Predictors of treatment outcome in an effectiveness trial of cognitive behavioral therapy for children with anxiety disorders.

A substantial number of children with anxiety disorders do not improve following cognitive behavioral therapy (CBT). Recent effectiveness studies have found poorer outcome for CBT programs than what is typically found in efficacy studies. The present study examined predictors of treatment outcome among 181 children (aged 8-15 years), with separation anxiety, social phobia, or generalized anxiety disorder, who participated in a randomized, controlled effectiveness trial of a 10-session CBT program in community clinics. Potential predictors included baseline demographic, child, and parent factor

Trait rumination, depression, and executive functions in early adolescence.

Although deficits in executive functions have been linked with both depression and rumination in adulthood, the nature of the relationship between these constructs is not well understood and remains understudied in adolescence. The present study examined the relationship of rumination and depression to deficits in executive functions in early adolescence, a critical developmental period for the emergence of depression and rumination and the development of executive functions. Participants were 486 early adolescents (52.7% female; 47.1% African American, 48.8% Caucasian; 4.2% Biracial/Multiraci

Suicide and war: the mediating effects of negative mood, posttraumatic stress disorder symptoms, and social support among army National Guard soldiers.

The mediating effects of posttraumatic stress disorder (PTSD) symptoms, negative mood, and social support on the relationship of war experiences to suicidality were examined. The research literature suggested a sequence among study scales representing these constructs, which was then tested on survey data obtained from a sample of National Guard soldiers (N=4,546). Results from structural equation modeling suggested that war experiences may precipitate a sequence of psychological consequences leading to suicidality. However, suicidality may be an enduring behavioral health condition. War exper

Source-specific oppositional defiant disorder: comorbidity and risk factors in referred elementary schoolboys.

OBJECTIVE: To examine differences in risk factors and comorbid conditions for oppositional defiant disorder (ODD) symptom groups in a sample of 248 elementary schoolboys (ages 6-10) recruited from 1994 to 1996. METHOD: The boys and their mothers received multiple assessments of cognitive, behavioral, academic, and family functioning, including a clinic-based evaluation in Stony Brook, NY. ODD was defined using four different strategies for aggregating data from mother and teacher reports of DSM-IV symptoms. RESULTS: Source-specific ODD symptom groups had better internal validity and were mor

Linking Brain, Heart, and Behavior in Children: Frontal Electroencephalogram Activity, Respiratory Sinus Arrhythmia, and Internalizing Behaviors.

Internalizing behaviors are characterized by inward-focused emotional distress, which often manifests as symptoms such as depression, anxiety, and negative affect. Previous research has found that physiological correlates of heightened emotional arousal, such as phasic changes in overall frontal electroencephalogram (EEG) alpha activity (FAA) and respiratory sinus arrhythmia (RSA), are independently associated with emotion dysregulation and internalizing behaviors in children. The present study examined whether the interaction of changes in overall FAA and changes in RSA from baseline to a str

Brief report: aggression and stereotypic behavior in males with fragile X syndrome--moderating secondary genes in a "single gene" disorder.

Although fragile X syndrome (FXS) is a single gene disorder with a well-described phenotype, it is not known why some individuals develop more significant maladaptive behaviors such as aggression or autistic symptoms. Here, we studied two candidate genes known to affect mood and aggression, the serotonin transporter (5-HTTLPR) and monoamine oxidase A (MAOA-VNTR) polymorphisms, in 50 males with FXS ages 8-24 years. Mothers and fathers of participants reported the frequency and severity of aggressive/destructive, self-injurious, and stereotypic behaviors. Polymorphism genotypes were unrelated to

[Clinical characteristics and pharmacotherapy of extremely disruptive behavior disorders in people with mental retardation].

People with mental retardation often have behavioral problems. Psychotropic medications are used for the treatment of extremely disruptive behavior disorders in many cases. We surveyed the clinical characteristics and the current status of the pharmacotherapy regimens for inpatients with severe intellectual disabilities and behavior disorders at 8 national psychiatric hospitals. Many of them were men between 20 and 40 years and more than half of them had both extremely mental retardation and high motor ability. Antipsychotic drugs and anticonvulsants were frequently used. As they had more seve

Structural correlates of auditory verbal hallucinations in patients with borderline personality disorder.

BACKGROUND: Borderline personality disorder (BPD) is a severe psychiatric condition marked by disturbances in self-image, affect regulation, and interpersonal functioning. Up to 54% of individuals with BPD experience psychotic symptoms, particularly auditory verbal hallucinations (AVH). While AVH's neural correlates have been studied in schizophrenia (SZ), their structural basis in BPD remains poorly understood within a transdiagnostic framework. METHODS: This cross-sectional study used voxel-based morphometry (VBM) on MRI data to assess gray matter volume (GMV) in BPD patients with AVH (n =

Symptom management and transitioning to engagement with post-treatment care for adolescent and young adult cancer survivors (AYA STEPS): Study protocol.

For adolescent and young adult (AYA) cancer survivors, treatment impacts are significant and long-lasting, including physical symptoms (e.g., pain, fatigue), emotional distress (e.g., anxiety, depression), and increased risk for long-term health problems (e.g., second cancers, early-onset cardiovascular disease). After cancer therapy, many AYAs lose contact with their oncology team and fail to establish care with providers knowledgeable of their risks and continued care needs. Consequently, AYAs experience inadequate surveillance for late effects and missed opportunities for symptom management

Deceleration Capacity as a Marker of Autonomic Cardiac Modulation in Prodromal and Manifest Parkinson's Disease, Multiple System Atrophy, and Progressive Supranuclear Palsy.

BACKGROUND: Degenerative parkinsonian syndromes, including the alpha-synucleinopathies (aSYN) Parkinson's disease (PD), and multiple system atrophy (MSA), and the tauopathy progressive supranuclear palsy (PSP), are characterized by motor and non-motor symptoms. The later subsume autonomic dysfunction, which may appear early or progress with the disease. Cardiac dysfunction varies by syndrome and can also occur in isolated REM sleep behavior disorder (iRBD), a prodromal stage of aSYN. Overlapping motor features make early differentiation challenging. Heart rate variability (HRV) analysis is a n

Assessment of a Clinical Trial Metric for Rett Syndrome: Critical Analysis of the Rett Syndrome Behavioural Questionnaire.

BACKGROUND: Rett syndrome is a neurodevelopmental disorder with potential for improvement through novel targeted therapeutics. Reliable outcome measures are critical to the development of treatments. We examined the merits and flaws of the Rett Syndrome Behavioural Questionnaire, an outcome measure for clinical trials. METHODS: The Rett Syndrome Behavioural Questionnaire was administered alongside other clinical scales in three cohorts, an online survey, a clinic-based study, and the screening period for a clinical trial. Data were collected from individuals with Rett syndrome and related dis

Development of a Composite Primary Outcome Score for Children with Attention-Deficit/Hyperactivity Disorder and Emotional Dysregulation.

Objective: Study goals were to (1) provide a rationale for developing a composite primary outcome score that includes symptom severity for attention-deficit/hyperactivity disorder (ADHD) and emotional dysregulation, plus symptom-induced impairment; (2) demonstrate weighting methods to calculate the composite score using a sample of children diagnosed with ADHD and aggression; and (3) identify the optimal weighting method most sensitive to change, as measured by effect sizes. Methods: We conducted secondary data analyses from the previously conducted Treatment of Severe Childhood Aggression (TO

Moderation of prior exposure to trauma on the inverse relationship between callous-unemotional traits and amygdala responses to fearful expressions: an exploratory study.

BACKGROUND: Previous work has shown that amygdala responsiveness to fearful expressions is inversely related to level of callous-unemotional (CU) traits (i.e. reduced guilt and empathy) in youth with conduct problems. However, some research has suggested that the relationship between pathophysiology and CU traits may be different in those youth with significant prior trauma exposure. METHODS: In experiment 1, 72 youth with varying levels of disruptive behavior and trauma exposure performed a gender discrimination task while viewing morphed fear expressions (0, 50, 100, 150 fear) and Blood Oxy

Prodromal Parkinsonism and Neurodegenerative Risk Stratification in REM Sleep Behavior Disorder.

OBJECTIVES: Rapid eye movement (REM) sleep behavior disorder (RBD) is the most specific marker of prodromal alpha-synucleinopathies. We sought to delineate the baseline clinical characteristics of RBD and evaluate risk stratification models. METHODS: Clinical assessments were performed in 171 RBD, 296 control, and 119 untreated Parkinson's (PD) participants. Putative risk measures were assessed as predictors of prodromal neurodegeneration, and Movement Disorders Society (MDS) criteria for prodromal PD were applied. Participants were screened for common leucine-rich repeat kinase 2 (LRRK2)/glu

Early Huntington's Disease: Impulse Control Deficits but Correct Judgment Regarding Risky Situations.

BACKGROUND: Huntington's disease (HD) patients show alterations in decision making tasks. However, it is still uncertain if these deficits are due to poor judgment regarding risky situations, or to impulse control deficits. OBJECTIVE: To elucidate whether decision-making in patients is related to genuine risk behavior or to impulse control deficits. METHODS: To test between these two alternative possibilities, we evaluated the performance of 19 prodromal HD patients and 19 matched healthy controls in the Cambridge Gambling Task (CGT). This task assesses decision-making while dissociating bet

Is Emotional Lability Distinct From "Angry/Irritable Mood," "Negative Affect," or Other Subdimensions of Oppositional Defiant Disorder in Children With ADHD?

OBJECTIVE: Emotional lability (EL) and oppositional defiant disorder (ODD) frequently co-occur with ADHD. This study evaluates whether EL merely represents the negative "mood/affect" component of ODD or forms a distinct dimension. METHOD: EL and ODD data from 1,317 ADHD participants were analyzed using exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) for binary data. RESULTS: Within ADHD, 39.4% children had ODD and 42.6% had EL. A sizable proportion expressed only either ODD or EL: 16.6% had ODD-only, 19.7% had EL-only, and 22.9% expressed both. In both EFA and CFA, E
