Study: observation is a good strategy when caring for children who present with minor blunt head trauma.

A new study bolsters earlier findings that children who present to the ED with minor blunt head trauma can benefit from a period of observation before physicians decide whether to order computed tomography (CT) scans. Researchers note that the strategy significantly reduces the number of CTs that are required in these cases, reducing the risks associated with exposure to ionizing radiation. In a single-center study, researchers compared children who were observed with children who were not observed prior to CT decisions being made. They found that every hour of observation time was associated

Diagnosis and management of urinary tract infection in the emergency department and outpatient settings.

Emergency physicians encounter urinary tract infections (UTIs) in a wide spectrum of disease severity and patient populations. The challenges of managing UTIs in an emergency department include limited history, lack of follow-up, and lack of culture and susceptibility results. Most patients do not require an extensive diagnostic evaluation and can be safely managed as outpatients with oral antibiotics. The diagnostic approach to and treatment of adults presenting to emergency departments with UTIs are reviewed.

National trends in pelvic inflammatory disease among adolescents in the emergency department.

PURPOSE: In 2002, the Centers for Disease Control and Prevention (CDC) broadened the pelvic inflammatory disease (PID) diagnostic criteria to increase detection and prevent serious sequelae of untreated PID. The impact of this change on PID detection is unknown. Our objectives were to estimate trends in PID diagnosis among adolescent emergency department (ED) patients before and after the revised CDC definition and to identify factors associated with PID diagnoses. METHODS: We performed a retrospective repeated cross-sectional study using the National Hospital Ambulatory Medical Care Survey f

Disparities in trauma care and outcomes in the United States: a systematic review and meta-analysis.

BACKGROUND: Race and socioeconomic disparities are pervasive and persist throughout our health care system. Inequities have also been identified in outcomes after trauma despite its immediate nature and the perceived equal access to emergent care. OBJECTIVES: Our objective was to systematically evaluate the current literature on the association between trauma mortality and race, insurance status, and socioeconomic status. Our secondary objective was to assess data investigating potential mechanisms underlying these outcome disparities. METHODS: We performed a systematic review and random eff

Coding practices affect the cost of distal radius fracture care.

The purpose of this study was to determine the impact of an Emergency Medicine Department's billing practices on the total cost of care for distal radius fractures. This study identified patients by International Classification of Diseases, Ninth Revision (ICD-9) billing codes treated by the Department of Orthopaedic Surgery (DOS) and Department of Emergency Medicine (DEM) at the University of Arizona. In every case, the surgical modifier 54 was used. The billing records in each case for the DEM and the DOS were reviewed. When the fracture was manipulated and the DEM was the attending of recor

Children presenting with acute pericarditis to the emergency department.

OBJECTIVE: The objective of the study was to evaluate the clinical features and the outcome of children who presented to the emergency department and were ultimately diagnosed with pericarditis. METHODS: A retrospective chart review of all children diagnosed with acute pericarditis from January 2000 through March 2007 was conducted. RESULTS: There were 94 children with pericarditis as the sole or one of the discharge diagnoses: 34 with postsurgical pericarditis and 38 with pericarditis as a component of a generalized illness were not examined further. Of the 22 children included in the study

Use of emergency ultrasound in the diagnostic evaluation of an infant with vomiting.

The use of bedside emergency ultrasound in pediatric abdominal emergencies is becoming more widespread and can be a useful adjunct in the assessment of children who present with abdominal pain. Our case describes an infant who presented to the emergency department with vomiting, in whom an emergency ultrasound evaluation led to timely diagnosis and intervention of an unanticipated condition.

Difficult airway management in the emergency department: GlideScope videolaryngoscopy compared to direct laryngoscopy.

BACKGROUND: Videolaryngoscopy has become a popular method of intubation in the Emergency Department (ED), however, little research has compared this technique with direct laryngoscopy (DL). OBJECTIVE: To compare the success rates of GlideScope (Verathon Inc., Bothell, WA) videolaryngoscopy (GVL) and DL in emergent airways with known difficult airway predictors (DAPs). METHODS: We evaluated 772 consecutive ED intubations over a 23-month period. After each intubation, the physician completed a data collection form that included: demographics, DAPs, Cormack-Lehane view, optical clarity, lens co

Improved glottic exposure with the Video Macintosh Laryngoscope in adult emergency department tracheal intubations.

STUDY OBJECTIVE: Glottic visualization with video is superior to direct laryngoscopy in controlled operating room studies. However, glottic exposure with video laryngoscopy has not been evaluated in the emergency department (ED) setting, where blood, secretions, poor patient positioning, and physiologic derangement can complicate laryngoscopy. We measure the difference in glottic visualization with video versus direct laryngoscopy. METHODS: We prospectively studied a convenience sample of tracheal intubations at 2 academic EDs. We performed laryngoscopy with the Karl Storz Video Macintosh Lar

Characteristics of undiagnosed liver abscesses on initial presentation at an emergency department.

Liver abscesses often present with nonspecific symptoms and laboratory examination abnormalities, resulting in missed diagnoses at emergency departments (ED). The purpose of this study was to determine if there are differences in presentation and prognosis between patients in whom liver abscess is diagnosed at an ED or once the patient has been transferred to a ward. Patients with a liver abscess who were discharged from our hospital between 2005 and 2007 were retrospectively reviewed. We compared the clinical characteristics between patients with liver abscess diagnosed at an ED or in a ward.

Bacterial blood cultures in children with sickle cell disease.

BACKGROUND: Children with sickle cell disease (SCD) are considered at risk for bacteremia, especially when they present to the emergency department (ED) with fever. OBJECTIVE: We aimed to determine the incidence of bacteremia in children with SCD presenting with or without fever to a pediatric ED. METHODS: A retrospective chart review of 692 pediatric ED visits of children with SCD during a 2-year period was conducted. RESULTS: Seven blood cultures (6 homozygous and 1 heterozygous) had bacterial growth (1.3%; 95% confidence interval, 0.5-2.1), 3 of which were among febrile children (1.7%; 9

Alcohol as a risk factor for injury: lessons from French data.

The relationship between alcohol and injury is complex and the evidence level varies depending on the type of injury. The role of alcohol as a causal factor of traffic accidents is clear. There is less evidence concerning home and leisure injuries which include a wide range of accident causes and mechanisms. However, several studies dealing with specific injuries, such as drowning and fracture injuries in seniors, suggest a correlation between alcohol and the occurrence of injuries. Intentional injuries (violence, suicide) are complex and multifactorial phenomena. It is therefore more complica

[Emergency care of vertigo patients: suggestions for efficient management].

Some diseases in which persons show vertigo or dizziness may be life-threatening, regardless of symptom severity, and require careful attention. These include diseases of the inner ear, central nervous system, and cardiovascular manifestation. In May 2006, a group in charge of primary emergency consultation began work enabling physicians to treat vertigo patients more efficiently and safely, as detailed in this report. Of the 173 persons with vertigo hospitalized from January 2004 to March 2008, six had cerebrovascular manifestations clarified only after hospitalization, underscoring the impor

Validation of the Wong-Baker FACES Pain Rating Scale in pediatric emergency department patients.

OBJECTIVES: The Wong-Baker FACES Pain Rating Scale (WBS), used in children to rate pain severity, has been validated outside the emergency department (ED), mostly for chronic pain. The authors validated the WBS in children presenting to the ED with pain by identifying a corresponding mean value of the visual analog scale (VAS) for each face of the WBS and determined the relationship between the WBS and VAS. The hypothesis was that the pain severity ratings on the WBS would be highly correlated (Spearman's rho > 0.80) with those on a VAS. METHODS: This was a prospective, observational study of
