Basal encephalocele associated with morning glory syndrome: case report.

The basal encephaloceles refer to rare entities and they correspond to herniation of brain tissue through defects of skull along the cribiform plate or the sphenoid bone. A rare morning glory syndrome, with characteristic retinal defect has been reported in association with basal encephaloceles. Hypophysis hormonal deficiencies may occur. We accounted for a pituitary dwarfism with delayed diagnosed transsphenoidal encephalocele associated with morning glory syndrome, showing the alterations found in retinography, computed tomography and magnetic resonance imaging.

Prophylactic root resection and periapical grafting for anterior implant aethetics: an integration of tissue-reconstruction and tissue-preservation concepts.

The presence of active infections involving the labial bony plate of a failing maxillary anterior tooth presents a challenging situation for aesthetic implant replacement. Not only would the septic state of an abscess be detrimental for immediate implant placement and bone augmentation, but the accompanying bony destruction would also increase the likelihood and magnitude of gingival recession, making this a bioaesthetic challenge. This article describes a technique that integrates tissue-reconstruction and tissue-preservation concepts to achieve peri-implant tissue aesthetics as well as its c

Radial clubhand-like deformity resulting from osteomyelitis of the distal radius.

Radial clubhand is an uncommon congenital deformity in which a part of or a whole radius is absent. This is a report on two cases of radial clubhand-like deformity resulting from osteomyelitis of the distal radius in which the distal radial metaphysis was destroyed and resorbed, resulting in a defect in that part. This is a very rare condition. The patients were successfully treated by cancellous bone graft and plating, combined with ulna shortening. At follow-up after 1 year, both patients showed good functional and cosmetic results.

Chondrocalcin and the calcification of cartilage. A review.

A protein recently identified and named chondrocalcin is associated with the calcification of hyaline cartilage matrix in health and disease. Chondrocalcin is absent from noncalcifying growth plate cartilages in disease. It is also present in noncalcifying developing epiphyseal cartilage. The molecule appears to be synthesized and secreted by chondrocytes, particularly those committed to becoming hypertrophic. It is not detectable in developing or mature bone, teeth, or other tissues, although trace amounts are present in blood. The assigned molecular weight is 69,000. The protein is basic and

Circadian variations in the committed stem cell responsiveness to granulopoiesis inhibitory factor (chalone) in vitro.

The effect of inhibitory extracts of human granulocytes (chalone) on the circadian rhythm of myelopoietic colony formation in soft agar (CFUc) was studied over one year. Normally, the number of bone marrow CFUc shows a strong circadian rhythm. Preincubation of the cells with the extracts before plating modulated this rhythm with phase shifts and modifications of amplitude. The effects of the extracts oscillated between significant inhibition and stimulation over a 24 h period. No significant seasonal variations in the inhibitory effects were found.

[Diagnostics and therapy of fractures and fracture-dislocations of the vertebral column (author's transl)].

Vertebral fractures with incomplete paraplegia should be reduced immediately. If the myelogram still shows compression of the medulla, an anterior decompression should be performed. All severe injuries to the cervical spine are stabilised with anterior bone block and plate osteosynthesis. Generally fractures of the lumbar spine are treated conservatively with reduction and plaster jacket. Operative treatment is necessary in dislocations with locked facets, secondary instability, and in cases with traumatic spondylolisthesis. Reduction and stabilisation can be performed either by interbody fusi

Fibular autografts for distal defects of the radius.

A protocol for osteoarticular grafting was established to avoid fracture, nonunion, and loss of motion when replacing the distal radius. Proximal fibular autografts were used and stabilized proximally by compression plating and, at the wrist, by ligamentous reconstruction. Postoperative splinting and therapy were coordinated with graft healing, which was monitored by bone scans and roentgenograms. Graft incorporation in three patients appeared to be well-established within 1 year, but functional use of the extremity and return to duty were achieved much earlier.

Predicting osteosynthesis screw failure by peri-implant bone morphology in multiple loading conditions.

Osteosynthesis screws are critical in orthopaedic surgery for stabilizing and aligning bone fracture fragments. Despite their importance, screw failure remains a significant complication, often due to excessive movement at the implant-bone interface resulting from both physiological loading and external mechanical forces. This study aims to enhance understanding of screw failure mechanisms by investigating the relationship between peri-implant CT-based trabecular bone morphology and screw failure under axial-, shear-, and mixed loading conditions, including the effect of plate elevation. Using

BMP-6 and BMPR-1a are up-regulated in the growth plate of the fractured tibia.

Bone overgrowth is a known phenomenon occurring after fracture of growing long bones with possible long-term physical consequences for affected children. Here, the physeal expression of bone morphogenetic proteins (BMPs) was investigated in a fracture-animal model to test the hypothesis that a diaphyseal fracture stimulates the physeal expression of these known key regulators of bone formation, thus stimulating bone overgrowth. Sprague-Dawley rats (male, 4 weeks old), were subjected to a unilateral mid-diaphyseal tibial fracture. Kinetic expression of physeal BMP-2, -4, -6, -7, and BMP recepto

Lateral Collateral Ligament Reconstruction Using the Triceps Brachii Fascia for Posterolateral Rotatory Instability in Cubitus Varus: A Report of Two Cases.

A combination of osteotomy and ligament reconstruction is recommended for posterolateral rotatory instability (PLRI) with large cubitus varus deformities. There is a lack of reports regarding ligament donor selection for ligament reconstruction of PLRI with cubitus varus. Two cases of PLRI with cubitus varus have been described. In case one, a 40-year-old woman presented with left elbow pain. She had a cubitus varus deformity, resulting from a childhood elbow fracture. Radiographs showed an 18-degree cubitus varus deformity. A lateral closing wedge osteotomy and double plate osteosynthesis wer

Spatial distribution of elements during osteoarthritis disease progression using synchrotron X-ray fluorescence microscopy.

The osteochondral interface is a thin layer that connects hyaline cartilage to subchondral bone. Subcellular elemental distribution can be visualised using synchrotron X-ray fluorescence microscopy (SR-XFM) (1 μm). This study aims to determine the relationship between elemental distribution and osteoarthritis (OA) progression based on disease severity. Using modified Mankin scores, we collected tibia plates from 9 knee OA patients who underwent knee replacement surgery and graded them as intact cartilage (non-OA) or degraded cartilage (OA). We used a tape-assisted system with a silicon nitride

Antenatal dexamethasone retarded fetal long bones growth and development by down-regulating of insulin-like growth factor 1 signaling in fetal rats.

OBJECTIVE: Dexamethasone (DEX), a synthetic glucocorticoid, has been widely used as a medication for premature delivery. However, the side effects of antenatal DEX treatment on fetal bone development, as well as the underlying mechanisms still remain to be elucidated. Here, we aimed to explore the effects and the related mechanisms of antenatal DEX exposure during late pregnancy on fetal bone growth and development. METHODS: Pregnant Sprague-Dawley rats were randomly divided into DEX group and vehicle group from gestational day 14 (GD14). Pregnant rats in DEX group were intraperitoneally inje

Modified fixation for periprosthetic supracondylar femur fractures: Two case reports and review of the literature.

BACKGROUND: Periprosthetic supracondylar femoral fractures (PSFs) present a challenge in terms of optimizing fixation in patients with poor bone quality. Surgical treatment and peri-operative management of PSFs in the elderly remain a burden for orthopedic surgeons. Among different treatment options, locking plate (LP) and retrograde intramedullary nail (RIMN) have shown favorable results. However, reduced mobility and protected weight-bearing are often present in the postoperative older population. With a purpose of allowing for early weight-bearing, a modified nail plate combination (NPC) wa

Massive bone allograft engineered with autologous vessels: A new perspective for the future.

Reconstruction is very important to ensure good function and quality of life after bone tumor resection. For metacarpals and phalanges, amputation and toe transfer are the gold-standard indications; nevertheless, allograft reconstruction must also be taken into account. Unfortunately, because of its inert biological behavior, it undergoes progressive resorption, with frequent fracture. Several attempts have been made to induce new vascularization in massive bone allograft, with poor results. However, neo-angiogenesis was reported with vascular loops, and we therefore hypothesized that heterolo

External Locking Plate Fixation for Femoral Subtrochanteric Fractures.

INTRODUCTION: Internal fixation is the treatment of choice for subtrochanteric fractures in most conditions. However, it may be an unsuitable procedure for patients with poor health status, osteomyelitis, and surrounding soft tissue compromise. This study aimed to ascertain the viability and reliability of using external locking plate fixation for these difficult cases. METHODS: Eleven patients with femoral subtrochanteric fractures who received external locking plate fixation in our institute from January 2014 to December 2019 were enrolled in our study. The bone union time, wound complicati
