The evaluation of secondary pneumomediastinum in children: the experience of a pediatric surgery clinic.

BACKGROUND: Pneumomediastinum, defined as mediastinal emphysema, is the term coined for the presence of air in the mediastinum. It is a rare condition that can occur in children due to various etiologies, especially trauma, and may appear without any underlying injury. We investigated the causes of secondary pneumomediastinum and the treatment approaches in children treated at our clinic. METHODS: We retrospectively assessed 26 patients under the age of 18 diagnosed with pneumomediastinum at our clinic between 2011 and 2023. We reviewed patient files, evaluating clinical data including patien

Identifying predictors of nosocomial pneumonia in trauma patients admitted to a level-1 trauma center.

BACKGROUND: Nosocomial pneumonia is common in trauma patients and associated with an adverse prognosis. We recently externally validated and recalibrated an existing formula to predict nosocomial pneumonia risk. Identifying more potential predictors could aid in a more accurate prediction of nosocomial pneumonia risk in level-1 trauma patients. This study aims to identify predictors of nosocomial pneumonia in level-1 trauma patients available in the emergency department or shortly after, and examine their added predictive value to an existing prediction model by Croce. METHODS: This retrospec

The efficacy and adverse effects of the Uniblocker and left-side double-lumen tube for one-lung ventilation under the guidance of chest CT.

One-lung ventilation (OLV) is essential in numerous clinical procedures, in which the left-sided double-lumen tube (LDLT) is the most commonly used device. The application of bronchial blockers, including the Uniblocker or Arndt blocker, has increased in OLV. The present study aimed to compare the efficacy and adverse effects of the Uniblocker and LDLT for OLV under the guidance of chest CT. A total of 60 adult patients undergoing elective left-side thoracic surgery requiring OLV were included in the study. The patients were randomly assigned to the Uniblocker group (U group, n=30) or the LDLT

Prognostic value of QRS duration after transcatheter aortic valve implantation for aortic stenosis using the CoreValve.

Transcatheter aortic valve implantation (TAVI) is effective in treating severe aortic stenosis in high-risk surgical patients. We evaluated the value of the QRS duration (QRSd) in predicting the mid-term morbidity and mortality after TAVI. We conducted a prospective cohort study of 91 consecutive patients who underwent TAVI using the CoreValve at our teaching hospital cardiology unit in 2008 to 2010 who survived to hospital discharge; 57% were women, and their mean age was 84 ± 7 years. The QRSd at discharge was used to classify the patients into 3 groups: QRSd ≤120 ms, n = 18 (20%); QRSd >120

Noninferiority of Ultrasound-Guided Modified Intercostal Block to Traditional Approach for Analgesia After Minimally Invasive Repair of Pectus Excavatum in Children: A Randomized Trial.

BACKGROUND: For pediatric pectus excavatum, the standard treatment is minimally invasive repair of pectus excavatum (MIRPE). A major challenge, however, is the severe postoperative pain. Although ultrasound-guided intercostal nerve block (UINB) offers effective analgesia, the technique's complexity and associated safety concerns are significant barriers, deterring its routine use. Modified intercostal nerve block (MINB) is effective in adult thoracic surgery but unvalidated in pediatric MIRPE. OBJECTIVE: To evaluate MINB's noninferiority to UINB for postoperative analgesia and safety in child
