BACKGROUND: Metformin is the first-line treatment for type 2 diabetes mellitus (T2DM). Long-term use of metformin has been associated with vitamin B12 deficiency, which may lead to serious complications such as anaemia and neuropathy. Although international bodies have recommended screening for vitamin B12 deficiency in patients on long-term metformin, it is unclear how aware clinicians are of this adverse effect and to what extent such guidance is being followed in practice.
METHODS: A scoping review was conducted using Joanna Briggs Institute (JBI) methodology. Databases searched included M

This paper presents a reflective account of the implementation of the Safe Steps for De-escalation practice model over a 12-month period across three acute mental health units. It describes the implementation process of the model, designed to reduce restrictive practices in acute mental health settings. The Safe Steps model is a mental health nurse-designed and led approach for person-centred, structured and relational de-escalation. It positions therapeutic engagement, emotional attunement, and psychological safety as central components of effective responding, particularly during high-stress

BACKGROUND: Cardiothoracic surgery patients have an increased risk for aspiration and may require enteral access for nutrition.
LOCAL PROBLEM: In a cardiothoracic intensive care unit, feeding start times were delayed because of scheduling conflicts with support services. An electromagnetic device (Cortrak 2 Enteral Access System, Avanos Medical) was introduced to allow advanced practice providers (nurse practitioners and physician assistants) to independently establish postpyloric access and reduce dependence on ancillary services.
METHODS: A quality improvement study was performed. Pre- and

BACKGROUND: Neonatal intensive care units (NICUs) commonly utilize peripherally inserted central catheters (PICCs) to provide nutrition and long-term medications to premature and full-term infants. However, little is known about PICC practices in these settings.
PURPOSE: To assess PICC practices, policies, and providers in NICUs.
METHODS: The Neonatal PICC1 Survey was conducted through the use of the electronic mailing list of a national neonatal professional organization's electronic membership community. Questions addressed PICC-related policies, monitoring, practices, and providers. Descr

Genetic counselling for presymptomatic testing is complex, bringing both ethical and practical questions. There are protocols for counselling but a scarcity of literature regarding quality assessment of such counselling practice. Generic quality assessment tools for genetic services are not specific to presymptomatic testing (PST). Therefore, the aim of this study was to identify aspects of effective counselling practice in PST for late-onset neurological disorders. We used the Delphi method to ascertain the views of relevant European experts in genetic counselling practice, ascertained via pu

OBJECTIVES: Description of methodological issues in a trial designed to evaluate if a multi-component intervention aimed at vascular risk factors can prevent dementia.
DESIGN, SETTING AND PARTICIPANTS: Multi-center, open, cluster-randomized controlled clinical trial (preDIVA) including 3535 non-demented subjects aged 70-78, executed in primary practice and coordinated from one academic hospital. General practices are randomized to standard care or intensive vascular care.
INTERVENTION: Vascular care consists of 4-monthly visits to a practice nurse who monitors all cardiovascular risk factors

OBJECTIVE: To evaluate differences in practice patterns between aerodigestive and nonaerodigestive providers in pediatric gastroenterology when diagnosing and treating common aerodigestive complaints.
STUDY DESIGN: A questionnaire comprised of clinical vignettes with multiple-choice questions was distributed to both aerodigestive and nonaerodigestive pediatric gastroenterologists. Vignettes focused on management of commonly encountered general gastroenterology and aerodigestive issues, such as gastroesophageal (GE) reflux, aspiration, and feeding difficulties. Tests of equal proportions were

INTRODUCTION: The prevalence of postpartum hemorrhage has increased in the United States despite the international promotion of active management of the third stage of labor. Adherence to the international recommendations in the United States is unclear. It is also not known how the components of active management are related to other practices that may be used during the third stage of labor. The purpose of this study was to determine routine practice patterns for managing the third stage of labor in the United States.
METHODS: A randomized national survey was conducted of certified nurse-mi

BACKGROUND: Interprofessional education (IPE) in palliative care is essential for fostering collaborative, patient-centred practice. However, existing programs often underrepresent non-medical professions and insufficiently address spiritual care, ethical reflection, and experiential learning, despite the relevance of these aspects to comprehensive, holistic palliative care. This study evaluates an innovative interprofessional curriculum integrating the Canadian Interprofessional Health Collaborative (CIHC) framework, Social Identity Theory (SIT), and Transformative Learning Theory (TLT), desi

BACKGROUND: This review explores strategies for implementing person-centred care (PCC) for older adults in institutional care settings with palliative care needs. Such settings present complex challenges, including loss of autonomy, multiple chronic conditions and psychological or existential distress. Integrating PCC principles with palliative care is an appropriate approach as it promotes shared decision-making, dignity, comfort and well-being. This approach contributes to individual satisfaction and the sustainability of care systems.

BACKGROUND: Population ageing and rising levels of non-communicable diseases are increasing the number of people living with and dying from advanced serious illnesses globally. Many of these people are hospitalised more than once in their last year of life. While there is sound evidence on what patients and their families require for safe and high-quality hospital palliative care, enabling this remains a challenge. This study aimed to understand the clinician, team, and organisational-level barriers and enablers to integrating good palliative care into acute care.
METHODS: An exploratory-desc

UNLABELLED: Optimal nutrition is an important part of the therapeutic process offered to patients in long-term care, as it can significantly influence their nutritional and health status. The aim of this study was to assess the impacts of a dietary intervention on the nutritional status, clinical outcomes and selected nutrient and salicylate intakes among older adults living in a long-term care nursing home. To achieve the research goal, a prospective, non-randomized, baseline-controlled intervention study was conducted. The study was conducted within the framework of the "Senior's Plate Proje

OBJECTIVE: To compare the length of the second stage in nulliparous women with respect to a range of factors related to staffing and outcomes at a community hospital.
METHODS: This was a retrospective cohort study of nulliparous women presenting for delivery over a 2-year period. The primary outcomes were the presence of passive descent and length of the second stage. Secondary outcomes were mode of delivery, chorioamnionitis, and postpartum hemorrhage.
RESULTS: Passive descent was observed in 198 women in the second stage. When passive descent occurred, the differences in the median length

BACKGROUND: Extremely low birth-weight (ELBW) infants frequently receive packed red blood cell (PRBC) transfusions. Recent studies have shown that more restrictive PRBC transfusion guidelines limit donor exposure and reduce transfusion-related costs without any increase in adverse clinical outcomes.
PURPOSE: We developed and implemented an evidence-based PRBC transfusion guideline for ELBW infants treated in our unit and then measured provider adherence to this guideline.
METHODS/SEARCH STRATEGY: We performed a retrospective review of all PRBC transfusions given to ELBW infants in 2012 (preg

BACKGROUND: Occupational acquisition of blood-borne infections has been reported following exposure to blood or body fluids. Consistent adherence to standard precautions will reduce the risk of infection.
OBJECTIVES: To identify: the frequency of self-reported adverse exposure to blood and body fluids among surgeons and scrub nurses during surgical procedures; contributory factors to such injuries; the extent of compliance with standard precautions; and factors influencing compliance with precautions.
DESIGN: A multi-site mixed methods study incorporating a cross-sectional survey and intervi

MATERIALS AND METHODS: Statutes, administrative codes, and medical, nursing, and cosmetology board guidance were reviewed for all 50 US states and the District of Columbia. Five office-based cosmetic procedure
