Hypo- and Hyper-Assembly Diseases of RNA-Protein Complexes.

A key aspect of cellular function is the proper assembly and utilization of ribonucleoproteins (RNPs). Recent studies have shown that hyper- or hypo-assembly of various RNPs can lead to human diseases. Defects in the formation of RNPs lead to 'RNP hypo-assembly diseases', which can be caused by RNA degradation outcompeting RNP assembly. By contrast, excess RNP assembly, either in higher order RNP granules, or due to the expression of repeat-containing RNAs, can lead to 'RNP hyper-assembly diseases'. Here, we discuss the most recent advances in understanding the cause of disease onset, as well

Phosphorylated neurofilament heavy chain in cerebrospinal fluid and plasma as a Nusinersen treatment response marker in childhood-onset SMA individuals from Serbia.

INTRODUCTION: Biomarkers capable of reflecting disease onset and short- and long-term therapeutic effects in individuals with spinal muscular atrophy (SMA) are still an unmet need and phosphorylated neurofilament heavy chain (pNF-H) holds significant promise. METHODS: We conducted a longitudinal prospective study to evaluate pNF-H levels in the cerebrospinal fluid (CSF) and plasma of 29 individuals with childhood-onset SMA treated with Nuinersen (SMA type 1: n = 6, 2: n = 17, 3: n = 6). pNF-H levels before and during treatment were compared with the levels of controls (n = 22), patients with

Burden of rare genetic disorders in India: twenty-two years' experience of a tertiary centre.

BACKGROUND: Rare disorders comprise of ~ 7500 different conditions affecting multiple systems. Diagnosis of rare diseases is complex due to dearth of specialized medical professionals, testing labs and limited therapeutic options. There is scarcity of data on the prevalence of rare diseases in different populations. India being home to a large population comprising of 4600 population groups, of which several thousand are endogamous, is likely to have a high burden of rare diseases. The present study provides a retrospective overview of a cohort of patients with rare genetic diseases identified

Cost Trends of New-To-Market Neurologic Medications: An Insurance Claims Database Analysis.

BACKGROUND AND OBJECTIVES: Costs for neurologic medications have increased considerably in recent years. Since 2014, more than 30 neurologic medications have been approved by the US Food and Drug Administration (FDA) for neurologic conditions. This study aims to characterize recent trends in annual costs and aggregate spending from 2012 to 2021 for new-to-market (NTM) medications for 9 neurologic conditions. METHODS: We used the Merative MarketScan commercial and Medicare supplemental databases to observe patients seen by a neurologist with neurologic diseases with newly FDA-approved medicati

Exploring factors impacting haplotype-based noninvasive prenatal diagnosis for single-gene recessive disorders.

Haplotype-based noninvasive prenatal diagnosis (NIPD) is applicable for various recessive single-gene disorders in proband families. However, a comprehensive exploration of critical factors influencing the assay performance, such as fetal fraction, informative single nucleotide polymorphism (SNP) count, and recombination events, has yet to be performed. It is critical to identify key factors affecting NIPD performance, including its accuracy and success rate, and their impact on clinical diagnostics to guide clinical practice. We conducted a prospective study, recruiting 219 proband families w

Population Physiologically-Based Pharmacokinetic Modeling to Determine Ontogeny: A Quantitative Clinical Pharmacology Example in Pediatric Rare Disease.

Pediatric physiologically-based pharmacokinetic (PBPK) modelling plays an increasing role in selecting doses in children and addressing clinical pharmacology questions. Ethical concerns often limit clinical pharmacology studies that have no direct therapeutic benefit in children, highlighting the value of PBPK model predictions. However, regulatory acceptance of pediatric PBPK models remains limited because of uncertainties in system-specific information and inadequate model qualification. Ambiguous ontogeny data of drug metabolizing enzymes (DME) and transporters are recognized as significant

Navigating new motor function trajectories: Consensus recommendations for assessment in the era of newborn screening and early treatment in SMA.

BACKGROUND: Approved disease modifying treatments (DMTs) and the implementation of newborn screening for spinal muscular atrophy (SMA) have allowed individuals treated early to gain new motor abilities and achieve more typical motor milestones. In this changing landscape, current motor function outcome measures (MF-OMs) used in SMA clinical practice may no longer be sufficiently sensitive enough to optimally quantify new disease trajectories and evolving functional progression. OBJECTIVE: To develop a consensus-derived recommendation for MF-OMs to be used in assessing infants and children liv

The silica-based sorbents for the mixed-mode extraction of risdiplam and its metabolite from serum samples.

Analysis of risdiplam and its main metabolite is critical for pharmacokinetic studies and therapeutic drug monitoring in spinal muscular atrophy. However, current analytical methods face challenges due to low analyte stability, limited selectivity, and pronounced matrix effects, with only four studies published to date. In this work, a comprehensive analytical strategy was developed for the first time, combining optimized chromatographic separation with a novel sample preparation approach. The effects of methanol content, salt concentration, and pH on retention, peak symmetry, and resolution w

Characterisation of the SMN1/2 locus using a highly specific variant caller on whole-genome sequence data from 500,000 individuals.

Spinal muscular atrophy (SMA) is a rare autosomal recessive condition caused by biallelic loss of the SMN1 gene that results in severe lower motor neuron degeneration in childhood. Targeted pre-symptomatic treatments have been shown to dramatically improve outcomes, supporting the inclusion of SMA in newborn screening programmes. However, ~95% of infantile-onset cases are caused by homozygous deletions of exon 7-8 of the SMN1 gene, which is challenging to detect using short-read next-generation DNA sequencing technology due to the high sequence similarity between SMN1 and its nearby paralog SM

A Prospective, Multi-Site Study of Performance of Cell-Free DNA Testing for Recessive Conditions in a Large, General-Risk Pregnancy Population.

OBJECTIVE: To evaluate the clinical performance of cell-free DNA (cfDNA) screening as a primary screening tool for autosomal recessive conditions in a large, prospective, general-risk population. METHODS: We conducted a prospective analysis of pregnant individuals who underwent carrier screening with a reflex cfDNA fetal risk assessment at nine participating U.S. institutions. The conditions evaluated included cystic fibrosis, spinal muscular atrophy, and beta and alpha hemoglobinopathies. The cfDNA results were categorized as high risk (at least 1-in-4 predicted fetal risk) or low risk. Test

Using Latent Representations to Link Disjoint Longitudinal Data for Mixed-Effects Regression.

Many rare diseases offer limited established treatment options, leading patients to switch therapies when new medications emerge. To analyze the impact of such treatment switches within the low sample size limitations of rare disease trials, it is important to use all available data sources. This, however, is complicated when the use of measurement instruments changes during the observation period, for example when instruments are adapted to specific age ranges. The resulting disjoint longitudinal data trajectories complicate the application of traditional modeling approaches like mixed-effect

Cross-disease LC-MS/MS plasma proteomics identifies reproducible shared and disease-enriched biomarker signatures in neurodegenerative disorders.

Neurodegenerative diseases (NDDs) exhibit considerable molecular heterogeneity, making it difficult to pinpoint robust, disease-specific biomarkers. Although proteomic studies have deepened our understanding of individual disorders, systematic cross-disease comparisons with cross-platform validation remain scarce, especially for rare conditions like spinal and bulbar muscular atrophy (SBMA). To address this gap, we conducted a comparative plasma proteomic analysis using liquid chromatography-tandem mass spectrometry (LC-MS/MS) in 264 participants across major neurodegenerative and related diag

Rare diseases in the Turkish-Cypriot community: a nationwide study.

Minor or genetically isolated populations like Turkish-Cypriots (TC) are usually challenging to diagnose and treat for uncommon genetic diseases. TC may exhibit several patterns of unusual genetic disorders based on their unique historical and demographic conditions. The objective of the current study is to identify and investigate the rarest genetic disorders in TC patients. Therefore, between 2019 and 2025, clinical and genetic data, which were confirmed by gene panels and exome sequencing, from 150 TC patients were retrospectively analysed in our clinic. Also, inheritance models and variant

Growth Hormone Strongly Induces hSMN2 Promoter Driving Construct Gene Expression in Mammalian Cells.

OBJECTIVE: Spinal muscular atrophy (SMA) is the most common neurodegenerative disease caused by the absence or insufficiency of the survival motor neuron (SMN) protein. Human SMN1 (hSMN1) produces fully functional SMN protein but hSMN2 produces only about 10% functional protein. Deletion or mutation in hSMN1 gene leads to SMA, while the hSMN2 copy number modifies disease severity. Increasing hSMN2 expression has emerged as a potential therapeutic approach. In this study, we investigated the effect of growth hormone (GH) on hSMN2 promoter activity using a reporter in Chinese hamster ovary (CHO)
