A Prospective Observational Study on the Clinical Outcomes of Diabetic Foot Ulcers Based on Diabetic Ulcer Severity Score at a Tertiary Care Center in Jharkhand, India.

Background People with long-standing or poorly controlled diabetes often experience problems in their feet due to nerve damage and poor blood circulation. These issues can make the feet more prone to injury and infection. When wounds do occur, they may not heal properly, sometimes resulting in ulcers, which may even lead to amputations of the foot if not managed properly and timely. This is a serious complication of diabetes. In order to manage the ulcers efficiently, a scoring system can be used to assess the severity and guide the treatment modality accordingly. Methods This was a prospectiv

TREOCAPA: prophylactic treatment of the ductus arteriosus in preterm infants by acetaminophen-statistical analysis plan for the randomized phase III group sequential trial.

BACKGROUND: Persistent patency of the ductus arteriosus (PDA) has challenged neonatologists for more than 40 years. Controversies persist about the management of PDA in extremely preterm infants. PDA is associated with morbidities, but no therapeutic strategy has resulted in an improved neonatal outcome. Acetaminophen appears to be a promising alternative with possibly fewer adverse effects. The primary objective is to determine whether a prophylactic pharmacological intervention with acetaminophen may increase the survival without severe morbidity at postmenstrual age of 36 weeks. METHODS AN

Cardiovascular risk associated with glucagon-like peptide-1 receptor agonists versus other conventional glucose-lowering drugs in patients with type-2 diabetes: protocol for a nationwide observational comparative study in routine care.

INTRODUCTION: Several cardiovascular outcome trials have been conducted to assess the cardiovascular safety and efficacy of glucagon-like peptide-1 receptor agonists (GLP1-RAs) on cardiorenal outcomes in patients with type-2 diabetes (T2D). However, the strict requirements of randomised controlled trials to avoid most confounding factors are at the expense of external validity. Using national real-world data, we aimed to evaluate the effectiveness of GLP-1RAs in association with metformin especially on cardiovascular events, hospitalisation for heart failure and all-cause death in comparison w

Thrombolysis as Part of Acute ST-Segment Elevation Myocardial Infarction (STEMI) Care: Experience From the STEMI Network in Tanzania.

Background and objective The burden of ischaemic heart disease (IHD) in low- and middle-income countries (LMIC) is increasing worldwide. A significant proportion of hospitalizations and deaths have been attributed to this condition. Promoting good clinical outcomes in patients with IHD requires timely diagnosis and management using percutaneous coronary intervention (PCI) and recommended pharmacotherapy. Fibrinolytics, combined with antiplatelet and anticoagulant agents, are recommended as initial therapy in resource-limited settings. The regional systems, which aim to provide access to timely

Comparison of ultrasound-guided continuous intertransverse process block and continuous erector spinae plane block for postoperative analgesia after uniportal video-assisted thoracoscopic surgery: protocol for a double-blind, prospective, randomised controlled trial.

INTRODUCTION: Video-assisted thoracoscopic surgery (VATS) has become the predominant method for lung cancer diagnosis and resection, with over 80% adoption in relevant surgeries. With technological advancements, uniportal VATS is now widely employed. However, it still poses a significant risk of moderate to severe acute postoperative pain, potentially leading to chronic post-thoracotomy pain syndrome (PTPS). Therefore, effective postoperative analgesia is crucial. Regional block techniques have gradually gained wide attention. Specifically, paravertebral block (PVB) has been considered the 'go

Efficacy and acceptability of different blood flow restriction training interventions during the rehabilitation of military personnel with lower limb musculoskeletal injuries: protocol for a two-phase randomised controlled trial.

BACKGROUND: Musculoskeletal injury (MSKI) is the leading cause of medical downgrading and discharge within the UK military, with lower limb MSKI having the greatest incidence, negatively impacting operational readiness. Pain is a primary limiting factor to rehabilitation progress following MSKI. Heavy-load resistance training (RT; ie, loads >70% 1-repetition maximum) is traditionally used but may be contraindicated due to pain, potentially prolonging recovery and leading to failure of essential physical employment standards for UK military personnel. Low-load RT with blood flow restriction (BF

Acute High Dose Melatonin for Encephalopathy of the Newborn (ACUMEN) Study: a protocol for a multicentre phase 1 safety trial of melatonin to augment therapeutic hypothermia for moderate/severe hypoxic ischaemic encephalopathy.

INTRODUCTION: Neonatal death and later disability remain common sequelae of hypoxic-ischaemic encephalopathy (HIE) despite the now standard use of therapeutic hypothermia (HT). New therapeutic approaches to brain protection are required. Melatonin is an indolamine hormone with free-radical scavenging, antiapoptotic, anti-inflammatory and gene regulatory neuroprotective properties, which has extensive preclinical evidence of safety and efficacy. Pharmacokinetic (PK) data suggest it is necessary to reach melatonin levels of 15-30 mg/L within 6-8 hours of hypoxia-ischaemia for brain protection. W

The LoaD study: the association between different types of physical activity and the progression of knee osteoarthritis-a cohort study protocol.

BACKGROUND: The global prevalence of osteoarthritis (OA) is expected to rise as the population ages, leading to increased physical impairments that often reduce physical activity. Knee OA, in particular, is a leading cause of chronic pain and disability in people over 40. OA progression varies among individuals, with factors such as higher age and body mass index contributing to a more rapid progression. However, research suggests that engaging in physical activity such as hiking and running may help alleviate pain and slow OA progression. However, current literature on the relationship betwee

Experimental wounding models of different bullet types and diameters on extremities.

BACKGROUND: Gunshot wounds are the second leading cause of life-threatening injuries and frequently affect the extremities, accounting for 63% of combat-related cases over the past 50 years. Although extremity injuries have lower mortality rates, they still require urgent treatment to reduce complications. Wound ballistics studies often use tissue surrogates like ballistic wax and gelatin, which have limitations due to their dissimilarity to living tissues. There is insufficient data on the effects of gunshots on extremities composed of bone and muscle, which differ in resistance and elasticit

Post-streptococcal acute glomerulonephritis in children: Association between proteinuria levels and renal outcomes.

BACKGROUND: Post-streptococcal acute glomerular nephritis (PSAGN) is mostly a benign condition. The usual sequelae of PSAGN include hypertension, its complications, and acute kidney injury. Severe PSAGN is associated with significant long-term morbidity, and histological abnormalities such as crescentic glomerulonephritis are infrequently reported. PSAGN has also been linked to late-onset chronic kidney disease in some populations due to high levels of proteinuria. AIM: To evaluate the association between proteinuria levels and renal outcomes in children with PSAGN. METHODS: This prospective

Long-term recovery from post-traumatic neck pain: protocol for a prospective cohort study in the emergency department.

INTRODUCTION: Post-traumatic neck pain is common, representing a substantial human and societal burden. About 15%-25% of individuals involved in an accident causing whiplash continue to experience moderate-to-severe symptoms and functional impairment 1 year post-trauma. Factors such as age, high pain intensity, hypersensitivity to pain and early post-traumatic hyperarousal are associated with persistent neck pain. However, multiple questions remain unanswered regarding how best to improve early care. As such, research on recovery patterns (including indicators for health economic burden) and t

Exploring exhaled breath biomarkers for lactose intolerance diagnosis: the Lactobreath pilot study protocol.

INTRODUCTION: Food intolerances are prevalent in Europe and can cause considerable physical discomfort, dietary restrictions and psychosocial challenges. Among the prominent causes of food intolerance are defects in the digestion and/or transport of short-chain fermentable carbohydrates, fermentable oligo-, di-, monosaccharides and polyols (FODMAPs). A common diagnostic tool for food intolerance is the hydrogen breath test, which monitors the production of H2 gas from the fermentation of ingested FODMAPs by colonic microbiota. However, this method is limited due to its relatively poor correlat

Acceptability of home-based HPV self-sampling for cervical cancer screening among users and providers in the West region of Cameroon: a cross-sectional study.

BACKGROUND: The World Health Organization's 90-70-90 goal aims to eliminate cervical cancer (CC) as a public health issue, with a target of up to 70% of women screened by 2030. However, many countries, including Cameroon, are far from achieving this goal. Home-based human papillomavirus (HPV) self-sampling is a promising approach to improve participation rates. The main objective of this study was to explore the acceptability and feasibility of home-based HPV self-sampling in the West region of Cameroon. METHODS: A quantitative, descriptive, cross-sectional study conducted between January 11

Intraperitoneal irinotecan with concomitant FOLFOX and bevacizumab for patients with unresectable colorectal peritoneal metastases: protocol of the multicentre, open-label, phase II, INTERACT-II trial.

INTRODUCTION: The peritoneum is the second most affected organ for the dissemination of colorectal cancer (CRC). Patients with colorectal peritoneal metastases (CPM) face a poor prognosis, despite the majority of patients being treated with palliative systemic therapy. The efficacy of palliative systemic therapy is limited due to the plasma-peritoneum barrier. The poor prognosis of unresectable CPM patients has resulted in the development of new treatment strategies where systemic therapy is combined with local, intraperitoneal chemotherapy. In the recently published phase I study, the maximum
