Combined treatment with medial unicompartmental knee arthroplasty and anterior cruciate ligament reconstruction is effective on long-term follow-up.

PURPOSE: The purpose of the present study was to evaluate the long-term outcome of combined medial unicompartmental knee arthroplasty (UKA) and anterior cruciate ligament reconstruction (ACLR). The authors hypothesized that the combined procedure leads to good long-term outcome in patients with isolated medial knee osteoarthritis (OA) and anterior cruciate ligament (ACL) deficiency. METHODS: Twenty-three patients with ACL deficiency and concomitant medial knee OA were treated from 2008 to 2016 with a combined UKA (Oxford Partial Knee) and ACLR using a hamstring tendon autograft. The follow-up

Role of Matrix-Associated Autologous Chondrocyte Implantation with Spheroids in the Treatment of Large Chondral Defects in the Knee: A Systematic Review.

Autologous chondrocyte implantation (ACI) is a cell therapy for the treatment of focal cartilage defects. The ACI product that is currently approved for use in the European Union (EU) consists of spheroids of autologous matrix-associated chondrocytes. These spheroids are spherical aggregates of ex vivo expanded human autologous chondrocytes and their self-synthesized extracellular matrix. The aim is to provide an overview of the preclinical and nonclinical studies that have been performed to ensure reproducible quality, safety, and efficacy of the cell therapy, and to evaluate the clinical dat

[Knee joint prosthesis implantation after fractures of the head of the tibia. Intermediate term results of a cohort analysis].

A significant number of patients with operated tibial plateau fractures develop osteoarthritis and require total knee arthroplasty. In cases of primary osteoarthritis the results are generally good. However, it is not known whether patients with post-traumatic osteoarthritis obtain comparably favorable results. In a retrospective study we analyzed 72 patients who had undergone a self-aligning (SAL) total knee arthroplasty. Ten patients received arthroplasty due to sequelae of a tibial head fracture (group I). The median time to follow-up in this group was 30 months. Clinical and radiological e

Altered inter-segmental coordination in athletes with a history of concussion.

Concussion-recovered athletes have a higher risk of injury following return to sport. This study investigated the effect of history of concussion on the pattern and variability of inter-segmental coordination in athletes during squat jumps and timed squat and hinge tasks. A human pose estimation algorithm was applied to videos of 111 athletes (72 with no history of concussion (NOHX), 9 within 1 year of concussion (CONC1), 30 more than one-year post-concussion (CONC2) performing a series of movement tasks. Continuous relative phase metrics, calculated from phase angles of two contiguous segment

The Size of Residual Patella Tendon Defect Following Bone-Patella Tendon-Bone Autograft Harvest Does Not Affect Patient-Reported Outcome Measures.

Consequences of ACL reconstruction (ACLR) utilizing the patellar tendon (PT) autograft include a residual defect in the PT and the potential for donor site morbidity, such as anterior knee pain and difficulty kneeling. The purpose of this study was (1) to evaluate the presence and size of PT defects following ACLR and (2) to determine if there is an association with knee pain and function. Patients who underwent ACLR with PT autograft by two surgeons between 2011 and 2023 were identified. One surgeon routinely reapproximated the PT harvest site with suture, and the other left the tendon open w

Enhanced joint pain in diabetic patients with knee osteoarthritis is associated with increased synovitis, synovial immune cell infiltration, and erythrocyte extravasation.

OBJECTIVE: Diabetes mellitus (DM) is an important risk factor for the development of osteoarthritis (OA), increasing OA progression and OA pain. To gain insight into the underlying mechanisms of how DM exacerbates OA processes and OA pain, this study analyzed histological differences of synovial tissues from non-DM and DM patients with OA and correlated these differences with knee pain severity. MATERIALS AND METHODS: Synovial tissue was obtained from 12 non-DM and 10 DM patients with advanced knee OA who underwent total knee arthroplasty. Synovial inflammation was assessed using the Synoviti

Resolving the Conflict Between Regional Analgesia and Developing Compartment Syndrome in Below-Knee Surgeries With the High-Volume Proximal Adductor Canal (Hi-PAC) Block: A Prospective Feasibility Study.

The consideration of regional analgesia (RA) in below-knee surgeries is always a controversial topic due to the fear of masking symptoms of developing compartment syndrome (CS) in the postoperative period. Compartment syndrome (CS) has been found frequently in below-knee surgeries, particularly among tibial diaphyseal fractures. Like any other surgery, below-knee surgeries have significant postoperative pain that requires effective postoperative analgesia protocol. The analgesia quality makes a big difference when compared with or without RA. Also, the presence or absence of RA cannot prevent

Predicting clinical outcomes after total knee arthroplasty from preoperative radiographic factors of the knee osteoarthritis.

BACKGROUND: Total knee arthroplasty (TKA) is the major surgical treatment for end-stage osteoarthritis (OA). Despite its effectiveness, there are about 20% of patients who are dissatisfied with the outcome. Predicting the surgical outcome preoperatively could be beneficial in order to guide clinical decisions. METHODS: One-hundred and ten knees of 110 consecutive patients who underwent TKAs for varus knees resulting from OA were included in this study. Preoperative varus deformities were evaluated by femorotibial angle (FTA), medial proximal tibial angle (MPTA) and lateral distal femoral angl

"Basket weave technique" for medial patellofemoral ligament reconstruction: Clinical outcome of a prospective study.

BACKGROUND: Bone tunneling and implants with rigid fixations for medial patellofemoral ligament (MPFL) reconstruction are known to compromise results and are avoidable, especially in skeletally immature subjects. This study was to assess if these deficiencies were overcome with the technique devised by the author which avoids implants and bone tunnels. Results were assessed for complication rate and outcome. MATERIALS AND METHODS: Fifty six knees of recurrent lateral patellar dislocation were treated in the past 49 months by MPFL reconstruction. Thirty nine were female and 17 male knees. The

Sex differences in rectus femoris morphology across different knee flexion positions.

UNLABELLED: PURPOSEBACKGROUND: Quadriceps function is an important outcome following lower extremity injury and surgery. Measurements of quadriceps function are particularly helpful initially post surgery, however traditional quadriceps strength measures like isokinetic testing are contraindicated during this time period. Inclusion of dynamic musculoskeletal ultrasound imaging in the clinical setting has been beneficial in understanding quadriceps activation specifically rectus femoris (RF) contraction; however, there is a paucity of literature in this area. The purpose of the current study wa

Delayed gadolinium-enhanced MRI of cartilage (dGEMRIC) shows no change in cartilage structural composition after viscosupplementation in patients with early-stage knee osteoarthritis.

INTRODUCTION: Viscosupplementation with hyaluronic acid (HA) of osteoarthritic (OA) knee joints has a well-established positive effect on clinical symptoms. This effect, however, is only temporary and the working mechanism of HA injections is not clear. It was suggested that HA might have disease modifying properties because of its beneficial effect on cartilage sulphated glycosaminoglycan (sGAG) content. Delayed gadolinium-enhanced MRI of cartilage (dGEMRIC) is a highly reproducible, non-invasive surrogate measure for sGAG content and hence composition of cartilage. The aim of this study was
