Red cell distribution width (RDW), neutrophil/lymphocyte ratio (NLR), and platelet/lymphocyte ratio (PLR) have been proposed as inflammatory markers; however, their diagnostic value in head trauma remains controversial. This study aimed to determine the value of complete blood count parameters in identifyin

Spontaneous Splenic Laceration in Plasmodium falciparum Malaria: A Case Report.

INTRODUCTION: Splenic complications of Plasmodium falciparum malaria are rare but potentially life-threatening. While splenic rupture is more commonly reported, high-grade splenic injury with associated vascular compromise and infarction is less well-described and may present diagnostic and management challenges. CASE PRESENTATION: A 32-year-old male presented with a three-day history of high-grade fever, chills, and progressive abdominal symptoms, including acute left upper quadrant pain radiating to the back. On examination, he was febrile with signs of peritonism, including involuntary gua

Molecular and Regenerative Effects of Platelet-Rich Plasma and Related Hemocomponents in Animal Models of Liver Injury-A Systematic Review.

Platelet-rich plasma (PRP) has been increasingly explored as a biologic strategy for liver repair; however, preclinical studies have evaluated not only intact PRP but also PRP related hemocomponents with distinct biological properties, complicating interpretation and translation of the evidence. A systematic review of experimental studies was conducted to assess the effects of PRP and related hemocomponents in animal models of liver injury, focusing on molecular, metabolic, biochemical, and histological outcomes, and evaluating methodological quality and risk of bias using the Cochrane ROB 2.0

Memory and anatomical change in severe non missile traumatic brain injury: ∼1 vs. ∼8 years follow-up.

In previous studies, we investigated a group of subjects who had suffered from a severe non missile traumatic brain injury (nmTBI) without macroscopic focal lesions and we found brain atrophy involving the hippocampus, fornix, corpus callosum, optic chiasm, and optic radiations. Memory test scores correlated mainly with fornix volumes [37,38]. In the present study, we re-examined 11 of these nmTBI subjects approximately 8 yr later. High-spatial resolution T1 weighted magnetic resonance images of the brain (1mm(3)) and standardised memory tests were performed once more in order to compare brain

Brachytherapy as part of the definitive management of squamous cancer of the base of tongue.

Between 1981 and 1986, 17 patients were treated at the Department of Radiation Oncology at the Memorial Sloan-Kettering Cancer Center with squamous cancer of the base of the tongue whose definitive treatment included brachytherapy. The patient sample consisted of 14 men and 3 women with age range of 35 to 71 years (median = 58). There were four patients with T1 lesions, six with T2, six with T3, and one with T4. In general, treatment consisted of 5000-5400 cGy with external beam radiation and 2000-3000 cGy boost to the base of tongue via an Ir-192 implant using afterloading catheters. Necks we

Flow Cytometry and AI-Driven Biomarker Discovery in Acute Kidney Injury: From Mechanistic Insights to Translation Applications in Clinical and Austere Environments.

Acute kidney injury (AKI) is a complex and multifactorial syndrome characterized by the rapid decline of renal function following ischemic, septic, traumatic, hypoxic, or nephrotoxic insults. Despite advances in critical care, exacerbation of AKI is associated with substantial morbidity, mortality, progression to chronic kidney disease (CKD) and multiorgan dysfunction. Current therapies remain largely supportive and do not directly target the molecular pathways driving AKI, underscoring the need for mechanism-based diagnostics and precision-guided therapeutic strategies. Accumulating evidence

Fatal complication from Foley's catheter: a case report.

INTRODUCTION AND IMPORTANCE: Foley's catheterization is a common and routinely performed procedure, yet it carries risks including urinary tract infection, urethral trauma, and, more rarely, bladder perforation. Reports of simultaneous bladder and bowel injury during catheter exchange are exceedingly uncommon. CASE PRESENTATION: We report an 86-year-old woman with hypertension, heart failure, and a history of uterine cancer treated with chemotherapy and pelvic radiotherapy 2 decades earlier. After a routine Foley's catheter exchange, she developed paraumbilical pain and decreased urine output

The radioprotective effect of olanzapine on intestinal injury via modulation of oxidative stress, NF-κB, and caspase-3 expression.

Irradiation (IR)-induced intestinal injury is a common complication in abdominal and pelvic radiotherapy in neoplastic diseases. It has been found that olanzapine (OLZ), an atypical antipsychotic medication, exhibits antioxidant and anti-inflammatory, and anti-apoptotic properties at low doses. The present study aimed to investigate the radioprotective effects of OLZ against acute intestinal injury of irradiation. In this experimental study, sixty adult male BALB/c mice (30–35 g) were split into six groups (10/group): Control, 10 mg/kg of OLZ (OLZ 10), 20 mg/kg of OLZ (OLZ 20) for 10 consecuti

Statistical and machine learning approaches to predict the necessity for computed tomography in children with mild traumatic brain injury.

BACKGROUND: Minor head trauma in children is a common reason for emergency department visits, but the risk of traumatic brain injury (TBI) in those children is very low. Therefore, physicians should consider the indication for computed tomography (CT) to avoid unnecessary radiation exposure to children. The purpose of this study was to statistically assess the differences between control and mild TBI (mTBI). In addition, we also investigate the feasibility of machine learning (ML) to predict the necessity of CT scans in children with mTBI. METHODS AND FINDINGS: The study enrolled 1100 childre

COVID-19: a viewpoint from hepatic perspective.

Coronaviruses as such are known since last century. The name is derived from their shape which has crown (corona) like radiating spikes. The recent one however is a different one from the Coronavirus involved in SARS (2002-2004) and MERS (2012) in being highly infectious. Initially COVID 19 had a high case fatality rate which has now decreased to a significant extent. Many cases of COVID 19 are asymptomatic with a significant number of positive cases developing a triad of fever, breathlessness and GI symptoms. Recent travel increases the probability of infection. The pathogenesis involves ACE

Cardiac MRI to Visualize Myocardial Damage after ST-Segment Elevation Myocardial Infarction: A Review of Its Histologic Validation.

Cardiac MRI is a noninvasive diagnostic tool using nonionizing radiation that is widely used in patients with ST-segment elevation myocardial infarction (STEMI). Cardiac MRI depicts different prognosticating components of myocardial damage such as edema, intramyocardial hemorrhage (IMH), microvascular obstruction (MVO), and fibrosis. But how do cardiac MRI findings correlate to histologic findings? Shortly after STEMI, T2-weighted imaging and T2* mapping cardiac MRI depict, respectively, edema and IMH. The acute infarct size can be determined with late gadolinium enhancement (LGE) cardiac MRI.

Do we have the guts to go? The abdominal compartment, intra-abdominal hypertension, the human microbiome and exploration class space missions.

Humans are destined to explore space, yet critical illness and injury may be catastrophically limiting for extraterrestrial travel. Humans are superorganisms living in symbiosis with their microbiomes, whose genetic diversity dwarfs that of humans. Symbiosis is critical and imbalances are associated with disease, occurring within hours of serious illness and injury. There are many characteristics of space flight that negatively influence the microbiome, especially deep space itself, with its increased radiation and absence of gravity. Prolonged weightlessness causes many physiologic changes th

Incidence, Risk Factors, Management, and Complications of Rectal Injuries During Radical Prostatectomy.

Rectal injury (RI) during radical prostatectomy (RP) is a severe complication. So far, only limited data describing the incidence, risk factors, management, and complications of RI are available. In an analysis of data for 24178 patients, we identified 113/24076 patients (0.47%) undergoing open or robotic RP and 7/102 patients (6.86%) after salvage RP who experienced an RI. Besides salvage RP, local tumor stage, Gleason grade, lymph node status, and surgical experience, but not surgical approach (robotic vs open), could be identified as risk factors for RI in univariate and multivariate analys

Morphometric and histological changes in the vascular wall after external radiation for the prevention of intimal hyperplasia.

BACKGROUND: Recent reports describe spontaneous dissections and aneurysms after coronary and peripheral artery irradiation for the prevention of intimal hyperplasia. We investigated histological changes and the vasomotor reaction in the vascular wall after external radiation for the prevention of intimal hyperplasia in rabbits. MATERIALS AND METHODS: The aorta was experimentally injured in 34 rabbits who were then assigned to one of three groups: irradiation with 20 Gy; with 25 Gy; and a control group with no irradiation. Before the arterial injury and 45 days later, vasomotor function was as

Operative management of tumors involving the cavernous sinus.

In the past, neurosurgeons have been reluctant to operate on tumors involving the cavernous sinus because of the possibility of bleeding from the venous plexus or injury to the internal carotid artery (ICA) or the third, fourth, or sixth cranial nerves. The authors describe techniques for a more aggressive surgical approach to neoplasms in this area that are either benign or locally confined malignant lesions. During the last 2 years, seven tumors involving the cavernous sinus have been resected: six totally and one subtotally. The preoperative evaluation included axial and coronal computerize
