Design, Synthesis, and Pharmacological Evaluation of Sulfur-Containing Pitolisant Analogs as Potent H3R Antagonists for Epilepsy Therapy.

Although multiple antiseizure drugs (ASDs) are clinically accessible, epilepsy remains a debilitating neurological disorder with a high unmet need for more effective and better-tolerated therapies. Antagonism of the histamine H3 receptor (H3R) represents a promising alternative strategy, exemplified by the clinically approved inverse agonist/antagonist pitolisant (PIT) for narcolepsy. Herein, we designed a novel series of non-imidazole H3R antagonists by introducing a sulfur atom into the PIT scaffold to modulate lipophilicity, flexibility, and target binding. The synthesis and pharmacological…

Comparing four immunosuppressive agents for chronic spontaneous urticaria-A network meta-analysis.

BACKGROUND: Immunosuppression is an integral part of treating chronic spontaneous urticaria (CSU), but there is no literature to evaluate the efficacy of multiple immunosuppressive agents. OBJECTIVE: The comparison of the efficacy, safety, and incidence of adverse effects of four immunosuppressive medicines (tripterygium glycosides, methotrexate, cyclosporine A, and azathioprine) in combination with antihistamines in treating CSU provides a clinical reference and evidence-based medicine for treating CSU. METHODS: PUBMED, The Cochrane Library, EMBASE, WANFANG, CNKI, CBM, and clinical trial re…

Case Report and Review of the Literature: Bullous Skin Eruption After the Booster-Dose of Influenza Vaccine in a Pediatric Patient With Polymorphic Maculopapular Cutaneous Mastocytosis.

Vaccination is a well-known trigger for mast cell degranulation in subjects affected by mastocytosis. Nevertheless, there is no exact standa … ed with a single dose of oral corticosteroid therapy with betamethasone (0.1 mg/kg) and an oral antihistamine therapy with oxatomide (1 mg/kg/daily) for a week, until resolution. To the best of our knowledge, in the literature, no documented case of reaction to influenza vaccine in maculopapular cutaneous mastocytosis is described. Subsequently, the patient …

Unmet medical needs for chronic spontaneous urticaria patients: highlighting the real-life clinical practice in Taiwan.

BACKGROUND: Treatment guidelines for chronic spontaneous/idiopathic urticaria (CSU) are available; however, only 50% of patients are well controlled with approved doses of H1-antihistamines, and certain patients remain symptomatic despite receiving up to 4× the approved dose of H1-antihistamines plus H2 antihistamines and/or leucotriene-receptor antagonists. OBJECTIVES: To highlight real-life clinical practice in Taiwan and to understand the unmet medical needs of CSU patients. METHODS: A nationwide cross-sectional, observational survey of 50 dermatologists and 200 CSU patients was conducted…

Next-day residual sedative effect after nighttime administration of an over-the-counter antihistamine sleep aid, diphenhydramine, measured by positron emission tomography.

Antihistamines often are self-administered at night as over-the-counter (OTC) sleep aids, but their next-day residual sedative effect has never been evaluated using a reliable quantitative method such as positron emission tomography (PET). We performed a double-blind, placebo-controlled, crossover study in which we evaluated the residual effect the next day after nighttime administration of diphenhydramine, a commonly used OTC sleep aid, in terms of brain H₁ receptor occupancy (H₁RO) measured using ¹¹C-doxepin-PET. We also compared the results of diphenhydramine with those of bepotastine, a se…

Efficacy of leukotriene receptor antagonist with an anti-H1 receptor antagonist for treatment of chronic idiopathic urticaria.

BACKGROUND: Chronic idiopathic urticaria (CIU) is often difficult to treat. Although histamine-releasing activity is detectable for up to 50% of CIU patients, antihistamine therapy provides only a limited response. OBJECTIVE: This study aimed to assess the clinical efficacy of combined leukotriene receptor antagonist (LRA) and H1 antihistamine, H1 and H2 antihistamine, and two H1 antihistamines as a synergistic therapeutic regimen for treating CIU compared with a matched placebo modality. METHODS: A total of 120 newly diagnosed adult patients were evaluated. Patients were single blinded and …

Prediction of biological targets using probabilistic neural networks and atom-type descriptors.

Prediction of biological targets for molecules from their chemical structures is beneficial for generating focused libraries, selecting compounds for screening, and annotating biological activities for those compounds whose activities are unknown. We studied the ability of a probabilistic neural network (PNN), a variant of normalized radial basis function (RBF) neural networks, to predict biological activities for a set of 799 compounds having activities against seven biological targets. The compounds were taken from the MDDR database, and they were carefully selected to comprise distinct biol

Burn wound itch control using H1 and H2 antagonists.

This study investigated the use of a combination of H1 and H2 antagonists and topical medications to control burn wound itch. Graeco-Latin square assignment provided an oral combination of 1) cetirizine and cimetidine or 2) diphenhydramine and placebo in four divided doses. The study protocol lasted 16 days divided into 4-day intervals, scoring itch before the initial dose of medication and at 1-hour, 6-hour, and 12-hour intervals after the first medication. A significant difference between mean itch scores across the four times was observed (Wilks' Lambda F = 26.52, df = 3, P <.0005). A three

Role of Gastric Acid Suppression Therapy in Erosive Esophagitis: From H2 Receptor Antagonists, Proton Pump Inhibitors, to Potassium-Competitive Acid Blockers.

Erosive esophagitis (EE) is an inflammation of the esophageal mucosa resulting from gastric and duodenal acid reflux, affecting approximately 55% of gastroesophageal reflux disease (GERD) patients in Indonesia. Effective acid suppression is essential for mucosal healing and symptomatic relief. Histamine-2 receptor antagonist (H2RA) was initially used for standard treatment for GERD, including EE, reducing gastric acid secretion by blocking H2 receptors. However, their efficacy is limited by inadequate acid suppression. Proton pump inhibitors (PPIs) became the mainstay therapy due to their stro…

Over-the-Counter Medications in Pregnancy.

High-quality research on the safety and effectiveness of over-the-counter medications in pregnancy is limited. Physicians should explore nonpharmacologic treatments before recommending medication. For nausea and vomiting in pregnancy, vitamin B6 (pyridoxine), H1 antihistamines, and ginger are safe and effective. Physicians can recommend calcium carbonate, H2 antihistamines, and proton pump inhibitors for gastroesophageal reflux disease. Osmotic laxatives, fiber preparations, and probiotics are safe and effective treatments for constipation. Many over-the-counter topical medications are safe in…

Efficacy of oral tofacitinib in refractory chronic spontaneous urticaria and urticarial vasculitis.

The treatment of chronic urticaria (CU) is difficult, currently, antihistamines (AH) are the mainstay of treatment, however, up to 40% of patients do not respond to even high (four-fold) daily doses of AH. Tofacitinib is, a small-molecule that blocks JAK1/3 and inhibits intracellular signaling of multiple key cytokines involved in the inflammatory cascade and its beneficial effects were reported in patients with mast cell activation disease but there is no report in patients with urticaria. Here, we present four cases of refractory CU and one case of urticarial vasculitis (UV) that were manage…

The treatment efficacy of adding prokinetics to PPIs for gastroesophageal reflux disease: a meta-analysis.

BACKGROUND: Gastroesophageal reflux disease (GERD) is a common gastrointestinal disorder. Proton pump inhibitors (PPIs) are first-line drugs for GERD. For those who fail to respond to PPIs, adding prokinetics to PPIs is recommended and several trials have been conducted to evaluate the efficacy of prokinetic-PPI combination therapy. METHODS: A systematic literature search was performed using PubMed and the Cochrane Library databases before February 2019 for randomized controlled trials (RCTs), which compared the efficacy of prokinetics plus PPI treatment with that of PPI monotherapy.
