Utility of bone marrow examination for workup of fever of unknown origin in patients with HIV/AIDS.

AIMS: The utility of bone marrow aspiration and biopsy (BMAB) as a diagnostic tool in patients with HIV/AIDS and fever of unknown origin (FUO) is a subject of debate. Because highly active antiretroviral therapy has reduced incidence of opportunistic infections, it is important to reassess the efficacy of BMAB for this diagnostic purpose. To our knowledge, no such studies have been performed in Harris County which has the highest incidence of HIV in the state of Texas. METHODS: We reviewed all BMABs from patients with HIV/AIDS and FUO or persistent cytopenia(s) from 2007 to 2011. RESULTS: Of

Dual-energy CT for the diagnosis of gout: an accuracy and diagnostic yield study.

OBJECTIVES: To assess the accuracy of dual-energy CT (DECT) for diagnosing gout, and to explore whether it can have any impact on clinical decision making beyond the established diagnostic approach using polarising microscopy of synovial fluid (diagnostic yield). METHODS: Diagnostic single-centre study of 40 patients with active gout, and 41 individuals with other types of joint disease. Sensitivity and specificity of DECT for diagnosing gout was calculated against a combined reference standard (polarising and electron microscopy of synovial fluid). To explore the diagnostic yield of DECT sca

Predicting response to cognitive therapy and interpersonal therapy, with or without antidepressant medication, for major depression: a pragmatic trial in routine practice.

BACKGROUND: Identifying patient characteristics that predict response within treatments (prognostic) or between treatments (prescriptive) can inform clinical decision-making. In this study, we sought to identify predictors of response to evidence-based treatments in a sample of depressed patients seeking help in routine practice. METHODS: Data come from a pragmatic trial of 174 patients with major depression who received an evidence-based treatment of their own choice: cognitive therapy (CT), interpersonal therapy (IPT), antidepressant medication (ADM) alone or in combination with either of t…

Tympanic membrane retraction pocket staging: is it worthwhile?

Our objectives were to review all reported staging systems of tympanic membrane (TM) retraction pockets (RP) and to report their reliability and utility to our daily clinical practice in terms of follow-up and decision making in the management of RP. We aim to propose a new management algorithm of TMRPs. We conducted a thorough research on Ovid Medline, Pubmed and Cochrane databases for English and French languages studies published between 1963 and 2012 on the retraction pocket. Studies were excluded if it were a short comments, photo clinical cases, experimental studies or round table articl

To CADe or not to CADe - evidence for the benefit of CAD for increased yield of colonoscopy.

Computer -Aided Detection (CADe) has emerged as a promising tool for enhancing colonoscopy during colorectal cancer screening. This manuscript critically examines the evidence supporting CADe's role in improving the yield of colonoscopy, focusing on its impact on lesion detection, miss rates, and real-time decision-making, especially for less experienced endoscopists. We review relevant clinical trials, studies, and meta-analyses to provide a comprehensive assessment of CADe's impact on detection rates, diagnostic accuracy, cost-effectiveness, colonoscopy training, reimbursement and patient ou

Role of microbiological tests and biomarkers in antibiotic stewardship.

Laboratory tests are critical in the detection and timely treatment of infection. Two categories of tests are commonly used in neonatal sepsis management: those that identify the pathogen and those that detect host response to a potential pathogen. Decision-making around antibiotic choice is related to the performance of tests that directly identify pathogens. Advances in these tests hold the key to progress in antibiotic stewardship. Tests measuring host response, on the other hand, are an indirect marker of potential infection. While an important measure of the patient's clinical state, in t

Ethical dilemmas and intervention strategies in doctor-patient-family communication in end-of-life pediatric oncology: a narrative review.

BACKGROUND AND OBJECTIVE: Communication in end-of-life pediatric oncology involves ethically sensitive exchanges among healthcare providers, children, and families. The main dilemmas include how to disclose prognosis without abandoning hope, how to balance parents' surrogate authority with the child's evolving participation, and how to align treatment decisions with family values, developmental capacity, and the child's best interests. This narrative review clarifies the conceptual scope of these dilemmas, synthesizes evidence on their psychological, institutional, and sociocultural determinan

Seeing through "brain fog": neuroimaging assessment and imaging biomarkers for cancer-related cognitive impairments.

Advances in cancer diagnosis and treatment have substantially improved patient outcomes and survival in recent years. However, up to 75% of cancer patients and survivors, including those with non-central nervous system (non-CNS) cancers, suffer from "brain fog" or impairments in cognitive functions such as attention, memory, learning, and decision-making. While we recognize the impact of cancer-related cognitive impairment (CRCI), we have not fully investigated and understood the causes, mechanisms and interplays of various involving factors. Consequently, there are unmet needs in clinical onc

An AI model classifies risks of early relapse post-CAR T-cell therapy in a multicenter real-world population with DLBCL.

Accumulating real-world (RW) evidence of axicabtagene ciloleucel (axi-cel) has demonstrated comparable performance to that of pivotal trials. However, ∼57% of patients eventually relapse, with most requiring additional therapies. Being able to identify patients with risk of early relapse enables clinicians to consider additional interventions to extend survival outcomes. This study aimed to first comprehensively evaluate the RW performance of axi-cel in the multicenter University of California Health Systems using automated computational approaches. Second, we developed a decision tree machine

Improved classifications of myocardial bull's-eye scintigrams with computer-based decision support system.

UNLABELLED: In a recent study, artificial neural networks were trained to detect coronary artery disease using scintigraphic data as input. The performance of the networks was better than that of human experts using coronary angiography as a gold standard. In clinical practice, this type of neural networks will not take over the decision-making process from the physician but will assist by proposing an interpretation of the scintigram. The purpose of this study was to assess the influence of such decision support on the interpretations of the physicians. METHODS: A population of 135 patients

Integration of dual-energy CT characteristics and biomarkers: Noninvasive prediction of Ki-67 expression in pancreatic ductal adenocarcinoma.

BACKGROUND: The Ki-67 proliferation index is a critical prognostic marker in pancreatic ductal adenocarcinoma (PDAC); however, its assessment relies on invasive tissue sampling. Ki-67 expression reflects active tumor cell proliferation and is associated with aggressive tumor behavior. A preoperative, noninvasive method to predict Ki-67 status would therefore be valuable for clinical decision-making. Dual-energy CT (DECT) can provide quantitative parameters related to tumor vascularity and composition, potentially reflecting proliferative activity. Additionally, clinical biomarkers such as CA12…

[Normal pressure hydrocephalus in an elderly patient].

INTRODUCTION: Idiopathic normal pressure hydrocephalus is a disease that most frequently concerns subjects of an advanced age. It is not easy to establish a definitive diagnosis, and the practitioner frequently encounters decision-making problems regarding the following question: should a shunt be performed, or not? Opinions remain contradictory, even though the available scientific data is increasingly precise. The aim of this review is to analyze the physiopathogenic, clinical, paraclinical and therapeutic data associated with this type of hydrocephalus. CURRENT KNOWLEDGE AND KEY POINTS: If
