Laminectomy and posterior cervical plating for multilevel cervical spondylotic myelopathy and ossification of the posterior longitudinal ligament: effects on cervical alignment, spinal cord compression, and neurological outcome.

OBJECTIVE: Multilevel anterior decompressive procedures for cervical spondylotic myelopathy or ossification of the posterior longitudinal ligament may be associated with a high incidence of neurological morbidity, construct failure, and pseudoarthrosis. We theorized that laminectomy and stabilization of the cervical spine with lateral mass plates would obviate the disadvantages of anterior decompression, prevent the development of kyphotic deformity frequently seen after uninstrumented laminectomy, decompress the spinal cord, and produce neurological results equal or superior to those achieved

Awake full-endoscopic transforaminal lumbar interbody fusion via the trans-Kambin approach under local anaesthesia in a nonagenarian.

Degenerative lumbar spine disease is common in the elderly, but spinal fusion is often avoided in patients over 90 years due to perceived risks of surgical or anaesthesia-related complications. Full-endoscopic transforaminal lumbar interbody fusion (FE-TLIF) via the trans-Kambin approach offers a minimally invasive alternative that can be performed without general anaesthesia. We report a nonagenarian patient with recurrent symptomatic foraminal stenosis at L4-L5 after prior decompression surgeries who underwent FE-TLIF under local anaesthesia. The procedure was well tolerated, with significan

Feasibility of the Minimally Invasive Lumbar Decompression Procedure in a Lumbar Stenosis Patient With Radiographic Evidence of Spinal Instability.

Lumbar spinal stenosis (LSS) can be challenging to treat in certain patient populations, particularly in patients for whom medical management is ineffective and surgical interventions carry a high risk of complications. This case report describes an 83-year-old woman with rheumatoid arthritis and LSS who presented with neurogenic claudication. Diagnosis was confirmed by physical examination and imaging, revealing canal stenosis and lumbar instability. Conservative measures failed to improve her symptoms, and she was deemed a poor surgical candidate given her age, advanced arthritis, and her cu

Brachioradial Pruritus Caused by Cervical Disc Herniation Precipitated by Trauma Treated with Anterior Cervical Discectomy and Fusion: Report of Two Cases and Review of the Literature.

Brachioradial pruritis (BRP) is a rare form of dermatomal pruritis that appears to be caused both by cervical radiculopathy and exposure to ultraviolet-light, although the exact pathophysiology for the manifestation of these symptoms remains to be determined. A diagnosis of BRP is typically confirmed with the "ice-pack" test and evidence of cervical spine pathology using magnetic resonance imaging. Treatment options consist of application of ice, reduction in sun exposure, and topical capsaicin, antiepileptics, or tricyclic antidepressants. Patients with refractory symptoms and cervical spine

Intradural extramedullary tuberculoma in a case of disseminated tuberculosis: A case report.

BACKGROUND: Intradural extramedullary tuberculoma of the spinal cord (IETSC) is an exceedingly rare manifestation of tuberculosis (TB) affecting the central nervous system. CASE DESCRIPTION: A 33-year-old immunocompetent female with disseminated TB, including pulmonary involvement and leptomeningeal tuberculomas, developed progressive paraplegia and urinary incontinence over 2 months. Magnetic resonance imaging revealed diffuse intradural extramedullary soft tissue from C7 to L2 vertebral levels, indicative of abscess formation and severe spinal cord compression. The surgical intervention inv

Application of Machine Learning in Developing Decision-Making Support Models for Decompressed Vertebroplasty.

BACKGROUND: The common treatment methods for vertebral compression fractures with osteoporosis are vertebroplasty and kyphoplasty, and the result of the operation may be related to the value of various measurement data during the operation. MATERIAL AND METHOD: This study mainly uses machine learning algorithms, including Bayesian networks, neural networks, and discriminant analysis, to predict the effects of different decompression vertebroplasty methods on preoperative symptoms and changes in vital signs and oxygen saturation in intraoperative measurement data. RESULT: The neural network s

Lumbar Interbody Fusion and Osteobiologics for Lumbar Fusion.

Lumbar interbody fusion (LIF) is an excellent treatment option for a number of lumbar diseases. LIF can be performed through posterior, transforaminal, anterior, and lateral or oblique approaches. Each technique has its own pearls and pitfalls. Through LIF, segmental stabilization, neural decompression, and deformity correction can be achieved. Minimally invasive surgery has recently gained popularity and each LIF procedure can be performed using minimally invasive techniques to reduce surgery-related complications and improve early postoperative recovery. Despite advances in surgical technolo

Ulnar nerve entrapment in Guyon's canal caused by a ganglion cyst: two case reports and review of the literature.

In this article, two cases of ulnar nerve neuropathy in Guyon's canal caused by a ganglion cyst are reported. A review of the literature found 73 cases reported in 35 articles of ulnar neuropathy at the wrist secondary to a ganglion cyst. We investigated the pathogenesis of ganglion cysts, which are separated as extraneural and intraneural. At this anatomical area, there are a variety of clinical factors that can also lead to ulnar nerve compression such us trauma, fracture of the hamate's hook, arterial thrombosis, tumors and other anatomical variations. The symptoms may include weakness, par

Combined site-specific sacral neuromodulation and pudendal nerve release surgery in a patient with interstitial cystitis and persistent arousal.

A variety of neuromodulation approaches have been described for the management of pelvic neuropathies, including interstitial cystitis, pudendal neuralgia and persistent genital arousal disorder. The benefits of a combined sacral and pudendal nerve neuromodulator has yet to be explored for these patients. In this report, we describe the case of a 35-year-old woman with a complex pelvic neuropathy resulting in urinary, sexual and gastro-intestinal dysfunction. She presented with an established diagnosis of interstitial cystitis; however, she also fulfilled diagnostic criteria for pudendal neura

Bifid median nerve in carpal tunnel syndrome: do we need to know?

INTRODUCTION: We tested the hypothesis that a bifid median nerve predisposes to development of carpal tunnel syndrome (CTS) and investigated differences in electrophysiological findings and outcome. METHODS: A total of 259 consecutive patients with clinically defined CTS were included and investigated clinically, electrophysiologically, and ultrasonographically. Fifty-four healthy asymptomatic volunteers were investigated ultrasonographically. RESULTS: The prevalence of bifid median nerves is equal in patients with CTS and controls. Electrophysiological and ultrasonographic abnormalities are

Unusual spinal metastases from an adenoid cystic carcinoma of the maxillary sinus.

Adenoid cystic carcinoma (ACC), the second most common cancer occurring in the sinonasal tract, is an aggressive malignancy with a poor five-year survival rate. Spinal metastases to the vertebral column related to this cancer are rare. This report presents a patient with maxillary sinus carcinoma with vertebral metastases at the thoracic level and compression of the spinal cord seven years after surgical resection of the primary tumor. Eleven years after detection of the primary tumor the patient is still able to walk. The role of decompression and/or fusion in spinal metastases with neurologi

Ludwig's Angina - An emergency: A case report with literature review.

Ludwig's angina is a form of severe diffuse cellulitis that presents an acute onset and spreads rapidly, bilaterally affecting the submandibular, sublingual and submental spaces resulting in a state of emergency. Early diagnosis and immediate treatment planning could be a life-saving procedure. Here we report a case of wide spread odontogenic infection extending to the neck with elevation of the floor of the mouth obstructing the airway which resulted in breathlessness and stridor for which the patient was directed to maintain his airway by elective tracheostomy and subsequent drainage of the

Surgical strategy for intracranial endodermal cyst--case report.

Two cases of endodermal cyst of the posterior fossa are reported. A 12-year-old girl presented with severe headache and vomiting caused by increased intracranial pressure. Computed tomography and magnetic resonance (MR) imaging showed a cystic mass occupying the ambient and quadrigeminal cisterns. A 65-year-old woman presented with dizziness, and MR imaging revealed a cystic mass in the posterior fossa. The two patients underwent surgery for decompression and resection of the cyst. Surgical specimens of the cyst walls consisted of a single layer of ciliated columnar epithelium. The diagnoses w

[Diagnostic value of CT discography in unclear radiculopathy].

A clinically evident radiculopathy without correlation in the imaging studies represents a serious problem regarding the indication, planning and execution of an operative procedure for its treatment. Both the diagnosis and treatment of such cases are deemed to be difficult without clear morphological correlation. Moreover, the surgeon lacks an important basis for the adequate planning and above all the justification of surgical treatment. Although discography with post-discographic computer tomography (CT discography) is still controversially discussed as an invasive diagnostic measure, the l

Pedicle subtraction osteotomy for adult tethered cord syndrome with lumbar canal stenosis: report of two cases.

Tethered cord syndrome with spinal lipoma is the most common form of occult spinal dysraphism. For the symptomatic patients, surgical treatment is recommended; however, there are many patients who have not been encouraged to seek medical attention until adulthood, since their symptoms are not severe enough to interfere with their daily activities. We performed pedicle subtraction osteotomy (PSO) to achieve indirect untethering and neural decompression in two senior patients with tethered cord syndrome, who showed deteriorating neurological condition due to coexisting lumbar canal stenosis. Nutcracker syndrome: when should it be treated and how?

Nutcracker syndrome refers to compression of the left renal vein by the superior mesenteric artery and aorta. Patients typically present with left flank pain and associated symptoms of pelvic congestion. Hematuria is frequently present, and vulvar or lower extremity varices are seen in a subset of patients. Clinical suspicion of the syndrome is confirmed by duplex scanning, computerized tomography, or magnetic resonance imaging. Documentation of a hemodynamically significant pressure gradient across the point of compression during venographic assessment in patients with appropriate symptom sev

Facial nerve paralysis: A three year retrospective study.

Of all the cranial nerves, the facial nerve is the one which is most commonly involved in disease. Facial paralysis leaves the patient severely disfigured. Timely diagnosis and treatment can lead to considerable recovery. 16 consecutive patients of facial paralysis of all age groups and due to different causes diagnosed and treated in a tertiary referral hospital have been studied retrospectively. The frequency of aetiological factors, the various factors governing the management of these cases and their actual outcomes after a minimum period of one year are discussed. The causes of facial ner

Venous angioma adjacent to the root entry zone of the trigeminal nerve: implications for management of trigeminal neuralgia.

Detection of a venous angioma at the root entry zone is important for surgical planning, so that the neurosurgeon will be aware that both veins and arteries may require microvascular decompression. In selected cases, alternative treatment may be indicated to avoid the potential surgical complication of a venous infarct. Trigeminal neuralgia typically occurs in the middle-aged to elderly population, usually the result of compression of the trigeminal nerve at its root entry zone by an ectatic, aging artery or, less commonly, a regional vein [1, 2, 3]. When associated with a venous angioma at th

Permanent postoperative anosmia: a serious complication of neurovascular decompression in the sitting position.

In posterior fossa surgery, the sitting position offers a number of advantages believed to outweigh complications such as air embolism and pneumatocephalus. For this reason, the sitting position is frequently used in neurovascular decompression for trigeminal neuralgia. Two years ago we reported on a previously undescribed complication: permanent postoperative anosmia. Following the recent occurrence of a second case, we conducted a nationwide survey to determine the frequency of this complication. Permanent postoperative anosmia following surgical procedures in the sitting position has been o
