[Argon laser photocoagulation of circumscribed choroidal hemangiomas].

PURPOSE: Therapeutic retrospective results obtained after Argon laser photocoagulation of 17 circumscribed choroidal hemangiomas were analyzed. MATERIAL AND METHODS: Seventeen circumscribed choroidal hemangiomas associated with serous retinal detachment were analyzed. All patients were symptomatic but one. Duration of visual symptoms varied from 2 weeks to 3 years. Mean tumor thickness was 3,3mm (1 to 6,5mm). Mean tumor diameter was 6,5mm (2.5 to 15mm). Tumors were juxtapapillary in 9 cases (57%). The mean distance from the fovea was 1,5mm (0 to 3mm), and the fovea was pathological in all cas

Cardiovascular Dysfunction in Sepsis and Septic Shock.

The optimal therapy for the treatment of sepsis and septic shock remains controversial. Many protocols are followed, using different strategies for initial resuscitation, cardiovascular monitoring, hemodynamic intervention, and eradication of infection. Overall, an aggressive approach to the management of cardiovascular dysfunction in septic shock is warranted. Initially, large volume fluid resuscitation is instituted. Our first choice of resuscitation fluid is 0.9% normal saline. Invasive hemodynamic monitoring using a flotation pulmonary artery catheter as well as invasive arterial blood pre

Tuberculous meningitis at a large inner-city medical center.

Tuberculosis in the United States has become primarily an inner-city disease. We examined the epidemiology of culture-confirmed tuberculous meningitis among patients cared for at an urban public hospital in Atlanta. During an 11.5-year period (January 1984-June 1995) cerebrospinal fluid cultures for Mycobacterium tuberculosis were positive in 34 patients, accounting for 1.5% of all culture-confirmed tuberculosis cases. All patients were born in the United States, 31 (91%) were black, 16 (47%) of 34 were human immunodeficiency virus (HIV) seropositive, 9 (26.5%) were HIV seronegative, and 9 (26

Leptospiral pneumonia.

Leptospirosis, a spirochetal infection, causes a wide spectrum of disease ranging from asymptomatic infection, or influenza-like symptoms, to severe jaundice and renal failure. Humans become infected through skin or mucous membrane contact with infected animal urine or urine-contaminated water or soil. The most common source of human infection worldwide is rats. However, in the United States, dogs, livestock, wild mammals, and cats are also sources. Once leptospires penetrate mucous membranes or breaks in the skin, they disseminate to all parts of the body. Five to ten percent of those infecte

[Relapsing polychondritis and mesenteric panniculitis: apropos of 2 cases].

We report two cases of polychondritis associated with mesenteric panniculitis. Case 1. In February 1989, a woman born in 1949 presented with 40 degrees C fever accompanied by pain in the abdomen and pelvis. Eight days later, nodular skin lesions appeared on her lower limbs. The abdomen was swollen with gas and undepressible. An abdominal CT scan revealed partitioned peritoneal collections, and a guided needle aspiration produced a chylous fluid. Direct and indirect bacteriological examinations gave negative results. Histology showed intense inflammatory reaction with giant cells and lipophages

Hypertonic saline dextran resuscitation during the initial phase of acute endotoxemia: effect on regional blood flow.

BACKGROUND AND METHODS: Small-volume resuscitation by means of bolus application of hypertonic saline solutions has been demonstrated to res … as analyzed for its effect on central hemodynamics, oxygen delivery (Do2), and regional blood flow. Fluid therapy was initiated when the pulmonary artery occlusion pressure (PAOP) tended to decrease (at 43 to 52 mins of endotoxemia), and consisted of 4 mL/kg bolus infusion of either 7.2% sodium chloride, 10% dextran, or 10% dextran in 7.2% sodium chloride; ther …

Nutritional support of patients with renal disease.

Current concepts in the nutritional support of patients with renal disease are reviewed. In chronic renal failure, alterations in fat, carbohydrate, and glycogen metabolism usually occur and may be worsened by acute illness. Total parenteral nutrient (TPN) therapy is rarely required unless complications occur. In contrast, acute renal failure is generally associated with hypovolemia, sepsis, soft tissue injury, and coagulation defects, all of which influence metabolism and extracellular fluid volume; the gluconeogenesis that often occurs in these patients masks the metabolic effects of uremia.

Bubble Trouble: Portal Venous Gas Embolism Following Hydrogen Peroxide Enema.

Hydrogen peroxide (H2O2) is a caustic agent capable of causing serious gastrointestinal injury, yet its use as an enema is promoted in some alternative medicine communities for constipation. We report a case of a 56-year-old man who developed portal and mesenteric venous air embolism and chemical colitis following self-administration of an H2O2 enema. Imaging revealed extensive portal venous gas and colitis. Colonoscopy confirmed diffuse mucosal injury, and biopsies showed acute colitis with ulceration. The patient was treated with intravenous fluids, broad-spectrum antibiotics, and hyperbaric

Disseminated cryptococcosis resembling miliary tuberculosis in a patient with acute myeloid leukemia.

Disseminated cryptococcosis, usually involving the lungs and central nervous system, carries a high risk of morbidity and mortality in immunocompromised hosts. In this report, we describe a case of miliary pulmonary cryptococcosis in a patient with acute myeloid leukemia, initially resembling miliary tuberculosis. The diagnosis of disseminated cryptococcosis was made based on transbronchial lung biopsy with subsequent detection of Cryptococcus neoformans in blood and cerebrospinal fluid. The patient was treated with liposomal amphotericin B as induction therapy, followed by fluconazole as cons

Immobilization hypercalcemia crisis.

Profound hypercalcemia associated with immobilization is rare. Hypercalcemic crisis occurring as a result of immobilization in which there was not a coexisting, contributing medical condition has not, to our knowledge, been reported previously. Failure to consider hypercalcemia as the source of progressive anorexia, nausea, vomiting, and irritability resulted in a respiratory arrest and nearly fatal outcome in the case of a 13-year-old boy one month after a simple femoral fracture. Therapy consisting of the intravenous administration of fluids and corticosteroids was successful in lowering the

Critical Management of Severe Hypotension Caused by Amlodipine Toxicity Managed With Hyperinsulinemia/Euglycemia Therapy Supplemented With Calcium Gluconate, Intravenous Glucagon and Other Vasopressor Support: Review of Literature.

Calcium channel blocker (CCB ) overdose, whether intentional or accidental, is a common clinical scenario and can be very lethal. Conventional treatments for CCB overdose include intravenous (IV) fluids, calcium salts, dopamine, dobutamine, norepinephrine, phosphodiesterase inhibitors, and glucagon. However, the conventional therapies are unsuccessful in reversing the cardiovascular toxicity of CCB, so they commonly fail to improve the hemodynamic condition of the patient. Blockade of the L-type calcium channels that mediate the antihypertensive effect of CCBs also decreases the release of ins

Isolated shortness of breath in a woman with a history of Hodgkin lymphoma.

BACKGROUND: Isolated shortness of breath in the patient with a history of a malignancy creates a diagnostic challenge and serves as a source of anxiety. Although cancer recurrence is typically the first concern of the patient and the clinician, toxicities of anticancer therapies must also be considered. METHODS: A case of a 49-year-old woman with a distant history of Hodgkin lymphoma with 2 months of progressive dyspnea is presented and discussed. RESULTS: Although the patient was found to have bilateral pleural and pericardial effusions that were concerning for a recurrence of malignancy, a

Macroprolactinoma in an adolescent female with primary amenorrhoea.

SUMMARY: Paediatric pituitary adenomas are rare in children and adolescents and differ from adults in both clinical presentation and management. We present the case of a 14-year-old female with primary amenorrhoea secondary to a macroprolactinoma, showing a modest radiological and biochemical response to dopamine agonist (DA) therapy. Despite a 10-month duration of increasing DA therapy, initial symptoms of primary amenorrhoea and hyperprolactinaemia persisted, with new symptoms of weight gain, lethargy and low mood. A transsphenoidal resection of the macroprolactinoma was successfully perform

[A clinical study on chemotherapies for chorioamnionitis].

Effects of imipenem/cilastatin (IPM/CS) therapy, flomoxef (FMOX) therapy and combined ceftazidime + aspoxicillin (CAZ/ASPC) therapy as initial therapies for chorioamnionitis were assessed clinically. 1. The subjects were 49 women with threatened abortion and 29 with premature rupture of membranes (PROM), complicated in all cases by chorioamnionitis. The inflammation was treated with IPM/CS in 19 patients, FMOX in 39, CAZ in 11, and CAZ/ASPC in 9. 2. The response rate to therapy for chorioamnionitis was 95.9% (47/49) in the threatened abortion group. Of the 49 patients in this group, 16 (32.7%)

The polymerase chain reaction in infectious and post-infectious arthritis. A review.

Polymerase chain reaction and other DNA amplification techniques to identify elusive infections should prove to be an effective tool for the clinical and investigative rheumatologist. The capability to identify and characterize infectious organisms in fluids and tissue will enable early, specific, and potentially curative treatment. Similarly, the capability to exclude infection and differentiate postinfectious diseases will enable other therapies to control the inflammation. Understanding these molecular techniques will most certainly improve clinicians' effectiveness for diagnosis and care.

Case Report: Dasatinib-associated chylothorax and occult Bartonella henselae infection in pediatric leukemia: a case report and systematic review.

Dasatinib is a second-generation tyrosine kinase inhibitor (TKI) used in Philadelphia chromosome-positive (Ph+) acute lymphoblastic leukemia (ALL), and chylothorax is one of its rare but serious lymphatic complications. We encountered a 14-year-old boy with Ph + acute lymphoblastic leukemia (ALL) who developed bilateral chylothorax while on dasatinib, with concurrent B. henselae infection (cat-scratch disease) confirmed in the pleural fluid. Prompted by this index case, we performed a systematic review of the literature to explore the potential association between dasatinib-associated chylotho

Enterococcus faecium pneumonia diagnosed by metagenomic next-generation sequencing in a patient with chronic obstructive pulmonary disease.

Enterococcus faecium is a rare pathogen in community-acquired pneumonia (CAP), and its diagnosis is challenging, particularly when prior antibiotic therapy hampers isolation by conventional culture. We report a 72-year-old man with a 6-year history of chronic obstructive pulmonary disease (COPD) who presented with fever, cough, and progressive dyspnea. Despite empirical broad-spectrum antibiotics (piperacillin-tazobactam followed by imipenem-cilastatin), his condition deteriorated into acute respiratory distress syndrome (ARDS) requiring invasive mechanical ventilation. All routine cultures of

Autoimmune glial fibrillary acidic protein astrocytopathy with full-length spinal cord lesions: a case report and literature review.

BACKGROUND: Autoimmune glial fibrillary acidic protein astrocytopathy (GFAP-A) is an autoimmune inflammatory disease of the central nervous system triggered by GFAP. Currently, the diagnosis is mainly based on the detection of autoantibodies that recognize GFAP in the cerebrospinal fluid. Its imaging manifestations are complex and variable. It is extremely necessary to enhance the understanding of this disease among radiologists and reduce the damage caused by invasive procedures. CASE PRESENTATION: We report a case of a 39-year-old female patient with lesions throughout the spinal cord. The p
