The case for optimal investment in combating HIV, tuberculosis, and malaria: a global modelling study.

BACKGROUND: The Sustainable Development Goals (SDGs) include ending the epidemics of HIV, tuberculosis, and malaria by 2030. With 5 years remaining to meet this goal, and with the Global Fund to Fight AIDS, Tuberculosis and Malaria seeking funding for programmes in 2027-29, establishing what can be achieved through continued investment in combatting these diseases is crucial. We aimed to estimate the potential for impact by analysing the funding landscape and epidemiological situations of these three diseases, the costs of key programmes, and the extent of possible future progress in the count

Maternal occupational exposure to endocrine-disrupting chemicals during pregnancy and semen parameters in adulthood: results of a nationwide cross-sectional study among Swiss conscripts.

STUDY QUESTION: Is there a relationship between maternal occupational exposure to endocrine-disrupting chemicals (EDCs) during pregnancy and the semen quality of their sons? SUMMARY ANSWER: Our results suggest an association between maternal occupational exposure to potential EDCs, especially to pesticides, phthalates and heavy metals, and a decrease in several semen parameters. WHAT IS KNOWN ALREADY: Sexual differentiation, development and proper functioning of the reproductive system are largely dependent on steroid hormones. Although there is some animal evidence, studies on maternal expo

Effect of a Smart Pill Bottle and Pharmacist Intervention on Medication Adherence in Patients with Multiple Myeloma New to Lenalidomide Therapy.

BackgroundU.S. specialty drug spend is expected to reach $400 billion by 2020, with significant growth in oncology. New oral oncology approvals have allowed for more convenient outpatient administration compared with physician-administered chemotherapies; however, patients may encounter challenges with adherence when taking medications at home. Emerging medication adherence technology (MAT) attempts to provide at-home adherence support, and while one such technology, smart pill bottles (SPB), claims to improve medication adherence, few studies have formally assessed their effects.ObjectivesTo

Early community reactions and acceptance of the Sliding-Scale Community Based Health Insurance Scheme in Ethiopia: Qualitative Findings from the treatment arm.

BACKGROUND: Ethiopia's Community-Based Health Insurance (CBHI) launched in 2011, has significantly improved access to healthcare. However, the program has faced several challenges, including concerns about equity, financial sustainability, and limited resource mobilization stemming from its flat-rate premium contribution system. To address these issues, a new sliding-scale premium scheme-based on households' socio-economic status-has been piloted in 13 districts across Oromia and Sidama regions in Ethiopia. However, empirical evidence on community responses to sliding-scale CBHI models remains

Food insecurity and its contributing factors among people living with HIV attending antiretroviral therapy in Addis Ababa, Ethiopia: a mixed-methods study.

BACKGROUND: Despite growing evidence that food insecurity and HIV/AIDS are interlinked in a vicious cycle exacerbating each other through nutritional deficiencies, psychological stress, and risk behaviors there remains a significant gap in understanding the specific factors contributing to food insecurity among patients on highly active anti-retroviral therapy. In Ethiopia, it ranges from 55% to 84.5% and can cause immunologic deterioration, as well as increased morbidity and death in individuals who are already afflicted. Therefore; this study was aimed to assess food insecurity and its contr

Diagnostic Yield After Negative Bidirectional Endoscopy for Iron Deficiency Anaemia: A Single-Centre Audit of Downstream Investigations in the UK Two-Week-Wait Pathway.

BACKGROUND: Iron deficiency anaemia (IDA) accounts for a significant proportion of all gastroenterology referrals. Current British Society of Gastroenterology (BSG) guidance recommends bidirectional endoscopy as first-line investigation and reserves small-bowel or renal-tract investigation for patients with recurrent IDA or inadequate response to iron replacement therapy (IRT) after negative endoscopy. Advanced imaging modalities represent high financial costs to the NHS, but their diagnostic yield in this specific cohort is debated. OBJECTIVE: To describe the utilisation and observed diagnos

Decision-making regarding dental treatments - What factors matter from patients' perspective? A systematic review.

BACKGROUND: Achieving oral health for the population should be a concern of public health care systems, as it may affect their expenditures in the long term. Patients often face individual challenges in dental care. Why patients decide for or against dental treatments can be determined by many factors, e.g., their own financial resources, preferences, and external circumstances. This cross-country study aims to identify those factors. METHODS: We systematically searched for literature in the biomedical databases PubMed (including MEDLINE), the Cochrane Library, and Web of Science to identify
