Sleep-related respiratory disturbance and dementia in elderly females.

Sleep-related respiratory disturbance was studied with a microprocessor-based portable monitoring system in female residents of a retirement village aged greater than or equal to 75 years. Comparisons were made between 29 demented subjects Mini-Mental State Examination Score (MMSE) less than 21 and 48 controls (MMSE greater than 25). Respiratory disturbance index (RDI, the number of episodes of apnea and hypopnea/hour of total sleep time) was higher in the demented subjects: mean RDI (+/- SD) 18.5 +/- 18.6 vs 7.3 +/- 10.8, p = .004. The number of minutes per hour of sleep spent with disturbed …

Central angiotensin alters blood pressure regulation during natural sleep.

Sino-aortic denervation (SAD) in rats alters completely the pattern of pressure changes during sleep: from unchanged to a rise in SS and from slight increase to a market drop in DS. Rats with mild renal hypertension (1K - 1C) behaved like normotensive rats, whereas in those rats with hypertension accompanied by overactivity of RAS, the pattern of MAP changes during sleep was similar to SAD rats. Since acute SAD also produced overactivity of RAS we studied SAD rats treated with Captopril or when RAS was normal at the chronic phase of SAD and we showed that impairment of baroreceptor function pe…

Familial 'sleep apnea plus' syndrome: report of a family.

We describe a familial disorder consisting of sleep apnea, anosmia, colorblindness, partial complex seizures, and cognitive dysfunction. The phenotypic expression of the syndrome suggests an autosomal dominant inheritance with incomplete penetrance.

[Midazolam used for premedication reinforces sleep induced by flunitrazepam].

This single blind study aimed to discover possible effects of intramuscular premedication with midazolam on the sleep induced by intravenous flunitrazepam. 24 male patients, aged 17 to 71 years, who were to undergo surgery to the distal parts of an upper limb under regional anaesthesia, were randomly assigned to two equal groups: in the midazolam group, an intramuscular premedication of 0.12 mg.kg-1 midazolam with 0.5 mg atropine was given, whereas in the control group atropine only was used. In all patients, 1 mg flunitrazepam was given intravenously 45 min after the premedication, before car…

Central sleep apnea reversed by continuous positive airway pressure.

Continuous positive airway pressure (CPAP) is an accepted and highly effective method used in the treatment of obstructive sleep apnea (OSA). We have explored the potential utility of CPAP in a patient with sleep apnea without a major contribution of airway obstruction as measured by the usual criteria. The patient showed dramatic clinical and laboratory improvement in central apnea when treated with CPAP. A low level of CPAP (7.5 cm H2O) resulted in the reduction of central apneas from 281 to 32 per night with no significant change in obstructive apneas, which increased from 43 to 59 per nigh…

Bimodality electrophysiologic evaluation of brainstem in sleep apnea syndrome.

Previous electrophysiologic studies in the sleep apnea syndrome (SAS) have used only a single modality (brainstem auditory evoked responses [BAERs]) and yielded conflicting, inconsistent, and inconclusive results. We utilized both BAERs and somatosensory evoked responses in 12 patients with SAS and found normal central conduction times in all patients. These data argue against a significant structural alteration in both rostral and caudal brainstem, insofar as the auditory and somatosensory pathways are concerned, in patients with SAS.

Periodic leg movements in sleep: effect of clonazepam treatment.

Periodic leg movements in sleep were diagnosed in 8 patients with insomnia and in 12 patients with hypersomnia. Polysomnography did not reveal any difference in the number of leg movements in these groups, the mean duration of a leg movement, or the number of activity epochs. Those with insomnia, however, had a shorter intermovement interval than those with hypersomnia, with more leg movements in each activity epoch. Treatment with 0.5 to 2 mg clonazepam improved subjective complaints and decreased the number of leg movements without affecting the intermovement interval or movement duration.

Nocturnal sleep in mentally retarded infants with cerebral palsy.

Sleep EEG patterns in 23 mentally retarded children with cerebral palsy (CP) (from 4 months to 5 years of age) and 39 reference mentally retarded children of no abnormality with the exception of psychomotor retardation (from 4 months to 12 years of age) were studied throughout nocturnal sleep, and the following results were obtained. Eleven CP cases and 30 reference cases evidenced normal sleep patterns that could be classified into 6 stages. Twelve other cases in CP and 9 reference cases showed some abnormal sleep EEG patterns as follows: (i) in CP cases absence of EEG patterns characteristic…

Prolonged expiratory phase in sleep apnea. A unifying hypothesis.

The mechanisms responsible for the various sleep apnea patterns have not been elucidated. In 5 hypersomnolent patients, we analyzed 613 events that would have been labeled mixed and central apneas had usual recording sensitivity been employed. Recording air flow at high sensitivity revealed that in most of these events expiratory flow was present throughout most of the prolonged period during which there was no inspiratory effort. In 378 events, resumption of inspiratory effort was unaccompanied by air flow (occluded events), whereas in 144 events resumption of inspiratory effort was accompani…

Abnormal cerebrovascular responses to CO2 in sleep apnea patients.

The responses of common carotid blood flow (CCF), pressure (BP), and resistance (R) to variations in respiratory gases were compared during waking periods in 10 sleep apnea patients (SA) and 10 healthy controls (N) of similar age. Respiratory gases were altered by 3-min CO2 rebreathing (RB), 3-min hyperventilation (HV), and 4-min hypoxia (HYP) procedures. CCF was measured continuously by a 5-MHz pulsed Doppler duplex scanner and R was calculated using brachial BP. During RB, which increased end-tidal PCO2 (PACO2) by 15 mm Hg, SA had a lower CCF and greater BP response and therefore a significa…

Sleep apnea and nocturnal hypoventilation after western equine encephalitis.

That viral encephalitis can produce daytime hypoventilation is well appreciated, but an association of this infection with the development of a prolonged disturbance of breathing during sleep once the ventilatory disorder during wakefulness has resolved, has not been reported. We describe a young male who required assisted mechanical ventilation in association with western equine encephalitis. Normal waking ventilation returned over 3 to 4 wk, but frequent apneas and severe hypoventilation persisted during sleep with a near absence of waking ventilatory response to chemical stimuli. Over 3 mon…

Daytime sleep stage organization in three-month-old infants.

Sleep monitoring at 3 months post term of clinically normal term infants, pre-term infants with no demonstrable pathologic clinical signs, and pre-term and term infants with a history of metabolic disorders show that these infants have EEG sleep stages resembling those seen in adults, as well as the adult pattern of sleep stage organization. The presence of NREM stage organization and stage sequencing suggest that sleep regulatory mechanisms are approaching a level of functional maturity in the human infant at 3 months post term. There is a significant relationship between sleep staging at 3 m…

Pathophysiologic mechanisms causing sleep apnea and hypoventilation in infants.

Complete absence of chemoreception in the medulla is characterized by sustained hypoventilation. Except for the first months of life, this deficit does not, of itself, cause apnea. Both in infants who lack central chemical drive, as well as those who are only partly deficient, it is necessary to invoke at least one further pathophysiologic event. Various alterations in gas exchange, especially during sleep, which promote hypoxemia can contribute to a positive feedback effect on ventilation. If gas exchange is further impeded by upper-airway obstruction, only a massive arousal response separate…

Narcolepsy: regional cerebral blood flow during sleep and wakefulness.

Serial measurements of regional cerebral blood flow were made by the 135Xe inhalation method during the early stages of sleep and wakefulness in eight normal volunteers and 12 patients with narcolepsy. Electroencephalogram, electro-oculogram, and submental electromyogram were recorded simultaneously. In normals, mean hemispheric gray matter blood flow (Fg) during stages I and II sleep was significantly less (-9.2 percent) than waking values (84.3 +/- 13 ml per 100 gm brain per minute). Maximum regional blood flow decreases during sleep occurred in the brainstem-cerebellar (-25.1 percent), righ…

EEG sleep in primary depression. A longitudinal placebo study.

Characteristic EEG sleep changes in depression are highlighted by a sleep continuity disturbance, delta sleep reduction, and a shortened REM latency. Since these findings have been derived primarily from only a few baseline recordings, questions regarding their persistence and/or variability have not been previously addressed. As part of an extensive set of investigations of EEG sleep in depression, we examined nightly the sleep of 12 hospitalized, non-delusional, primary depressives who were involved in a program of active psychosocial treatment intervention and received only placebo during a…

Transient hypoxaemia during sleep in chronic bronchitis and emphysema.

Arterial oxygenation, breathing pattern, and electroencephalogram were studied during sleep in patients with chronic bronchitis and emphysema and in healthy subjects. All of the 10 "blue and bloated" patients had episodes of transient hypoxaemia lasting 1--100 min, when their oxygen saturation fell by more than 10%, whereas such desaturation did not occur in 2 "pink and puffing" patients or in 4 healthy subjects. Hypoxaemic episodes usually occurred during the rapid-eye-movement stage of sleep. It is suggested that these hypoxaemic episodes result from a combination of hypoventilation and impa…
