The use of TachoSil for the prevention of postoperative complications after groin dissection in cases of gynecologic malignancy.

OBJECTIVE: To evaluate the effect of TachoSil in preventing postoperative complications after groin dissection performed for primary or recurrent gynecologic malignancy. METHODS: In a case-control analysis, the incidence of postoperative complications-including lymphocyst formation, wound breakdown and/or infection, and chronic lymphedema-was examined among 8 patients who received TachoSil and 16 controls (standard technique) treated for vulvar cancer or recurrent ovarian/breast cancer at San Gerardo Hospital, Monza, Italy, from 2008 to 2011. RESULTS: Thirty-eight inguinal dissections were p

[Management of mesh-related infections].

Infections of an implanted hernia mesh are a major challenge. The incidence of mesh infections after incisional hernia repair is about 1% for endoscopic techniques and can be more than 15% in open techniques. Intraoperative mesh contamination is considered to be the primary cause. All woven or knitted hernia meshes have recesses where bacteria may adhere and establish colonies. The bacterial spectrum for mesh infection includes skin pathogens, such as Staphylococcus aureus (including MRSA), Streptococcus spp., as well as E. coli, Enterococcus and Mycobacteria. The therapy approach needs to be

[Results of one-stage reconstruction of anal sphincter in surgical treatment of fistulas-in-ano combined with fecal incontinence].

The investigation included 20 patients. Mean index of the severity of incontinence before operation by the Wexner scale was 9.3+/-2.4 scores. After radical dissection of the fistula and sphincter plasty in 7 patients (the first group) the wound of the rectum mucosa was sutured in longitudinal direction, in 13 patients (the second group) the rectum wall graft was brought down to the edge of the created anal canal. Uncomplicated post-operative period was noted in 15 (75%) patients. Suppuration of the wound developed in 3 (42.9%) patients of the first group and in 2 (15.3%) patients of the second

The burden of invasive early-onset neonatal sepsis in the United States, 2005-2008.

BACKGROUND: Sepsis in the first 3 days of life is a leading cause of morbidity and mortality among infants. Group B Streptococcus (GBS), historically the primary cause of early-onset sepsis (EOS), has declined through widespread use of intrapartum chemoprophylaxis. We estimated the national burden of invasive EOS cases and deaths in the era of GBS prevention. METHODS: Population-based surveillance for invasive EOS was conducted in 4 of the Centers for Disease Control and Prevention's Active Bacterial Core surveillance sites from 2005 to 2008. We calculated incidence using state and national l

Factors associated with delay to emergency department presentation, antibiotic usage and admission for human bite injuries.

OBJECTIVES: Evidence and consensus on best practices on the management of human bite injuries is lacking. Our objective was to identify factors that are associated with delay to emergency department (ED) presentation, antibiotic usage and patient admission. METHODS: We present a retrospective chart review of adults treated for human bites. Multivariable logistic regression models used demographic characteristics and bite circumstances and characteristics as factors associated with ED presentation more than 24 hours after the bite, antibiotic usage and hospital admission. RESULTS: Of the 388

Risk factors for invasive, early-onset Escherichia coli infections in the era of widespread intrapartum antibiotic use.

OBJECTIVE: The goal was to evaluate risk factors for invasive Escherichia coli infections in the first week of life (early onset), focusing on the role of intrapartum antibiotic use. METHODS: We conducted a retrospective case-control study. Between 1997 and 2001, case infants, defined as infants < 7 days of age with E coli isolated from blood or cerebrospinal fluid, were identified in selected counties of California, Georgia, and Connecticut by the Active Bacterial Core Surveillance/Emerging Infections Program Network. Control infants (N = 1212) were identified from a labor and delivery recor

Antegrade continence enema in the treatment of obstructed defaecation with or without faecal incontinence.

BACKGROUND: Obstructed defaecation and faecal incontinence are complex functional disorders that pose management challenges. In recent times, the antegrade continence enema (ACE) has been tried both as a primary procedure and as a final resort to avoid a colostomy in patients with a variety of functional problems. The purpose of this study was to evaluate the role of the ACE procedure as a treatment option for adult patients suffering from obstructed defaecation with or without faecal incontinence, some of whom also had slow transit constipation. METHODS: Twenty female patients underwent ACE

Complicated appendicitis: is there a minimum intravenous antibiotic requirement? A prospective randomized trial.

The proper duration of postoperative intravenous (IV) antibiotics in patients suffering complicated (perforated or gangrenous) appendicitis is debatable. Some advocate a set minimum number of IV antibiotic days whereas others discontinue IV antibiotics depending on the patient's clinical course regardless of the length of therapy. Our objective was to determine whether there are differences in morbidity and resource utilization between the two treatment methodologies. Ninety-four patients with intraoperative findings of complicated appendicitis were included. In all patients IV antibiotics wer

Wild Boar-Related Injuries in Wilderness Environments: Field-Oriented Management Principles and Evidence-Informed Clinical Recommendations.

Wild boar-related trauma is uncommon but may result in severe injuries that pose diagnostic and therapeutic challenges, particularly in wilderness or resource-limited environments. Most scientific literature consists of isolated case reports, regional series, and forensic descriptions; clinical guidance is therefore fragmented. This study synthesizes two complementary evidence sources: a structured observational cohort documenting 101 injuries sustained by boar hunters during organized hunts in Germany, and a comprehensive systematic review of all globally published wild boar- and feral pig-re

Postoperative complications in patients with chemotherapy-induced acute myeloid leukemia (Review).

The present comprehensive review examines the latest clinical and translational research on postoperative complications in adult patients with chemotherapy-treated acute myeloid leukemia (AML) undergoing elective and emergency surgery. Patients with chemotherapy-treated AML face a markedly elevated risk of postoperative complications, with nearly one-half of patients experiencing complications such as severe infections and bleeding, with associated postoperative mortality rates of 10-20%. These risks are primarily driven by profound neutropenia and thrombocytopenia, impairing immune defense an

Ocular infections and stewardship practices for Ophthalmologists: The RED EYE AMSP model.

Ocular infections, ranging from conjunctivitis and keratitis to postoperative and device-associated endophthalmitis, remain a significant cause of preventable visual morbidity worldwide. In India and other low- and middle-income countries, the irrational use of topical and systemic antimicrobials, often without microbiological confirmation, has contributed to the emergence of multidrug-resistant ocular pathogens. Fluoroquinolone-resistant Pseudomonas aeruginosa , methicillin-resistant Staphylococcus aureus , and resistant Fusarium and Candida species are now major challenges in ophthalmic prac

Prevention and Management of Driveline Infections in Ventricular Assist Device Patients: The DESTINE 2.0 Staging Proposal and Expert Recommendations.

BACKGROUND: Driveline infections (DLIs) remain the predominant device-specific complication in patients supported by left ventricular assist devices (LVADs) serving as a major source of morbidity, recurrent hospitalization, and mortality. Despite their clinical impact, the prevention and early management of DLIs lack standardization, particularly within outpatient and home-based care environments. METHODS AND OBJECTIVES: The original 2020 DESTINE proposal introduced a standardized dressing protocol and a prevention-oriented staging system for driveline exit-site management. However, subsequen
