Liver transplantation : official publication of the American Association for the Study of Liver Diseases and the International Liver Transplantation Society2003eng
Osteoporosis remains a serious potential complication of liver transplantation, although its incidence may be significantly reduced by the use of lower doses of glucocorticoids. Additional factors likely to contribute to its pathogenesis include other immunosuppressive agents, particularly cyclosporin A and FK506, vitamin D insufficiency, secondary hyperparathyroidism, hypogonadism and pre-existing bone disease. Bone density assessment and spinal X-rays should be performed before transplantation to assess subsequent fracture risk and vitamin D and gonadal status assessed. Measures should be ta

We encountered a case of capitellum fracture with radial head dislocation in which it was challenging to diagnose whether the dislocation was coincident with the fracture or this was chronic dislocation that presented before the fracture. Chronic radial head dislocation may be congenital or could occur in patients with untreated post-traumatic dislocation, although diagnosis is particularly challenging if the radial head dislocation is unilateral. Classical capitellum fracture involves anterosuperior bone fragment dislocation. However, in the present case, the bone fragment was present beneath

A 33-year-old woman presented with acute nonspecific knee pain, 6 months postpartum. MR imaging, computed tomography and radiography were performed and a proximal tibia plateau insufficiency fracture was detected. Bone densitometry demonstrated mild postpartum osteoporosis. To our knowledge these findings have not been described in this location and in this clinical setting. The etiology of the atraumatic fracture of the tibia is presumed to be due to a low bone mineral density. The bone loss was probably due to pregnancy, lactation and postpartum hormonal changes. There were no other inciting

INTRODUCTION: Historically, cerclage wires were not used in the treatment of clavicle fractures because of their invasiveness. The purpose of this study was to evaluate the radiologic results and the incidence of complications following cerclage wire application and plate fixation in the treatment of comminuted mid-shaft clavicle fractures.
MATERIALS AND METHODS: A total of 116 patients with comminuted mid-shaft clavicle fractures who underwent open reduction and internal fixation were reviewed. We analyzed the postoperative length ratio and bone union period according to the fracture classif

PURPOSE: To evaluate a bone morphogenetic protein (BMP) implant with and without platelet-rich plasma (PRP), which is supposed to accelerate fracture consolidation in the orbit fracture treatment.
METHODS: Thirty-six white rabbits were subjected to orbital fracture and treated in three groups: BMP implant fracture repair (G1), BMP plus PRP implant fracture repair (G2), and fracture and spontaneous repair (G3). The animals were sacrificed at 7, 30, 90, and 180 days after surgery. A radiology evaluation was carried out on the 7th day after the fracture and at the sacrifice moments. After the an

Periprosthetic femur fractures most often occur in elderly, frail patients and represent a devastating injury with high rates of morbidity and mortality even with appropriate surgical management. Hip arthroplasties with cemented femoral fixation have a decreased risk of periprosthetic fracture relative to cementless designs, especially when composite beam stems are used compared to polished, taper-slip stems. Early mobilization is the primary goal of treatment for periprosthetic femur fractures, which can be achieved either through open reduction internal fixation or through revision arthropla

Hip fractures are recognized as a major public health problem worldwide. Demographic changes will lead to enormous increases in the number of hip fractures and projections indicate that the number of hip fractures occurring worldwide each year will rise from 1.26 million in 1990 to 4.5 million by 2050. However, preventive strategies are available. Supplementation with Ca2+ and vitamin D restores bone quality through suppression of secondary hyperparathyroidism and decreases the risk of falling through improvement of neuromuscular coordination and body sway. This leads to a reduction of hip fra

The late presence of contour defects, as well as one eye being lower than the other, were directly related to a lack of interosseous wire fixation in this series of mid-facial fractures. In orbital floor fractures, the use of alloplastic material was found to give more satisfactory long-term results than bone grafts. The direct approach through the lower lid produced no complications of its own in this series. Associated injuries and preexisting disease were found in 61 percent of these patients, supporting the thesis that an M.D. should direct their care. The late presence of diplopia and/or

Hypophosphatasia (HPP) is an inborn disease that causes a rare form of osteomalacia, a mineralization disorder affecting mineralized tissues. Identification of patients at high risk for fractures or other skeletal manifestations (such as insufficiency fractures or excessive bone marrow edema) by bone densitometry and laboratory tests remains clinically challenging. Therefore, we examined two cohorts of patients with variants in the ALPL gene grouped by bone manifestations. These groups were compared by means of bone microarchitecture using high-resolution peripheral quantitative computed tomog

Steroids affect bone health causing osteoporosis and fractures. The study aims to compare dual-release hydrocortisone (DR-HC) and conventional steroids on bone metabolism in patients with primary adrenal insufficiency (PAI). Thirty-five patients with PAI on conventional steroids (group A) and 35 patients switched to DR-HC (group B), consecutively referred at our hospital, were evaluated at baseline and after 18, 36 and 60 months of treatment. After 60 months of follow-up, patients in group A had a significant increase in body mass index (p = 0.004) and waist circumference (WC) (p = 0.026) and

Men with prostate cancer suffer substantially from bone-related complications. Androgen deprivation therapy itself is a cause of loss of bone mineral density and is associated with an increased incidence of osteoporotic fractures. In advanced disease, bone is by far the most common site of metastasis. Complications of bone metastases prominently include pain and the potential for skeletal events such as spinal cord compression and pathologic fractures. Elevated osteoclast activity is an important aspect of the pathophysiology of both treatment-related osteoporosis and skeletal complications du

Paediatric distal tibial and fibular fractures are seen quite regularly in orthopaedic trauma practice. Most patients are managed conservatively with closed reduction or casting while only a selected few required surgical treatment. Surgical options include plating, percutaneous Kirschner wires, rigid intramedullary nails, and flexible intramedullary nailing. This is dependent upon the patient's age, fracture site, comminution, and concomitant injuries. Here, we present an interesting case of a patient with an unusual lesion seen at the fracture site. This lesion was curetted out during surger

Osteoporosis is an increase in bone fragility resulting from a decrease in bone mineral density and abnormalities in the bone micro-architecture. The frequency of osteoporosis is increasing with the aging of patients, and its clinical consequences, vertebral or peripheral fractures, are potentially extremely serious, especially in the very elderly. It is probable that osteoporosis is underdiagnosed--and when it is diagnosed it is often not well treated. It should be realised that a loss in bone mineral density is not the only risk factor for osteoporotic fractures: iatrogenic and environmental

Bone damage because of chronic kidney disease (CKD) represents a daily challenge for nephrologists. The impact of CKD on bone health may be immediate (serum phosphocalcic disturbances) or delayed (bone fractures and vascular calcifications). Histomorphometry remains the gold standard to evaluate bone, but it is rarely performed in clinical practice. Areal measurement of bone mineral density by dual x-ray absorptiometry is routinely performed to evaluate bone mass. However, this technique presents some limitations. In 2000, the United States National Institutes of Health defined new "quality" c

Metacarpal shaft fractures often require surgical correction of displacement, angulation, or rotational deformities to restore function. Kirschner wires (K-wires), plates, and intramedullary screw fixation each have advantages and limitations. However, inconsistent biomechanical evidence in existing studies has precluded definitive conclusions regarding the optimal fixation technique. This studycompared K-wires, plates, and intramedullary screw for metacarpal shaft fractures using finite element analysis (FEA). In normal bone, intramedullary screw matched plate stability under low loads but wer

Osteoporotic fractures are induced by osteoporosis, which may lead to the degradation of bone tissues and microstructures and impair their healing ability. Conventional internal fixation therapies are ineffective in the treatment of osteoporotic fractures. Hence, developing tissue engineering materials is crucial for repairing osteoporotic fractures. It has been demonstrated that nanomaterials, particularly graphene oxide (GO), possess unique advantages in tissue engineering due to their excellent biocompatibility, mechanical properties, and osteoinductive abilities. Based on that, GO-nanocomp

Osteogenesis imperfecta (OI) is commonly associated with fragility fractures. It is due to abnormality in the quantity and quality of collagen type 1 caused by mutations in COL1A1 and COL1A2 genes. Patients with OI would also have blue sclera, ligament hyperlaxity, dentinogenesis imperfecta, hearing abnormality, and short stature. Surgical management is preferred to conservative treatment in long bone fractures. For malunited fractures, Sofield-Millar or multiple osteotomies at different sites of deformities are performed with additional intramedullary device to stabilize the bone. This is a c

Stress fractures are common athletic injuries of the foot and ankle, described in every bone except the lesser toes, and reviewed here. Early diagnosis usually allows for simpler treatment and quick recovery. Early clinical presentations can be subtle, so a high degree of suspicion and a systematic approach, coupled with an understanding of the diagnostic limitations present in early injury, is required. Such a rigorous approach ultimately pays dividends for these patients, who are usually keen to return quickly to athletic activity. "High-risk" fractures include the medial malleolus, the talu

Osteoporosis is a common skeletal disease characterized by reduced bone strength and increased risk for fractures. Fragility fractures are associated with serious clinical consequences, including chronic pain, skeletal deformities, loss of independence, and increased mortality. Although osteoporosis is underdiagnosed and undertreated, many who are treated take medication incorrectly or do not continue it long enough to benefit. Measures to prevent osteoporosis include a healthy lifestyle, with regular physical activity, adequate intake of calcium and vitamin D, and avoidance of cigarette smoki
