Temporal changes in key maternal and fetal factors affecting birth outcomes: a 32-year population-based study in an industrial city.

BACKGROUND: The link between maternal factors and birth outcomes is well established. Substantial changes in society and medical care over time have influenced women's reproductive choices and health, subsequently affecting birth outcomes. The objective of this study was to describe temporal changes in key maternal and fetal factors affecting birth outcomes in Newcastle upon Tyne over three decades, 1961-1992. METHODS: For these descriptive analyses we used data from a population-based birth record database constructed for the historical cohort Particulate Matter and Perinatal Events Research

Maternal and perinatal outcome in nulliparous women aged 35 and older.

OBJECTIVE: To compare the maternal and perinatal outcome of nulliparous women 35 years and older at the time of delivery with nulliparous women 25-29 years old. METHODS: A retrospective review of maternal and newborn records of singleton gestations only for first birth in women aged 35 and older (study group n = 143) were compared with pregnancies of women aged 25-29 (control group, n = 148) delivered at the same period with respect to pregnancy complications and outcome. The study was performed at the Princess Badeea Teaching Hospital in North Jordan between January 1, 1996 and July 1, 2000.

Risk factors for neonatal death in twin gestations in the state of South Carolina.

OBJECTIVE: The aim of the study was to describe risk factors for neonatal death among twins born in South Carolina. STUDY DESIGN: South Carolina publicly accessible live birth and infant death cohort files for live-born twins >500 g were reviewed for the years 1988 through 1993. Infants with lethal anomalies were deleted from the cohort before analysis. Risk factors for neonatal death were determined for the entire cohort and for the birth weight categories of 500 to 999 g, 1000 to 1499 g, 1500 to 2499 g, and >/=2500 g. Statistical comparisons were performed with the Mantel-Haenszel chi2 test

Population density and perinatal mortality in Norway and Sweden 1975-1988.

Data from the medical birth registries in Norway and Sweden were used to study geographical variations in perinatal mortality in the two countries. The study population comprised 1.4 million single births during 1975-79 and 1985-88. Perinatal death (n=9,834) for infants with a birthweight of 1,000 g or more was the main measure of outcome. In both periods and in both countries, perinatal mortality was inversely associated with population density and annual number of births, with relative risks amounting to 1.4 in Norway and 1.2 in Sweden. Among live births, these gradients were observed in dea

[New perspectives for an old problem: premature labor].

Preterm birth has been and continues to be one of the most serious problems in Obstetrics. It is the most common cause of neonatal death, and morbidity in surviving infants. The presence of bacterial vaginosis, and other genital infections is associated with increased risk of preterm labor. Cytokines promote the release of prostaglandin, and have been implicated as cause of uterina activity. Recently, new methods of detection as transvaginal ultrasonography, and measures of cervical fetal fibronectin have been acclaimed as useful to detect the problem. Atosiban, cytokines, glyceril trinitrate,

Expectant management of twin pregnancy with single fetal death.

OBJECTIVE: To evaluate the course and outcome of expectantly managed twin pregnancies with single fetal death in the second half of gestation. DESIGN: Case-controlled study of 29 consecutive pregnancies from 1973 to 1993, with sonographic evidence of a twin at 20 weeks gestation with antenatal demise later in pregnancy, matched for maternal parity with 58 twin pregnancies without fetal death and delivered in the same year as the index case. Outcome measures included the incidence of complications of pregnancy, gestational age and mode of delivery, placentation, and perinatal outcome. SETTING

Female mortality in reproductive ages in Dar es Salaam, Tanzania.

An audit of all cases of female mortality in the reproductive age groups has been performed in Dar es Salaam during 1991-93. Multiple sources of information was used to collect information on deaths. None of the sources of information on death was alone adequate. 73% of the interviews was done at home. The mother or another close relative was interviewed in more than 70% of the cases. A total of 645 deaths were identified and 18% of these were related to pregnancy. Fever, shortness of breath and weight loss were the most common symptoms before death followed by pallor and vomiting. Almost 90%

Reports on confidential enquiries into maternal deaths: an audit of previous recommendations.

An audit of facilities recommended to be available in consultant obstetric units (COUs) in the United Kingdom (UK) received responses from 248 (95%) of all 260 COUs. Of the respondents, 86% were on the site of an acute general hospital, 76% had an intensive therapy unit on site, and 87% had a blood bank on site. Eighty-two per cent had a protocol for management of massive haemorrhage, and 91% had a local protocol for the management of severe pre-eclampsia/eclampsia. The development of Regional protocols and advisory centres for the management of hypertensive disorders is at present fragmentary

The impact of maternal age on pregnancy and its outcome.

There were 28,600 deliveries of 500 g or more to women at the Rotunda Hospital between January 1st 1985 and December 1st 1989. Of these, 595 were to women aged 40 years and over. Thirty-five variables of clinical significance were analyzed, comparing those of 40 years of age and more with those under 40. The older group had significant increases in gestational diabetes, ante-partum hemorrhage, fetal distress, prematurity, low birth weight and perinatal mortality. Chromosome congenital abnormalities were significantly higher, particularly Down syndrome. There were significantly increased rates

Development and validation of a multivariate predictor of mortality in very low birth weight.

Accurate prognosis is critical to the design of all prospective research aimed at improving survival. Predictions based on birth weight, gestational age, or any other single variable, fail to take into account the potentially important contribution of other factors. In order to develop a practical and accurate multivariate model, we studied all singleton pregnancies resulting in viable liveborn infants who weighed less than or equal to 1500 g at birth during 1984 and 1985 at the New York Hospital-Cornell Medical Center. When gestational age, birth weight, and/or crown-heel length were consider

Reproductive performance following active management of diabetic pregnancies at the University College Hospital, Ibadan, Nigeria.

This article presents a report of the first 5 years of an active approach to the management of diabetic pregnancies at the University College Hospital, Ibadan. During this period the incidence rate of diabetic pregnancy was 0.64 per 1000 deliveries per year. The mean birthweight (3.40 +/- 1.68 kg) of babies whose mothers had good diabetic control was lower than the mean birthweight (4.19 +/- 1.05 kg) of babies born to mothers with poorly-controlled diabetes. The difference was however not statistically significant (P less than 0.5). Whilst the overall perinatal mortality rate was 10.8%, there

Amphetamine addiction and pregnancy. II. Pregnancy, delivery and the neonatal period. Socio-medical aspects.

The adverse effects of amphetamine addiction during pregnancy and the neonatal period were studied in 69 Swedish women. Almost one-third of the women (Group I) succeeded in overcoming their addiction in early pregnancy. The women in Group I (n = 17), unlike those in Group II (n = 53), received the same amount of prenatal care as the average Swedish woman. An increased rate of preterm deliveries (25%) as well as a higher perinatal mortality (7.5%) was found in Group II. During the neonatal period an increased incidence of mother-infant separation was found since many of the infants (46%) were t

Birthplace and attendants: Oregeon's alternative experience, 1977.

In October of 1977 an article entitled "Birthplace Alternatives" appeared in the Oregon State Health Bulletin. This article presented a narrative based on facts taken from the data on birth certificates of all births occurring outside of a hospital in the State of Oregon in 1976. Tabulations were compiled to show where the infants were being born, who was delivering them, the educational level of the parents, and the prenatal care given. Insofar as was possible--from complications listed on the birth certificates, information from matched infant death reports, and full term fetal death reports

A prospective study of drugs and pregnancy. II. Anti-emetic drugs.

In a prospective study of anti-emetic drug use during 6 376 pregnancies, the previously demonstrated negative correlation between the use of such drugs and miscarriage was confirmed. It is probably completely due to less frequent and possibly less severe morning sickness complaints in pregnancies ending with a miscarriage. A slight positive correlation is found between the use of anti-emetics and infant malformation. A detailed analysis showed that this was mainly due to a high incidence of drug use (mainly promethazine) among women giving birth to infants with congenital dislocation of the hi

Chromosome studies in investigation of stillbirths and neonatal deaths.

Chromosome studies have been attempted on all late abortions, stillbirths, and neonates coming to necropsy in Edinburgh over a 1-year period. Results were obtained in 153 cases and of these 7·2% had chromosome abnormalities. It is suggested that chromosome studies should become a routine part of the perinatal necropsy.
