Combining 24-Hour Continuous Monitoring of Time-Locked Heart Rate, Physical Activity and Gait in Older Adults: Preliminary Findings.

Hemodynamic homeostasis is essential for adapting the heart rate (HR) to postural and physiological changes during daily activities. Traditional HR monitoring, such as 24 hour (h) Holter monitoring, provides important information on homeostasis during daily living. However, this approach lacks concurrent activity recording, limiting insights into hemodynamic adaptation and our ability to interpret changes in HR. To address this, we utilized a novel wearable sensor system (ANNE@Sibel) to capture time-locked HR and daily activity (i.e., lying, sitting, standing, walking) data in 105 community-dw

If it does not help, it might hurt: Pharmacodynamics of a second IVIg course in Guillain-Barré syndrome.

OBJECTIVES: Intravenous immunoglobulin (IVIg) is an effective treatment for Guillain-Barré syndrome (GBS), but recovery varies between patients. This study aims to evaluate the pharmacokinetics (PK) and pharmacodynamics (PD) of a single and a second IVIg dose (SID) in patients with GBS. METHODS: Data were analyzed from the SID-GBS trial, a double-blind, randomized, placebo-controlled study. Patients with poor prognosis (modified Erasmus GBS Outcome Score, mEGOS ≥6) after a standard course of IVIg (0.4 g/kg for 5 days) were randomized to receive either SID or placebo. Serum IgG levels were mea

Serum microRNAs as peripheral markers of primary aldosteronism.

BACKGROUND: Primary aldosteronism (PA) is the principal cause of secondary hypertension; it leads to significantly elevated cardiovascular morbidity and mortality, but only a fraction of its cases ever get detected, partially due to diagnostic procedures that are difficult to perform and to interpret. More straightforward diagnostic methods are needed. Lateralized, or unilateral PA (uPA), is best treated by surgery. Bilateral PA (bPA) is treated medically. AIM: The aim of our study was to explore microRNA (miRNA) in peripheral blood as markers of PA, uPA and bPA. METHODS: In groups of subjec

Health-related quality of life in cancer survivors aged 65 and older: impact of diagnosis and treatment and perceived decline associated with aging.

PURPOSE: The health-related quality of life (HRQOL) among older adult cancer survivors can be influenced by physiological and psychosocial challenges stemming from treatment and disease experience. Despite assumptions that aging might exacerbate these difficulties, research indicates higher HRQOL in this subgroup compared to younger adult cancer survivors. OBJECTIVES: (i) to analyze HRQOL in this subgroup, contrasting with younger survivors and (ii) to explore potential moderating factors related to sociodemographic and disease variables. METHOD: A cross-sectional study was conducted involvi

Genetic Heterogeneity Underlying Familial Short Stature.

Background and Clinical Significance: Familial short stature is a common reason for referral in clinical genetics. While often attributed to a single genetic cause, genetic heterogeneity can complicate diagnosis and management. This report describes a family in which three distinct pathogenic variants in SHOX, PDE4D and ACAN caused overlapping phenotypes of familial short stature. Case Presentation: Clinical, radiological and molecular data were collected retrospectively at the Reference Centre for Constitutional Bone Diseases at Montpellier University Hospital. Targeted gene panels, whole gen

Association of glycemic variability with short- and long-term mortality in critically ill patients with trauma.

The relationship between glycemic variability (GV) and mortality in critically ill trauma patients remains unclear. We evaluated whether GV, quantified by the coefficient of variation (CV) of glucose during the ICU stay, is associated with short- and long-term mortality in this population. Using the Medical Information Mart for Intensive Care IV (MIMIC-IV) database, we identified 4,009 adult trauma patients admitted to the intensive care unit and assessed the association between CV and 30-day and 1-year mortality with Cox proportional hazards models, Kaplan-Meier analysis, restricted cubic spl

Scapulothoracic Disorders: Anatomy, Kinematics, Clinical Assessment, and Multimodality Imaging.

Shoulder and neck concerns are prevalent musculoskeletal issues prompting medical attention, often stemming from scapulothoracic disorders that can serve as both the cause and consequence of other shoulder abnormalities. Scapular dyskinesis, fractures, benign and malignant masses, and neuropathies are frequently overlooked yet can substantially affect shoulder pain and function, particularly in athletes. Scapulothoracic disorders may lead to and/or be worsened by common shoulder abnormalities including those of the rotator cuff, neighboring bursae, and the glenohumeral articulation and labrum.

A Novel SIL1 Variant (p.E342K) Associated with Marinesco-Sjögren Syndrome Impairs Protein Stability and Function.

Marinesco-Sjögren syndrome (MSS) is a rare autosomal recessive neuromuscular disorder marked by ataxia, muscle weakness, cataracts, and often intellectual and skeletal abnormalities. It is commonly caused by loss-of-function variants in the SIL1 gene, which impair binding immunoglobulin protein (BiP) function, leading to protein misfolding and activation of the unfolded protein response. In a 2-year-old patient with typical MSS symptoms, we identified a previously unreported c.1024G>A (p.E342K) variant in SIL1 via whole-exome sequencing. The pathogenicity of this Sil1 variant was supported by
