[Bacterial endocarditis in pregnancy. Report of a clinical case diagnosed postpartum].

Bacterial endocarditis in pregnancy shows a low incidence; it is often associated with a prior history of rheumatic or congenital heart disease. In the large part of reports the illness tends to run a subacute course and to recognize a major frequency in the third trimester of pregnancy. We presented the case of a 29 year-old woman with mitral and aortic bacterial endocarditis. Transthoracic echocardiography performed one week after spontaneous delivery suggested valve vegetations. Antibiotic therapy turned out to be partially successful, in fact cerebral embolizations subsequently occurred. C

The association between breast cancer and diet and other factors.

To explore the effects of diet and other factors on breast cancer development, a case-control study was conducted in Tianjin between 1986-87. After adjusting for confounding factors, it was found that factors associated with increased risk of breast cancer included --early age at menarche; late age at menopause; late age at first birth; high Qutelet's index; histories of breast wound, disease and benign tumor; and a family history of malignant tumor, high fat, high calorie, low vegetable intake; elevated serum cholesterol and LDL-cholesterol; and lower levels of serum HDL-cholesterol and whole

[Possible complications of transpleural neurotomy].

The occurrence of complications in course of thoracoscopic vegetative denervation has been found low in the author's analysis of his 2679 cases. There arose several small lung lesions with consecutive pneumothorax or tension pneumothorax, furthermore some events of emphysema of the skin, one bleeding of an intercostal artery, which was stopped under thoracotomy, one division of the thoracic duct, one thrombosis of the anterior spinal artery, one cardiac failure. On the other hand there could be avoided air embolism, Horner's syndrome, lesions of recurrent and phrenic nerve, damage on trachea,

Fatal disseminated infection caused by Myceliophthora thermophila, a new agent of mycosis: case history and laboratory characteristics.

We report a case of human infection caused by the hyphomycete Myceliophthora thermophila. A 7-year-old male with neurofibromatosis (type 1) was diagnosed in 1987 with acute myeloblastic leukemia associated with the chromosomal abnormality monosomy 7. The patient experienced multiple serious infections over a three-year period before expiring in 1990 while in the end stage of leukemia. Autopsy findings included fungal vegetations of the left atrium, ascending aorta, and pulmonary arteries and fungal invasion of both lungs. Cultures yielded M. thermophila. We believe that this is the first repor

[A case of rupture of the tendinous cord caused by infective endocarditis].

The paper reports a case of infective endocarditis of the valve, with an insidious and slow onset accompanied by low fever, debility, loss of weight, anemia, and the concomitant echocardiographic observation of pericardial effusion. Subsequent echocardiographic tests produced images which probably referred to valvular vegetation. As a matter of fact these findings proved to be result of the rupture of the latero-posterior tendinous cord of the mitral flap and other similar cords whose stumps, covered in fibrin, had adhered to the edge of the anterior cups. This finding was discovered during su

[Pathophysiology of orofacial pain].

The pathophysiology of different kinds of orofacial pains is discussed. (a) Stimulus-induced pain may be elicited e.g. by surgical interventions at the teeth. Nociceptors stimulated by noxious stimulation induce not only pain sensations but contribute also to regulating the blood flow, directly by releasing vasoactive peptides, and indirectly by inducing sympathetic reflexes, (b) Inflammatory pain has its origin in an sensitization of nociceptor units by several kinds of endogenous neuro-vasoactive substances. (c) Chronic pains are either the consequence of motor or vegetative dysregulations o

[Features of the regulation of autonomic nervous system functions in various paroxysmal states of cerebral origin].

The authors studied a total of 218 patients with such pathological states as neuroses with vegetovascular crises, the hypothalamic syndrome, temporal epilepsy, and neurogenic syncopes (four groups). Clinical peculiarities of psychovegetative manifestations and the status of autonomic regulation were analyzed in the above groups. The authors consider the role of a number of factors exercising influence on the organization of the functional regulation of the autonomic nervous system, the degree of changes in the psychological sphere, the nature of the pathological process, the level of damage to

Ecological study of Rocky Mountain spotted fever in Costa Rica.

Since Rocky Mountain spotted fever (RMSF) was recognized in Costa Rica, ecological studies have been undertaken to discern its distribution and the natural cycle of Rickettsia rickettsii. The population of ticks parasitizing animals and vegetation was scarce. Rickettsiae were not isolated from pools of ticks nor from spleens of the animals studied. However, the active circulation of rickettsiae was evidenced through the demonstration of spotted fever group antibodies in humans, dogs, and wild rabbits. These findings concur with those in a number of fatal human cases as well as with the isolati

Cardiopulmonary arrest. Pathophysiology and neurologic complications.

Cardiopulmonary arrest is a test of the brain's tolerance to global ischemia. New insights into the pathophysiology of global ischemia have led to the potential use of early prophylactic anticonvulsants, hypothermia, barbiturate coma, glucose manipulations, calcium-blocking agents, and hemodilution. A wide spectrum of neurologic sequelae may follow global ischemia, ranging from brain death, vegetative states, and impairment of higher intellectual function to syndromes of amnesia and cortical blindness, post-anoxic myoclonus, delayed leukoencephalopathy, and spinal stroke. The distinctive featu

A reconsideration of psychological specificity in psychosomatic disorders.

In this paper, the author identifies the evolution of the specificity hypothesis in the psychogenesis of psychosomatic illness across the last half century. Specifically identified is the distinction made between the mechanism of conversion and that of vegetative neurosis. Considerable attention is addressed to the personality features thought to be common to all psychosomatic patients and the refinement of this theory over the past 40 years, including the present interest in alexithymia. These concepts are explored in the consideration of a man who sustained two different specific conflictual

The authors analyse the results of examination of 96 patients with administration of a new water-soluble radiopaque medium amipak (metrisamide) into the ventricles of the brain (22 cases) and subarachnoid space of the spinal cord (74 cases). The best results were obtained in the diagnosis of Schmorl's body in the cervical spinal segment and in location of new growths in the posterior cranial fossa and those of supratentorial and intraventricular localization. Convincing diagnostic signs of the level of CSF communications obstruction were obtained. EEG was performed in 54 patients. The possible

Periventricular leukomalacia in adults. Clinicopathological study of four cases.

The pathological findings in four patients with courses characterized by acute coma and respiratory insufficiency occurring in obscure circumstances are presented. Carbon monoxide intoxication was excluded. After an early partial recovery from coma, the patients remained in a persistent vegetative state, with a tetrapyramidal syndrome. Pathologic changes consisted of infarction and demyelination of periventricular white matter, with associated necrotic foci in the basal ganglia in some cases. We propose that the prolonged hypoxia and ischemia produce a "no reflow" phenomenon causing brain edem

Typical results of computer-olfactometry.

After pulsed peripheral olfactory impulses it is possible, under suitable testing conditions, for specific sensory potentials to be read from the surface of the skull and, after repetitions, added up (olfactorially evoked cortical potentials). Typically, there developes a so-called twin-potential containing the equivalents of the electrical activites of the nervus trigeminus and the nervus olfactorius in two peaks of the cortex equivalents. In an objective manner, in other words without intentional or vegetative influence by the test person, it is possible by the weakening or lack of one or th

The role of intraoperative transesophageal echocardiography in the successful outcome after mitral replacement for infective endocarditis on the background of hypertrophic obstructive cardiomyopathy: a case report.

Hypertrophic cardiomyopathy is most commonly an autosomal genetic disorder characterized by increased morbidity. Infective endocarditis, although rare, can complicate the course of the disease. The mitral and aortic valves are more commonly affected. Mitral valve vegetations may cause severe regurgitation and acute heart failure. In the event that the anterior valve leaflet is affected, this may result in severe left ventricular outflow tract obstruction (LVOTO), requiring immediate intervention and resulting in a significant increase in mortality. We report a rare clinical case of a patient w

Resected Pulmonary Achromobacter xylosoxidans Mimicking Aspergillosis Fungus Ball.

Achromobacter xylosoxidans, a rare gram-negative rod, often causes pneumonia. Here, we describe the case of a previously healthy 78-year-old woman who worked at a vegetable plant nursery. A 4.5-cm cavitary lesion with a 2.5-cm-diameter lobulated nodule in the left lingula segment, resembling an aspergillosis fungus ball, was coincidentally detected on chest computed tomography. Bronchoscopic culture eventually revealed A. xylosoxidans, which was sensitive to penicillin; however, the infection did not respond to oral antibiotics. Thus, surgical resection is likely to be indicated for A. xylosox

Mucormycosis in a Diabetic Patient: A Case Report From Georgia.

Mucormycosis is a rapidly progressive fungal infection that typically affects immunocompromised patients and diabetics. These fungi are found in soil and vegetation and can be easily inhaled by humans in day-to-day life; however, infection is rare unless specific risk factors are present. This case report presents a typical case of a 39-year-old male with poorly controlled type 1 diabetes mellitus. Initially presenting with a toothache in the upper molars, the infection quickly progressed to sinusitis, necrosis, and multiorgan failure. Based on our experience, prompt recognition of mucormycosi

A Case of Relapse Pseudomonas aeruginosa Tricuspid Valve Endocarditis After AngioVac Vegectomy and Antibiotic Treatment in a Patient Using Intravenous Drugs.

Infective endocarditis (IE) is a bloodstream infection affecting the valves of the heart. IE is highly associated with morbidity and mortality if not properly managed. Pseudomonas aeruginosa (P. aeruginosa) as a cause of IE is extremely rare. This is a case of IE involving a male patient with a history of intravenous drug use (IVDU), secondary to P. aeruginosa, with associated relapse of bacteremia and native tricuspid valve endocarditis, complicated by septic pulmonary emboli, despite undergoing recent vegetation debulking using the AngioVac system (AngioDynamics, Inc., New York, USA) along w

A new species of planthopper in the genus Anotia (Hemiptera: Auchenorrhyncha: Derbidae) from the Los Angeles cloud forest in Costa Rica.

Recent survey work in Costa Rica has resulted in the discovery of new species of derbid and cixiid planthoppers associated with palms. During this survey, one species belonging to the genus Anotia has been discovered and described, A. firebugia. A second species was collected while sweeping trailside vegetation in the Los Angeles cloud forest in Costa Rica and determined to represent a new species of Anotia. Herein, Anotia cerebro sp. n. is described and supplemental molecular data for the cytochrome c oxidase subunit I (COI), 18S rRNA gene and D9-D10 expansion region of the 28S rRNA gene are

Foreskin necrosis - Complication following self-circumcision.

Male circumcision (MC) seems to be practiced since ancient times by Muslims and Jewish, and it is considered as commandment. Attempts at self-circumcision for any reason are very rare in our country. This article aims to highlight another case of self-circumcision in a 30 year-old male, using a sharp vegetable knife. The reasons behind this act are various and the most common complications, especially in the use of sharp tools, in addition to bleeding or laceration of the penis, are infection, penile curvature, urethral injury or necrosis of the foreskin and penile tissues. To our knowledge, t

Bezoar: an uncommon cause of upper gastrointestinal bleeding.

Bezoars are aggregates of non-digestible material that accumulate in the gastrointestinal tract. They can be classified according to their composition, and phytobezoars (composed of vegetable or fruit fiber) is the most common type. They most often occur in patients with some risk factors, namely prior gastric surgery, neuropsychiatric, endocrine or other disorders that lead to abnormal gastric function, or poor gastric peristalsis. Bezoars may be asymptomatic but most commonly cause abdominal discomfort or pain, nausea, vomiting, fullness, difficulty in swallowing, or anorexia and weight loss
