Experiments carried out in pregnant rats show that combination of nifedipine with verapamil or partusystene (in a half dose) depresses contractility in a more pronounced extent than of these drugs in the case of separate application. The effects of several representative calcium channel antagonists on depolarization-evoked release of [3H]-norepinephrine were investigated in isolated, superfused rabbit iris-ciliary bodies. The clinical effects of terodiline hydrochloride were evaluated in forty-four patients with urinary frequency and/or sense of residual urine. After a four-week treatment with 24 mg terodiline hydrochloride once daily, subjective symptoms were improved and micturition frequency was decreased significantly. Slight dysuria was reported as a side effect only in one patient. Terodiline hydrochloride is considered to be superiorly effective and safe for the treatment of urinary frequency and sense of residual urine.

Restenosis after percutaneous transluminal coronary angioplasty (PTCA) remains the principal factor preventing broader application of this form of myocardial revascularization. Although approximately 500,000 patients suffer from a stroke each year in the United States, treatment of these patients to date has consisted primarily of prevention, supportive measures, and rehabilitation. The modification of experimental cerebral infarction by new pharmacologic agents, along with encouraging results from the restoration of blood flow to areas of focal ischemia in both laboratory and clinical trials, suggests that a more aggressive approach might be considered in selected patients with acute stroke.

Secondary pulmonary hypertension is seen in disorders that either interfere with gas exchange within the lung or in which large portions of the pulmonary vasculature are obliterated. Because the causes are diverse, there is no uniform approach to therapy. The approach to treatment should be methodic, but aggressive and primarily directed at improving the underlying disease process. Other forms of therapy, which are more controversial, should be reserved for patients with refractory cor pulmonale and should be instituted cautiously and their effects monitored closely.

Anti-ischaemic effects and safety of nicardipine were assessed in 14 patients with vasospastic angina using a placebo comparison, cross-over design study for 8-13 weeks. The average daily dose of nicardipine for optimal angina prevention was 84 mg (range 40-160 mg). Nicardipine administration, as compared with placebo, significantly reduced anginal frequency and nitroglycerin consumption during the single- and double-blind phases of the study. Nicardipine appears to be effective in the prevention of vasospastic angina and not to cause major adverse effects.
