[Bite injuries in the head and neck area].

In Germany about 8500 dog-bite injuries in the face occur every year; more than 50% of the victims are infants and schoolchildren. Besides dogs, other animals such as cats or horses may be responsible for these accidents. Even human bites are reported. The predominant areas are the nose and the auricles. The tissue defects may be superficial, but they can even cause amputations, including severe vascular and nerve or bony destruction. Systemic antibiotic therapy is needed when the wound is infected. The surgical approach to bite injuries includes local wound cleansing, careful excision of necr

The incidence of TMJ dysfunction in patients who have suffered a cervical whiplash injury following a traffic accident.

Forty consecutive patients with a cervical whiplash injury were examined and compared with 40 matched controls. The incidence of signs and symptoms was high in both groups. Temporomandibular joint pain (P < .001), limitation of mouth opening (P < .01), and masticatory muscle tenderness (P > .01) were found significantly more frequently in the patient group, while the presence of joint sounds, deviation during mouth opening, and the overall presence of a symptom were not significantly different between groups. When asked whether they would seek treatment for the observed signs and symptoms, a s

Patho-anatomic findings in neck structures in asphyxiation due to hanging: a survey of 80 cases.

By examining 80 consecutive cases of death due to hanging, fractures of the thyroid cartilage or the hyoid bone or both, were noted in 45%. Fracture of the cricoid cartilage did not occur. No fracture occurred below the age of 25, and the frequency was slightly increasing with increasing age. There was a preponderance of fractures in males. The highest incidence of fractures was noted in typical hangings, while incomplete hanging to a very great extent was combined with congestion to the face, especially if the location of the ligature was atypical. Typical hanging in general did not produce c

A review of 324 rhytidectomies, emphasizing complications and patient dissatisfaction.

The results and complications after 324 rhytidectomies, done between 1962 and the middle of 1973, were reviewed. The major complications were hematoma, skin slough, and facial nerve damage. In addition, respch as numbness of portions of the face, loss of hair, bothersome scarring, and general dissatisfaction with the operation. In general, these latter were of more concern to them than the major complications.

Traumatic optic neuropathy prediction after blunt facial trauma: derivation of a risk score based on facial CT findings at admission.

PURPOSE: To determine the specific facial computed tomographic (CT) findings that can be used to predict traumatic optic neuropathy (TON) in patients with blunt craniofacial trauma and propose a scoring system to identify patients at highest risk of TON. MATERIALS AND METHODS: This study was compliant with HIPAA, and permission was obtained from the institutional review board. Facial CT examination findings in 637 consecutive patients with a history of blunt facial trauma were evaluated retrospectively. The following CT variables were evaluated: midfacial fractures, extraconal hematoma, intra

[Traumatic optic neuropathy].

Traumatic optic neuropathy is a severe complication of a head or face trauma. Diagnosis is established by analyzing pupillary responses to light. Fundus examination and orbital CT scan help to localize the site of the lesion and may show a fracture involving the optic canal. Surgical decompression or megadose steroids are therapeutic options. No therapeutic approach has proven with sufficient evidence to be effective. Spontaneous improvement is definitely possible. Based on experiences in single cases direct compression of the optic nerve by bone fragments or optic nerve sheath haemorrhage are

Pain remapping in migraine: a novel characteristic following trigeminal nerve injury.

The location of pain during the headache phase of migraine varies between individuals as well as between attacks in some individuals. We have observed a "remapping" or a change in the location of migraine pain following injury to the trigeminal system that is a novel characteristic to migraine and has not been described in other trigeminal pain syndromes of the head, neck, and face. Recognition of this clinical feature implies that the pathophysiology of migraine is impressionable and may be why diagnosis and treatment are often delayed.

Preservation of mouth region processing in two cases of prosopagnosia.

Although most adults are considered experts in face recognition, brain trauma can produce a selective loss in this ability, a condition referred to as prosopagnosia. This study examined the processing strategies of prosopagnosic patients LR and HH using the Face Dimensions Test. In this test, featural and configural information in the upper and lower halves of the face was parametrically varied and sensitivity to these changes measured. We found that relative to age-matched control participants, LR and HH exhibited an impaired ability to discriminate differences in the eye region, but a preser

Axial pattern composite prefabrication of high-density porous polyethylene: experimental and clinical research.

Currently, various alloplastic materials are being used for reconstruction of three-dimensional structures, and high-density porous polyethylene is so far the best and the most commonly used material. Various indications for high-density porous polyethylene have been defined for closure of craniofacial defects, correction of congenital anomalies, and aesthetic augmentations. A common property of various studies published so far is that after being fixed to the bone or underlying structures, high-density porous polyethylene has been covered primarily or by skin flaps. For reconstruction of comp

Trigeminal pain caused by a cerebellopontine-angle lipoma. Case report and review of the literature.

A 16-year-old girl complained of 2-year history of right facial pain, episodic vertigo and progressive hearing loss in the right ear. The facial pain was described as an episodic lancinating event confined to the second and third branch of the right trigeminal nerve. Computed tomography and magnetic resonance imaging revealed a 2 cm lesion in the right cerebellopontine-angle. At surgery, a soft, yellowish mass was found incorporating the 7th and 8th cranial nerves. The anterior-inferior cerebellar artery (AICA) was displaced medially and pushed into the sensory portion of the trigeminal nerve

Aesthetic Unit Preservation in Emergency Facial Lacerations: A Prospective Evaluation of Landmark-Based Nerve Blocks.

BACKGROUND: Facial trauma repair requires precise reconstruction while preserving aesthetic units. Traditional local anesthesia can distort tissue planes and compromise surgical precision. METHODS: This prospective study evaluated landmark-based nerve blocks versus local infiltration for complex facial laceration repair in emergency settings from January 2022 through February 2023. A total of 162 patients were randomized to receive either landmark-based nerve blocks (n = 80) or traditional local anesthesia (n = 82). The nerve block group received targeted injections at anatomical landmarks, w

Extending the Reach of Craniofacial Free Flaps Using the Descending Branch of the Lateral Circumflex Femoral Vessels.

SUMMARY: Craniofacial free tissue transfer is sometimes complicated by insufficient pedicle length and/or paucity of recipient vessels. A saphenous vein graft can be used to reach the high-flow, large-caliber vessels of the neck, but because of the vein's taper and thick wall, there is often a mismatch. Following the principle of like-for-like, the authors prefer the descending branch of the lateral circumflex femoral vessels to achieve a more anatomical pedicle extension for free tissue transfer in complex craniofacial reconstruction. The authors' experience using the descending branch of the

Effectiveness of C5 or C6-Cz assembly in predicting immediate post operative facial nerve deficit.

BACKGROUND: Intraoperative neurophysiology monitoring (IOM) is a valuable tool in cerebellopontine angle (CPA) surgeries posing risk to the cranial nerves. Transcranial electrical stimulation (TES) for cranial nerves has been performed in the last 7 years, for obtaining the facial nerve motor evoked potential (MEP), using either C3/C4-Cz or C3-C4 (or inverse) stimulating points, which have been correlated with facial nerve functional outcome. METHOD: Intraoperative surgical and electrophysiological findings were documented prospectively. Patient files were reviewed for clinical data. We studi
