Sinus Infections, Inflammation, and Asthma.

There is an important link between the upper and lower respiratory tracts whereby inflammation in one environment can influence the other. In acute rhinosinusitis, pathogen exposures are the primary driver for inflammation in the nose, which can exacerbate asthma. In chronic rhinosinusitis, a disease clinically associated with asthma, the inflammation observed is likely from a combination of an impaired epithelial barrier, dysregulated immune response, and potentially infection (or colonization) by specific pathogens. This review explores the associations between rhinosinusitis and asthma, wit…

Injection Rhinoplasty Using Filler.

Rhinoplasty is a commonly performed cosmetic surgery in Asia. Rhinoplasty using filler is preferred because has fewer side effects and shorter down time. The part of external nose between the skin and bone or cartilages consists of 4 layers. To prevent vascular compromise, the injection should be into deep fatty layer, preventing embolization. Filler is usually injected in the order of radix, rhinion, tip, and the supratip area. To minimize asymmetry, the surgeon should always mark the midline on the nasal bridge and perform the procedure without deviating from it.

Principles and practice of reconstructive surgery for head and neck cancer.

Reconstruction after surgical treatment of head and neck cancers can be challenging. Goals for reconstruction include restoration of appearance as well as function when appropriate. Commonly encountered sites requiring reconstruction include the soft tissues of the face (including the critical areas of the eyes, ears, nose, and lips), scalp, tongue and oral cavity, maxilla, mandible, and pharynx. Advanced reconstructive techniques using microsurgery may be preferable to simpler techniques to obtain optimal outcomes. In this article, techniques for reconstruction in these areas as well as antic…

[Management of frostbite in and outside of the doctor's surgery].

Frostbite is most likely to happen in combination with accidents, intoxications or psychiatric emergencies and typically affects smaller, more exposed areas of the body, such as fingers, toes, nose, ears, cheeks and chin. The preclinical treatment consists of rapid rewarming of the injured tissue in a water-bath that is held between 38 - 42 °C (hand-hot) in a stable, warm environment. A new clssification allows a faster assessment of the outcome. New retrospective studies suggest the efficacy of thrombolysis or prostacyclin analogues against spasm and thrombosis for a better outcome.

Treatment of a double-giant Rhinophyma with electrocautery and Versajet hydrosurgery system.

Rhinophyma is a disfiguring condition etiologically related to rosacea and due to hypertrophy of the sebaceous glands of the nose. It leads to a progressive thickening of the skin up to the development, in some cases, of severe deformities that result in significant functional deficits and serious cosmetic damage. We report a case of giant rhinophyma consisting of 2 large masses that interfered with feeding and respiration of the patient, and we describe the surgical treatment by resection with electrosurgery and razor-thin saline jet (Versajet Hydrosurgery System). This combined approach is s…

[Standard rhinoplasty].

Most patients who consult a surgeon for rhinoplasty do not want a radical change in their nose. They seek a reduction in the volume of the nasal pyramid and correction of a precise element that they judge to be ungainly--most often an osteocartilaginous hump. The procedure that we qualify as "standard" will eliminate the osteocartilaginous hump, decrease the dimensions of the septum and reduce the size of the alar crus of the alar cartilage. Although the required technical maneuvers are simple, their sequence must be coherent with a few basic rules that are simple but rarely explained in order…

[Hands cutaneous exam].

There are four different areas to consider: palms, back of the hands, fingers, periungual folds (and nails). Palmoplantar keratodermas are a group of inherited or acquired disorders. Dysidrosis is a peculiar form of eczema on the palms and lateral aspects of the fingers. SAPHO syndrome (Synovitis - Acne - Pustulosis - Hyperostosis - Osteomyelitis) presents pustules on palms. Photo-ageing is frequently noticed on the back of the hands. Paraneoplastic acrokeratosis (Bazex syndrome) affects the nose, ears, and periungual folds of fingers and toes. Spontaneous blue finger syndrome can be a benign …

Cholesterol granuloma of the frontal sinus: a case report.

Cholesterol granulomas are common in the mastoid antrum and air cells of the temporal bone. In the paranasal sinuses, especially in the frontal sinus, they have occasionally been mentioned in the literature. The pathogenesis is unknown, but the majority of the authors support the concept of airway obstruction in the cells well pneumatised of temporal bone and paranasal sinuses. The authors report a case of cholesterol granuloma of the frontal sinus treated with radical surgical techniques, and they also recommend an endoscopic approach to frontal sinus to restore or enlarge the nose-frontal ca…

The otolaryngologist's approach to the patient with Down syndrome.

As more patients with Down syndrome are living into adulthood, attention has focused on health factors that affect the quality of the patient's life and their ability to reach full potential. Patients with Down syndrome have several morphologic abnormalities that predispose them to problems with the ear, nose, and throat, and appropriate treatment can have a significant impact on the quality of life of these patients. Otolaryngologists are likely to see many patients with Down syndrome throughout their careers. This article reviews the literature to provide information and recommendations rega…

Using acoustic sensors to discriminate between nasal and mouth breathing.

The recommendation to change breathing patterns from the mouth to the nose can have a significantly positive impact upon the general well being of the individual. We classify nasal and mouth breathing by using an acoustic sensor and intelligent signal processing techniques. The overall purpose is to investigate the possibility of identifying the differences in patterns between nasal and mouth breathing in order to integrate this information into a decision support system which will form the basis of a patient monitoring and motivational feedback system to recommend the change from mouth to nas…

Epididymo-orchitis as a presenting feature of relapsing polychondritis: a case report.

Relapsing polychondritis (RP) is a rare multisystem disease. It is characterized by recurrent inflammation of cartilaginous structures including the ears, nose, tracheo-bronchial tree and peripheral joints. Proteoglycan-rich structures such as the heart, eyes and blood vessels can also be affected. Systemic symptoms including fever, weight loss and lethargy are common. RP is difficult to diagnose as it presents in a wide variety of ways and there is no diagnostic test. Corticosteroids are the mainstay of treatment but other immunosuppressive drugs can be used in combination with steroids. We p…

[Combined approaches to diagnostics and treatment of traumatic otitis].

A total of 216 patients with the injured tympanic membrane were examined and treated during the last 5 years. 198 (91.6%) patients presenting with traumatic otitis were found to have pathological changes in the nose and paranasal sinuses. In patients who applied for medical aid soon (within 1-2 weeks) after injury to the tympanic membrane, the resulting perforation was closed with the use of hen's egg amniotic membrane. Audiological examination demonstrated that 84.7% of traumatic tympanic membrane ruptures were associated with acute sensorinerual loss of hearing.

Orbital emphysema after a protracted episode of sneezing in a patient with no history of trauma or sinus surgery.

Orbital emphysema is a benign self-limiting condition. It can occur directly (as a result of trauma to the face) or indirectly (secondary to a blowout fracture). We report a case of orbital emphysema in a 38-year-old man who presented with ecchymosis of the right eye, pressure within the right orbit, and periorbital swelling following a protracted episode of vigorous sneezing. The diagnosis was confirmed by computed tomography. Systemic antibiotics were given, and the patient was cautioned to avoid blowing his nose. His signs and symptoms resolved within 1 week.

Median cleft of the upper lip: a rare case.

Median cleft lip is a midline vertical cleft through the upper lip. This is a rare anomaly with very few cases described in the literature. Median cleft lip has been thought to be associated with hypertelorism or hypotelorism. It is caused by the failure of fusion of the medial nasal prominences. In this case report, a 4½-year-old girl with a median cleft is presented. The patient has no other anomalies of the nose or alveolus. The patient has normotelorism. A shortened bifid frenulum of the upper lip is present. A technique using an elliptical excision of the cleft with muscle approximation i…

Biofilm--the other name for the regular mucosal blanket.

Bacterial biofilms have been found in many human bacterial infectious processes including chronic rhinosinusitis. It was found also at the surface of nasal polyps, and both at the surface of the nasal part and the stalk of the antrochoanal polyp. However, our recent research clearly demonstrated the presence of the biofilm at the surface of the mucosa of the healthy paranasal sinuses as well. Our hypothesis therefore is that the biofilm in the nose and paranasal sinuses is nothing else but regular respiratory mucosal blanket, a part of the mucociliary system itself, containing variable number …

Disseminated cutaneous coccidioidomycosis masquerading as lupus pernio.

Coccidioides immitis, a pathogenic fungus endemic to arid regions, is the etiologic agent of cutaneous coccidioidomycosis. Primary cutaneous coccidioidomycosis is rare. The majority of cutaneous coccidioidomycosis infections are caused by dissemination of the fungus from the lungs to the skin. Diagnosing cutaneous coccidioidomycosis often can be difficult because it can mimic a variety of other clinical conditions. We present a case of a 45-year-old man presenting with cutaneous coccidioidomycosis on the tip of his nose. This patient had pulmonary symptoms in addition to his cutaneous findings…

Wegener's granulomatosis: a rare presentation.

Wegener's granulomatosis (WG) is a necrotizing granulomatous vasculitis involving the nose, paranasal sinuses, lungs, and kidneys. There are two types of WG-systemic, which is characterized by focal segmental necrotizing glomerulonephritis and limited in which the kidneys are spared. Without proper immunosuppression, WG can be aggressive and often fatal. There are very few reports on WG presenting as parotitis and lacrimal gland involvement. We report a lady who presented recurrent parotitis, focal segmental glomerulosclerosis, and orbital cellulitis, in whom the final diagnosis was revealed a…

The management of alar columellar disproportion in revision rhinoplasty.

An otherwise attractive nose can be diminished aesthetically if the relationship of the nostril border and ala to the columella is not refined and proportional. Compared with other aspects of rhinoplasty, there has been little attention devoted to the proper diagnosis and treatment of alar columellar disproportion. This article highlights the relevant anatomic components, defines the proper alar columellar relationship, systematically analyzes the different types of alar columellar dis proportion, and stresses the importance of identifying the causative factors in formulating the optimal treat…
