Identification and characterization of the carboxy-terminal region of Sip-1, a novel autoantigen in Behçet's disease.

Given the lack of a serological test specific for Behçet's disease, its diagnosis rests upon clinical criteria. The clinical diagnosis is ne … diseases that share clinical features with Behçet's disease, such as inflammatory bowel disease and uveitis. IgM immunoreactivity to Sip1 C-ter was significantly higher in patients with Behçet's disease and in patients with primary vasculitis than in the other groups of patients and healthy subjects tested (P < 10-4 by Mann-Whitney test). ELISA detected I …

A comparison of two regimens for the treatment of Mycobacterium avium complex bacteremia in AIDS: rifabutin, ethambutol, and clarithromycin versus rifampin, ethambutol, clofazimine, and ciprofloxacin. Canadian HIV Trials Network Protocol 010 Study Group.

BACKGROUND: Bacteremia with the Mycobacterium avium complex is common in patients with the acquired immunodeficiency syndrome (AIDS), but th … of rifabutin was reduced by half after 125 patients were randomized, because 24 of 63 patients had uveitis. RESULTS: Among 187 patients who could be evaluated, blood cultures became negative more often in the three-drug group than in the four-drug group (69 percent vs. 29 percent, P<0.001). Among patients treated for at least four weeks, the bacteremia …

[Q Fever: what's new?].

Q fever is caused by Coxiella burnetii, a Gram-negative, intracellular bacterium. Sheep and goats are the main reservoirs of this zoonosis, which is transmitted either by aerosols, over long distances via the wind or through milk or dairy products. Acute Q fever may be asymptomatic, or present as a flu-like syndrome, atypical pneumonia or hepatitis. Chronic Q fever manifests in a wide variety of ways, including blood culture-negative endocarditis, chronic hepatitis, osteoarticular involvement (spondylitis, arthritis, osteitis) or uveitis. Autoimmune diseases can be sometimes induced by chronic…

Diagnosing Behçet's disease in the presence of pulmonary artery aneurysm and systemic symptoms: A case report.

Behçet's disease is a chronic inflammatory disorder that often presents with a triad of uveitis, oral aphthous ulcers, and genital ulcerations, along with systemic manifestations such as vascular involvement. We report a case of a 36-year-old male with a significant smoking history who presented with recurrent hemoptysis and a progressive cough, leading to the diagnosis of Behçet's disease. Physical examination and imaging studies revealed aphthous and genital ulcerations, erythematous nodules, and a pulmonary artery aneurysm. Elevated inflammatory markers supported the diagnosis and treatment…

West Indian (Caribbean) Punctate Keratopathy in a Puerto Rican Girl.

We report on a case of West Indian (Caribbean) punctate keratopathy (WIPK) in a pediatric patient living in Puerto Rico, USA. A 9-year-old Hispanic girl presented for a routine ophthalmic follow-up. The patient had a history of juvenile idiopathic arthritis and chronic bilateral anterior uveitis. At the presentation, her visual acuity was 20/30 in the right eye and 20/20 in the left eye. An examination of the right eye was remarkable for one round, white subepithelial corneal opacity of approximately 0.1 mm in height by 0.1 mm in width, located slightly lateral to the center of the cornea, whi…

Parvimonas micra Necrotizing Panophthalmitis Involving the Sclera, Cornea, Uvea, Retina, and Orbit.

We present an unusual case of a patient who acquired a pansinusitis and orbital cellulitis with necrotizing features, subsequently developing scleritis, keratitis, and anterior uveitis. To date, there are no reported cases of the simultaneous involvement of these ocular structures from a pansinusitis. Our patient was urgently taken to the operating room for drainage of the abscesses within his sinuses and the orbit. Intraoperative cultures were positive for Parvimonas micra, an odontogenic anaerobic bacteria. He was additionally found to have a central retinal artery occlusion. He was treated …

Tubercular Retinal Vasculitis: Diagnostic Dilemma and Management Strategies.

Ocular tuberculosis (OTB) is a rare, extrapulmonary manifestation of systemic TB, which has been a global etiology of uveitis for centuries, though concentrated in the developing world. OTB remains difficult to diagnose clinically despite a plethora of conventional and modern investigations. Tubercular retinal vasculitis (TRV) is a common and specific presentation of OTB but is variably defined in the literature in terms of clinical profile and the investigations essential for diagnosis and treatment. Ironically, the need and duration of antitubercular treatment is uncertain for this manifesta…

Intraocular Chondrosarcoma in a Rabbit.

Post-traumatic intraocular sarcomas are rarely reported in domestic animals and are most common in cats. An 8-year-old rabbit (Oryctolagus cuniculus) was referred to a veterinary clinic due to ocular discharge, uveitis and protein precipitate in the anterior chamber of the right eye. The eye was enucleated and histopathological examination revealed a poorly demarcated tumour within the ciliary body with invasion to adjacent eye structures. Neoplastic cells formed chaotic cartilage lacunae and were immunopositive for vimentin but immunonegative for pancytokeratin. On this basis, the neoplasm wa…

Case Report: Japanese Encephalitis Associated with Chorioretinitis after Short-Term Travel to Bali, Indonesia.

Japanese encephalitis (JE) virus is a mosquito-borne flavivirus endemic throughout Asia. Incidence in non-endemic countries is rare, with an estimate of less than one case per one million travelers. Most human JE infections are asymptomatic or cause a mild, nonspecific febrile illness. Neurological involvement, if present, is usually severe and associated with high mortality or ongoing neurological sequelae in survivors. Ocular manifestations are rare with JE, but uveitis has been described to be associated with other flavivirus infections, including West Nile virus. We report the first probab…

Infliximab in the treatment of Budd-Chiari syndrome in Behçet's disease.

Behçet's disease is a chronic autoinflammatory disorder that usually presents with recurrent oral and genital ulcers, uveitis, skin lesions and arthritis. Any organ, however, can be involved and in a subgroup of patients the large vascular lesions are the predominant feature (vasculo-Behçet disease). We report a case of a 27-yearold man with Behçet's disease who developed Budd-Chiari syndrome while on immunosuppressive therapy. Infliximab resulted in a rapid and total clinical remission. Literature data have shown that antitumor necrosis factor alpha (anti-TNF-α) agents including infliximab ca…

Scleritis, keratitis, and orbital cellulitis: isolated ocular manifestation of systemic lupus erythematosus.

Systemic autoimmune diseases are associated with ocular inflammatory conditions such as episcleritis, scleritis, keratitis, and uveitis. However, ocular manifestations have been reported to correlate with the extent of systemic disease. We present a patient with scleritis, keratitis, and orbital cellulitis, as the isolated manifestation of systemic lupus erythematosus (SLE). No microbial etiology was identified and antibiotics did not produce clinical improvement. The patient improved significantly with steroids and disease-modifying antirheumatic drugs. Although ocular inflammation has been p…
