Patella tendon anterior tibial cortex angle (PTATA): An indirect marker of anterior cruciate ligament (ACL) tear?

BACKGROUND: Anterior cruciate ligament (ACL) injuries are common in high-impact sports. While direct MRI evaluation is preferred for diagnosis, indirect signs are often crucial. The Patella Tendon Anterior Tibial Cortex Angle (PTATA) could serve as an additional indirect marker of ACL injury, reflecting altered knee biomechanics. We hypothesise that knees presenting complete ACL rupture will exhibit a smaller PTATA on MRI due to the associated anterior tibial translation. METHODS: We reviewed 100 knee MRI scans of patients under the age of 30 years, presenting with acute knee injuries and cli

Early-life exercise effects on Achilles tendon in mice selectively bred for high voluntary wheel-running behavior.

Exercise increases muscle and bone strength and mass, but effects on tendons are less documented. We investigated the impact of voluntary exercise (wheel running) during early-life exercise (weanling; 3 weeks old) compared to post-skeletal maturity (young adult; 9 weeks old) on tendon morphology and material properties. We utilized a selectively bred High Runner (HR, N = 40) mouse line and a control line (N = 40). Mice underwent 8 weeks in cages either with or without wheels. HR mice ran ~3-fold more and were smaller than controls, but exercise reduced body mass in both lines. Tendon cross-sec…

Caffeine decreases muscle and tendon protein synthesis and engineered ligament strength in vitro and attenuates adaptation to exercise in mice.

Caffeine is a well-known stimulant that is widely used to increase alertness and performance. However, there is little information on the effect of chronic caffeine consumption on exercise adaptations. We first sought to characterize the effect of caffeine on protein synthesis in vitro using immortalized C2C12 and TTD6 muscle and tendon cells. Total protein synthesis, as measured by puromycin incorporation, decreased 31% in TTD6 cells with 4 mM caffeine (P < 0.05) and 41% in C2C12 cells with 0.5 mM caffeine (P < 0.01). The structure and function of in vitro engineered ligaments were also reduc

Middle trapezius tendon transfer for isolated irreparable supraspinatus tears shows favorable outcomes despite subscapularis tear.

INTRODUCTION: Middle trapezius tendon (MTT) transfer has emerged as a viable treatment option for isolated irreparable supraspinatus tears (IIST). This study aimed to compare the clinical outcomes of MTT transfer between patients with intact subscapularis and those with concomitant subscapularis tears. METHODS: A retrospective review was conducted on patients who underwent arthroscopically assisted MTT transfer for IISTs between January 2018 and December 2023, with a minimum follow-up of 24 months. Patients were stratified into two groups: subscapularis intact and subscapularis tear (Lafosse

The COVID rebound effect: Incidence of tendon rupture surgeries in the peripandemic COVID era in a single tertiary academic institution.

BACKGROUND: The COVID-19 pandemic led to increased sedentary behavior and body mass index during 2020, which may decondition musculotendinous units and lead to increased risk for injury. As patients return to pre-COVID levels of activity, we hypothesize that there will be a resultant rebound increase in tendon ruptures. LEVEL OF EVIDENCE: III, descriptive epidemiology study. METHODS: The electronic medical record was queried for current procedural terminology codes for tendon ruptures (Achilles, patella, quadriceps, hamstring, distal biceps, triceps, and pectoralis major) from January 2017 t

Minimally Invasive vs. Classical Technique in Primary Repair of Achilles Tendon in Men Aged 20-40 Years.

BACKGROUND: Injuries to the Achilles tendon in men aged 20-40 years are most commonly caused by intensified physical activity leading to prolonged overload of the tendon. Most injuries affect the tendon's proximal third, which is less vascularised. Surgery for Achilles tendon injuries aims to obtain the best possible gait biomechanics and quickly restore the patient's functional performance. Clinical outcomes vary with the surgical technique used. This article presents the results of a study comparing the minimally invasive technique and the classical technique for primary repair of the Achill

Temporal vascular pattern remodeling mediated by the FHL2/sFRP2 signaling pathway in tenocytes affects tendon repair and regeneration.

Although angiogenesis following tendon injury was expected to provide nutrients for regeneration and repair, excessive angiogenesis may be associated with poor long-term outcomes in tendinopathy. Here we aim to explore the pathological role of angiogenesis in the progression of tendinopathy. Patients with tendinopathy were categorized into a hypervascularization group (HyperV) and a hypovascularization group (HypoV), and postarthroscopic outcome and histopathology were compared. In addiiton, tendon injury models and tenocyte stress models were employed to investigate the temporal-spatial vascu

Gastrocsoleus lengthening in combination with tibialis anterior tendon shortening for equinus deformity in children with cerebral palsy: a systematic review.

BACKGROUND: Equinus is the most common deformity in cerebral palsy, and gastrocsoleus lengthening (GSL) is the most common surgical intervention in children with cerebral palsy (CwCP). GSL is a dose-dependent surgical concept carried out with consideration of the severity of equinus contracture, aimed at addressing this problem during the stance phase of gait. The addition of the novel procedure, tibialis anterior tendon shortening (TATS), may offer benefit in correcting the swing phase problem by addressing foot drop due to the agonist/antagonist relationship with gastrocsoleus. Given the sma

Effects of ankle joint biomechanics and foot postures on Achilles tendon force during running through a simulation study.

Achilles tendinopathy is a prevalent injury observed among runners. Achilles tendon force (ATF) and differences in foot posture are considered factors in the occurrence of Achilles tendinopathy. This study was conducted using computer simulations. The simulation ranges were determined for the ankle joint angle (-25° to + 25°) and plantarflexion moment (0-160 Nm) at variable running speeds using a public dataset. The ankle angle was used to compute the Achilles tendon moment arm. Moment arms specific to the normal foot, pes cavus, and pes planus were calculated using radiographs from a previous

Leptin Enhances M1 Macrophage Polarization and Impairs Tendon-Bone Healing in Rotator Cuff Repair: A Rat Model.

BACKGROUND: Rotator cuff tears are common, affecting more than 60% of individuals older than 80 years, and they have been implicated in 70% of patients with shoulder pain. M1 polarization-related inflammation has been reported to be associated with poor healing outcomes of rotator cuff injury, and leptin, an adipokine, has been reported to be a potential activator of inflammation. However, whether leptin affects rotator cuff repair remains unknown. QUESTIONS/PURPOSES: Using in vitro cell experiments and an in vivo rat rotator cuff tear model, we therefore asked: (1) Does leptin promote the M1

Exercise Initiation After Surgical Reconstruction for Rat Rotator Cuff Injury Affects Tendon Healing and Muscle Preservation.

Rotator cuff tear is a common disease, and surgical repair is often conducted, but the re-tear rate after surgery is still high. Mechanical stress is the key factor that promotes tendon healing, but the optimal exercise protocol to induce rotator cuff healing is unknown. This study aimed to elucidate the effects of postoperative exercise initiation timing on tendon healing and muscle degeneration after rotator cuff tear and reconstruction by controlling the shoulder joint motion with immobilization and exercise intervention. A total of 110 Wistar rats were used in this study and the supraspina
