Anatomical shoulder arthroplasty in epileptic patients with instability arthropathy and persistent seizures.

PURPOSE: Epileptic seizures can cause shoulder dislocation, recurrent instability and eventually arthritis. The purpose of this study was to report the results, complications and rate of revision surgery following anatomical shoulder arthroplasty in epileptic patients with instability arthropathy. METHODS: A consecutive series of eight patients with epilepsy underwent anatomical shoulder arthroplasty for recurrent shoulder instability and were retrospectively reviewed after a mean of 4.7 years (range, 2-7.5 years). These included three total shoulder replacements and five humeral resurfacing

Return to play after shoulder instability in National Football League athletes.

HYPOTHESIS: We hypothesized that National Football League (NFL) players sustaining a shoulder destabilizing injury could return to play (RTP) successfully at a high rate regardless of treatment type. METHODS: We identified and evaluated 83 NFL players who sustained an in-season shoulder instability event while playing in the NFL. NFL RTP, incidence of surgery, time to RTP, recurrent instability events, seasons/games played after the injury, and demographic data were collected. Overall RTP was determined, and players who did and did not undergo operative repair were compared. RESULTS: Ninety-

Professional Pitchers With Glenohumeral Internal Rotation Deficit (GIRD) Display Greater Humeral Retrotorsion Than Pitchers Without GIRD.

BACKGROUND: Dominant shoulder glenohumeral internal rotation deficit (GIRD) has been associated with pitching arm injuries. The relationship of humeral torsion on development of GIRD is not clear. HYPOTHESIS: Pitchers displaying GIRD will display greater humeral retrotorsion when compared with those without GIRD. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: Humeral torsion and shoulder range of motion (ROM) were measured in 222 professional pitchers before spring training from 2009 to 2012. Shoulder external rotation (ER) and internal rotation (IR) ROM were assessed i

Clinical outcomes following revision anterior shoulder arthroscopic capsulolabral stabilization.

INTRODUCTION: Traditionally surgeons have treated failed shoulder instability with open capsulolabral repair. Despite improved instrumentation, technique and familiarity in shoulder arthroscopy, few studies have reported the outcomes of arthroscopic revision shoulder instability repair. The purpose of this study was to assess clinical outcomes in patients following revision shoulder arthroscopic anterior capsulolabral stabilization. MATERIALS AND METHODS: Sixty-two patients (63 shoulders) with failure of primary instability repairs were treated with revision arthroscopic anterior shoulder sta

Denervation of the infraspinatus and release of the posterior deltoid muscles in the management of dyskinetic external shoulder rotation in cerebral palsy.

OBJECT: The dyskinetic subtype of cerebral palsy is difficult to manage, and there is no established gold standard for treatment. External rotation of the shoulder(s) is often managed nonsurgically using injections of botulinum toxin A into the external rotator muscles. This article reports a new surgical technique for managing external rotation when botulinum toxin A treatment is not sufficient or possible. METHODS: Six patients with dyskinetic cerebral palsy underwent denervation of the infraspinatus muscle and release of the posterior part of the deltoid muscle. Postoperative questionnaire

The effect of humeral version on teres minor muscle moment arm, length, and impingement in reverse shoulder arthroplasty during activities of daily living.

BACKGROUND: External rotation can be compromised after reverse total shoulder arthroplasty (RTSA). A functional teres minor (TM) is relatively common in patients with posterosuperior tears of the rotator cuff, and its function should be enhanced for better postoperative results. The aim of this study was to investigate how the version of humeral fixation can affect the TM rotational moment arm and muscle length as well as impingement after RTSA. METHODS: A 3-dimensional shoulder model was used to describe RTSA. Four humeral fixation versions were tested: +20°, 0°, -20°, and -40° (+, anteverte

A new anatomic technique for type II SLAP lesions repair.

PURPOSE: A new and more anatomical technique for SLAP II lesions repair is described. It consists in the reattachment of the medial aspect of the biceps anchor to the superior glenoid neck with a mattress stitch posterior and medial to the biceps anchor and a simple stitch placed anteriorly to the biceps. METHODS: From 2011 to 2012, 14 patients matching the inclusion criteria were selected for the study. A visual analogic scale, ROWE, UCLA, ASES and Constant scores were used to make evaluation. The passive ROM before surgery, at final follow-up, and the resumption of sports activities were an

Clinics in diagnostic imaging. 148. Bursal-sided partial-thickness supraspinatus tendon tear.

Rotator cuff injury comprises a continual spectrum of lesions ranging from tendinopathy, which may progress to partial- or full-thickness tear. This progression may be influenced by the interplay of extrinsic and intrinsic factors. We describe the case of a 29-year-man who presented with right shoulder pain for one year. His initial magnetic resonance (MR) arthrogram showed supraspinatus tendinopathy. Subsequent MR arthrogram, obtained after a road traffic accident, showed a bursal-sided partial-thickness tear of the supraspinatus tendon. In view of his worsening clinical symptoms, surgery was

Biomechanics of open Bankart and coracoid abutment procedures in a human cadaveric shoulder model.

The specific aims of this experiment were (1) to develop a clinically relevant model of anteroinferior shoulder dislocation in the apprehension position to compare the biomechanics of the intact anterior capsuloligamentous structures, and (2) to evaluate the initial strength of an open Bankart and of a coracoid abutment procedure. Fifteen shoulders from deceased donors were used. For the intact shoulders, mean peak load was 486 N, and stiffness was 26,7 N/mm. For the Bankart repair, the mean peak load was 264 N, and mean stiffness was 14.1 N/mm. Transosseous repairs failed by suture pullout th

Dynamic visualization of the coracoacromial ligament by ultrasound.

Subacromial impingement syndrome (SIS) is prevalent in athletes who make throwing motions over their heads, as well as in the normal population, but it is difficult to diagnose precisely using physical examination and traditional imaging modalities. Furthermore, the diagnostic testing protocols have not been strictly standardized. We used ultrasound to dynamically visualize coracoacromial ligament (CAL) morphology during shoulder impingement tests: the CAL is the key impinging structure in SIS. Fifty normal shoulders were examined. With the transducer placed on the CAL, the shoulders were exam

Exposure to low amounts of ultrasound energy does not improve soft tissue shoulder pathology: a systematic review.

BACKGROUND: Although therapeutic ultrasound is commonly used to treat shoulder injuries, research to date on the ability of ultrasound to improve outcomes for shoulder pathologies is conflicting. OBJECTIVE: This study aimed to systematically and critically review available literature to ascertain whether beneficial effects of ultrasound were associated with certain shoulder pathologies or particular ultrasound treatment protocols. METHODS: Five electronic databases were searched, and the included studies, identified through pair consensus, were randomized controlled trials (RCTs) that utiliz

Recurrent anterior dislocation after the Latarjet procedure: outcome after revision using a modified Eden-Hybinette operation.

We assessed the results of the modified Eden-Hybinette operation as a salvage procedure after failure of an index Latarjet. Outcomes, complications, and evidence of arthritis of the glenohumeral joint were evaluated in 46 patients. Thirty-four (74%) were clinically and radiologically assessed at a mean of 6.8 years (range, 2-17 years). Good or excellent results were recorded in 27 of the 34 patients (79%). Four patients had another dislocation, and 2 became recurrent dislocators. One required an arthrodesis. Thirteen complained of a sensation of persistent apprehension. Twenty-three (68%) retu

Extracorporeal shock wave therapy for calcifying tendinitis of the shoulder.

We prospectively studied extracorporeal shock wave therapy (ESWT) for calcific tendinitis of the shoulder in 46 consecutive patients. All patients were randomly divided into 2 groups: treatment and control. The 33 patients in the treatment group received 2 courses of ESWT at the energy density of 0.55 mJ/mm(2) (1000 impulses). The control group underwent sham treatment with a dummy electrode (13 patients). Evaluation included the Constant score, pain scale, and radiographs. The ESWT results were good to excellent in 87.9% of shoulders (29/33) and fair in 12.1% (4/33), and the control results w

Contralateral brachial plexus neuropathy after arthroscopic shoulder surgery.

This case report presents a neurologic complication that occurred after shoulder arthroscopic surgery on the contralateral arm. This brachial plexus palsy has been documented as a reversible C7-T1 lesion, which was the consequence of the patient's unknown cervical rib and the applied lateral position during the operation. In this case, the recovery was almost complete within 3 months postoperatively; however, one should take care to prevent these complications with an appropriate positioning of patients with an anatomic variant.

Cutaneous nerve lesions of the shoulder and arm after arthroscopic shoulder surgery.

Increasing use of shoulder arthroscopy has caused developing awareness of the associated complications. A consecutive series of patients who had undergone arthroscopic surgery of the shoulder was reviewed. The overall incidence of a sensory deficit was 21 (7%) in 304 patients at 2 weeks after operation, and in approximately half (3.3%) of these patients this condition was still present at 8 months' average follow-up. These deficits fell into three distinct patterns, suggesting that damage was occurring to three different nerve branches. Most of these areas of hypesthesia corresponded to lesion

[Therapeutic procedure in initial traumatic shoulder dislocation (arthroscopy--limbus refixation)].

This is a report of the therapeutic measures following primary traumatic dislocation of the shoulder in young patients (under 40 years of age). In order to rule out a disruption of the glenoid labrum, all patients were subjected to arthroscopy and disruption thus diagnosed in 38 of 48 cases. Surgical refixation of the glenoid labrum was performed on these patients. The ten patients with a stable labrum were given a Gilchrist dressing, immobilizing the shoulder for two to three weeks. Eight of the ten patients who had only had arthroscopy were followed up for an average of three years (two to s

[Degenerative shoulder diseases--their differentiation and surgical therapy].

Degenerative shoulder diseases can be clearly differentiated from each other and from different ailments by careful recording of the case history, through clinical examination, and arthrography. Considerable therapeutic achievements have been recordable from operations on patients who had suffered for years. Eighty anterior acromion repairs had been performed with encouraging results by Juli 1987. Postoperative physiotherapy should be carried out with persistency and may last several months.
