Motility function of the esophagus before primary anastomosis in esophageal atresia.

Two patients with a long gap esophageal atresia without tracheoesophageal fistula underwent an esophageal manometric study before primary anastomosis and one of them underwent an identical study after operation. Before operation both patients showed peristaltic contraction in the proximal esophagus when swallowing which was always followed by a coordinated contraction of the distal esophagus the same as in that of the normal esophagus. These peristaltic contractions induced a reflex relaxation of the lower esophageal sphincter (LES). After operation the manometric studies of the repaired esoph…

[Chronic cough].

Cough is a powerful physiological reflex mechanism that causes central airways to be cleared of foreign material and excess secretions. Chronic cough is a very common presenting symptom in general practice with a prevalence in the population of about 5%. As a general rule, persistent cough represents organic disease of the upper and lower airways, the lung parenchyma, the pulmonary circulation, the pleural space, the mediastinum and the upper gastro-intestinal tract. The most common cause is cigarette smoking, but virtually any chronic lung disease may be occasionally associated with cough. Co…

[Treatment of algodystrophy of the knee: diagnostic error and misplacement of the peridural catheter].

A fifty-year-old female patient, with a history of reflex sympathetic dystrophy of the left hand and right ankle, complained of pain in her right knee. The skin was slightly oedematous and red, whereas the knee X-ray was quite normal. These findings were thought to be related to another episode of reflex sympathetic dystrophy, and treatment with continuous epidural morphine and lidocaine was prescribed. Catheter insertion was uneventful. However, the lack of pain relief led to the suspicion of femoral neuralgia. Plain X-ray films of the lumbar spine showed the epidural catheter to be passing t…

Impairment of sympathetic activation during static exercise in patients with muscle phosphorylase deficiency (McArdle's disease).

Static exercise in normal humans causes reflex increases in muscle sympathetic nerve activity (MSNA) that are closely coupled to the contraction-induced decrease in muscle cell pH, an index of glycogen degradation and glycolytic flux. To determine if sympathetic activation is attenuated when muscle glycogenolysis is blocked due to myophosphorylase deficiency (McArdle's disease), an inborn enzymatic defect localized to skeletal muscle, we now have performed microelectrode recordings of MSNA in four patients with McArdle's disease during static handgrip contraction. A level of static handgrip th…

Carotid sinus receptors participate in glucose homeostasis.

This paper describes (a) the influence of glucose on carotid chemoreceptor activity, and (b) the participation of carotid receptors in gluco … tic venous-arterial glucose difference above basal levels. Bilateral adrenalectomy eliminated these reflex responses. Cyanide injected in the same conditions caused a sharp increase in glucose retention by the brain. In control experiments, after sectioning the carotid nerve, NaCN injections were ineffective. However, electrical stimulation of the centra …

Acquisition of acoustic startle response in relation to growth and thyroid function in rats.

Thiourea was administered by gavage to pregnant and lactating rats from gestational day 18 until postnatal day 10, at doses of 100 or 250 mg per day (approximately 350 or 900 mg/kg/day). From the day after birth until postnatal day 14 the neonates were weighed, and beginning on postnatal day 10 acquisition of acoustic startle reflex was assessed. Serum thyroxine and thyrotropin were measured on postnatal day 14. High dose thiourea severely inhibited thyroid function in pups, significantly depressed body growth, and retarded development of the acoustic startle response. At the low dose there wa…

Persistence of habituation deficits after neurological recovery from severe thiamine deprivation.

Rats were fed a thiamine-deficient diet for 4 weeks and injected daily with pyrithiamine during the last two weeks of the diet. This regime induced severe neurological anomalies such as ataxia, loss of righting reflex and visual place reflex, and finally full tonic-clonic seizures. These symptoms are reminiscent of the clinical Wernicke-Korsakoff syndrome. Injection of thiamine dramatically reversed these symptoms within one or two hours as seen in Wernicke patients. Six weeks later these rats showed a marked deficit in habituation of exploratory behavior and of the auditory orienting response…

Neurotensin-induced colonic motor responses in dogs: a mediation by prostaglandins.

The effects of systemic infusion of neurotensin (NT) were studied in four dogs fitted with strain-gauge transducers implanted on the ascending and descending colon. The motility index of the colon was enhanced for the duration of the 20 min NT infusion (20 pmol X kg-1 X min-1). Such stimulation was comparable to the colonic motor response elicited by feeding, i.e. the gastrocolic reflex. A period of hypomotility, lasting 40-90 min, occurred after either feeding or completion of NT infusion. Pretreatment with prostaglandin synthetase inhibitors, ketoprofene (KTP) and acetylsalicylic acid (ASA),…

Airway receptors in cough.

Sulphur dioxide inhalation (200 ppm) suppresses the inflation apnea in rabbits, but not in dogs. In rabbits, SO2 blocks airway slowly adapting stretch receptors (SAR) while rapidly adapting irritant receptors (RAR) remain largely unaffected. We studied cough elicited by mechanical irritation of the extrapulmonary airways and larynx in 13 rabbits and 4 dogs, anesthetized and spontaneously breathing, before and after SO2 inhalation. In rabbits, once SO2 had blocked the Hering-Breuer inflation reflex, mechanical stimulation of the trachea and carina failed to elicit cough; laryngeal stimulation w…

Clinical findings in patients with acoustic neurinoma.

Herein we review the symptoms, physical findings, and test results in 131 patients with acoustic neurinoma. The earliest symptoms are unilateral hearing loss, tinnitus, and dysequilibrium. As the disease progresses, facial numbness, facial weakness, and headaches become more prominent. Physical findings other than hearing loss are uncommon; the most frequently observed are a decreased corneal reflex, nystagmus, and facial hypoesthesia. Routine audiometry provides objective information about the hearing loss. Some degree of asymmetry is found in the pure-tone tests. In addition, the speech disc…

Nicotinic nociceptors on perivascular sensory nerve endings.

Intra-arterial injection of increasing doses of acetylcholine, nicotine, carbachol and pilocarpine into the pain reflex ear of the rabbit dose-dependently caused a reflex fall in mean arterial blood pressure. The order of potency of the algesic substances was: nicotine greater than acetylcholine much greater than carbachol much greater than pilocarpine, whereas the purely muscarinic agonist bethanechol was inactive up to 3 mg. Atropine at 1 microgram/ml did not influence the effect of acetylcholine. At 30 micrograms/ml it reduced the effect of acetylcholine but also that of bradykinin suggesti…

Parasympathetic cardiovascular control in human disease: a critical review of methods and results.

A variety of experimental approaches have been used to alter reflex parasympathetic cardiovascular control in normal human subjects and in patients with cardiovascular diseases. This review critically appraises several of these methods (the Valsalva maneuver, upright tilt, lower body negative pressure, pressor drug infusion, neck suction, and coronary arteriography) and summarizes the normal response patterns of healthy subjects and the abnormal response patterns of some patients with cardiovascular disease. Tests of parasympathetic function tend to be heterogeneous; they are not standardized,…

[Investigation of orthostatic hypotension by the provoked hypercapnia test (author's transl)].

Orthostatic hypotension results from an alteration in the baroreflex at any level with subsequent disturbance in blood pressure homeostasis in response to variations. The second part of this reflex arc, from the bulbar vasopressor centre to the peripheral effector sites, can be explored directly by studying the hemodynamic response to an increase in arterial CO2 levels. Normally the response is an increase in blood pressure, but under abnormal conditions there is a reduction. Orthostatic hypotension with a normal hemodynamic response to increased CO2 levels indicates that the first part of the…

[Normal and abnormal response of the arterial pressure to standing (author's transl)].

Redistribution of blood volume during orthostatism is followed by a reduction in venous return and therefore cardiac output. A drop in the blood pressure following postural changes is normally prevented by the action of the baroreceptor reflex, which leads to an increase in total peripheral resistance and heart rate. Orthostatic hypotension may arise from a variety of causes. In some cases, the disorder of postural regulation is due to an autonomic nervous system disease, and various tests are available that enable the diagnosis to be confirmed and the mechanism of action to be elucidated. Cer…

A manometric assessment of anorectal pressures and its significance in the diagnosis of Hirschsprung's disease and idiopathic megacolon.

Anorectal manometric studies were performed on nine patients with Hirschsprung's disease and ten patients with idiopathic megacolon for the purpose of differentiation, comparing with those of the normal subjects. The anorectal reflex was absent in all nine of the patients with Hirschsprung's disease while in all of ten patients with idiopathic megacolon rectal distension produced a relaxation in the anal canal like that of the normal subjects. Manometric studies proved to be a reliable and non-invasive technique for the diagnosis of Hirschsprung's disease and were of particular value as simple…

Regional distribution of barbital in the brain of mice during the development of tolerance and physical dependence.

ICR male mice each receiving a 16 mg barbital pellet implanted subcutaneously for three days developed about 40 percent tolerance to barbital and more than 50 percent tolerance to pentobarbital as measured by sleeping time. The development of physical dependence in these mice was demonstrated by an increased sensitivity to convulsions with pentylenetetrazol. The concentration of barbital rose to high levels in the subcortex and the cerebellum during the time between the implantation of the pellet and the loss of the righting reflex and distributed uniformly thereafter. After a challenge dose o…

Cortical hyperexcitability in corticobasal syndrome as revealed by giant somatosensory evoked potentials.

OBJECTIVE: Corticobasal syndrome (CBS) is characterized by asymmetric symptoms, including myoclonus. Few studies have reported the observation of giant somatosensory evoked potentials (SEPs) in CBS patients. We aimed to clarify the origin and characteristics of myoclonus by examining the relationships among clinical symptoms and electrophysiological and brain imaging findings. METHODS: We conducted a cross-sectional study of 23 patients with CBS who underwent SEPs at Kyoto University Hospital from 2008 to 2022. We compared SEPs, cortical reflex (C-reflex), and cerebral perfusion in the precen…

The role of pedunculopontine nucleus in isolated REM sleep behavior disorder and REM sleep without atonia.

INTRODUCTION: The aim of this study was to analyze the functions of pedunculopontine nucleus (PPN) in isolated REM sleep behavior disorder (iRBD) and REM sleep without atonia (RSWA) to investigate the role of PPN in dream-enacting motor behaviors in RBD. We evaluated the activity of PPN through the prepulse modulation (PPM) together with other brainstem reflexes to investigate the differences in changes at brainstem. METHODS: We included nine patients with isolated RSWA and 10 patients with iRBD. For diagnosis, all patients underwent polysomnography. None of the patients had parkinsonism or d…
