[Operative Approaches to the distal radioulnar Joint and distal Ulna].

At the distal radioulnar joint (DRUJ) and the ulnocarpal joint several anatomical structures are in a limited space. Therefore, the operative approach has to consider the pathology to be treated. The DRUJ may be approached from the dorsal or the palmar side. Procedures to treat the arthrotically destroyed DRUJ involving bone resection of the ulnar head and destabilizing ligamentous injuries of the triangular fibrocartilage complex (TFCC) are best approached from the dorsal side. This approach allows excellent visualization of the ulnar head and the ulnar and radial attachment of the TFCC. The

Biodegradable versus titanium plates and screws for paediatric facial skeleton fractures.

OBJECTIVE: The use of biodegradable materials represents a new option in the treatment of paediatric facial skeleton fractures. The benefits of a resorbable system include reductions in time for long-term stability, diminished immobilisation period, and elimination of painful procedures for implant removal. The resorption of the material did not influence bone repair and growth. Bioresorbable plates and screws get completely excreted through physiological routes. MATERIAL AND METHODS: The age of the patients ranges from that of pre-schoolers till 18 years. The mean age of boys and girls was 1

Influence of contralateral radiographs on accuracy of anatomic reduction in surgically treated calcaneus fractures.

BACKGROUND: The purpose of this study was to determine whether obtaining a radiograph of the uninjured contralateral calcaneus (template) before surgery allowed for more precise restoration of Bohler's angle and calcaneal length when performing open reduction internal fixation (ORIF) of intra-articular calcaneus fractures. METHODS: Patients who sustained an intra-articular calcaneus fracture requiring surgery who met inclusion criteria were prospectively randomized into those who had a preoperative template obtained for review at the time of ORIF (group 1) versus those who had the contralater

The pattern and incidence of nasolacrimal injury in naso-orbital-ethmoid fractures: the role of delayed assessment and dacryocystorhinostomy.

A detailed review of forty-six patients with severe naso-orbital-ethmoid injury confirms that naso-lacrimal system injury is less common than originally suspected. Post-operative epiphora is more frequently due to eyelid malposition than naso-lacrimal obstruction. Eight patients (17.4%) required eventual dacryocystorhinostomy. Three out of five patients (60%), treated with closed reduction and external splint fixation, needed dacryocystorhinostomy. This treatment predisposes to external compression of the naso-lacrimal system by malpositioned bone fragments and segments. Open reduction and int

Fractures of the capitellum.

Forty intact cadaver elbows were studied to determine the contribution of the capitellum to elbow stability. With the elbow at 10 degrees of flexion, valgus motion of the elbow after capitellum excision demonstrated a minimal increase. Although some increase in valgus motion did occur after capitellum excision and radial head resection it was not until the ulnar collateral ligament was released that a severe valgus deformity was produced. In addition, isolated capitellum excisions occurring with release of the medial collateral ligament produced severe valgus motion, demonstrating the importan

Posterior tibial artery pseudoaneurysm after titanium elastic nailing for tibial shaft fracture in a 7-year-old boy.

BACKGROUND: Flexible intramedullary nailing is a minimally invasive method used for pediatric tibial shaft fractures stabilization. CASE PRESENTATION: A seven-year-old boy sustained a midshaft oblique tibial fracture due to a fall from one meter height and was treated by closed reduction and intramedullary nailing. On the first postoperative day, a swelling of the proximal half of the lower leg occurred and progressed to compartment syndrome. The lower leg was edematous, firm and painful on palpation, and the pain at rest was out of proportion to injury. CT angiography showed a posterior tibi

Pseudoaneurysm of the subclavian artery as a delayed complication of a clavicle fracture.

Vascular complications are relatively rare following surgical fixation of midshaft clavicle fractures. Here, we report a case of a woman in her 30s presenting 10 years after right clavicular open reduction and internal fixation with revision 6 years prior with sudden and rapidly progressive neck swelling. Physical examination revealed a soft pulsating mass in her right supraclavicular fossa. Ultrasound and CT angiography of the head and neck showed a pseudoaneurysm of her right subclavian artery with a surrounding haematoma. She was admitted to the vascular surgery team for endovascular repair

Early Neuropathic Treatment May Prevent the Chronic Stage of Complex Regional Pain Syndrome Type II (CRPS II).

Complex regional pain syndrome (CRPS) is a debilitating condition characterized by autonomic and inflammatory features, often caused by fractures, surgeries, or other injuries. Multimodal treatment is utilized, which often includes neuropathic medications and physical therapy. We had a case of a 24-year-old man who was diagnosed with CRPS II following an open reduction and internal fixation of a trimalleolar fracture. Significant improvement of edema, pain, and function was achieved with early initiation of gabapentin, nortriptyline, and physical therapy. In this case report, we discuss the th

Surgical Approaches for Managing Femoral Head Fractures (FHFs); What and How to Choose from the Different Options?

Femoral head fractures (FHFs) are considered a relatively uncommon injury; however, they carry a challenge to the trauma surgeon. Choosing the appropriate management option, either conservatively or surgically, if the latter was chosen, should it be open reduction and internal fixation or simple excision of the fragment, or acute total hip arthroplasty. Furthermore, selecting the best surgical approach through which surgical management could be performed depends on the fracture classification, the familiarity of the surgeon, and the presence of associated injuries. All approaches to the hip, i

Intraoperative stereotactic navigation for reconstruction in zygomatic-orbital trauma.

Reconstruction of the maxillofacial skeleton after traumatic injury is not always straightforward because of its three dimensional complex anatomy and aesthetic significance. In case of zygomatic-orbital injury induced by trauma resulting in enophthalmos, the patient is affected on both function and cosmesis. To prevent diplopia or conspicuous asymmetry, exact correction of the orbital symmetry is required. The aim of this case report is to demonstrate the use of an intraoperative image guided navigation system for identification of adequate reconstruction of the orbital floor and orbital symm

Submental intubation: An alternative to tracheostomy when nasoendotracheal intubation is unsuccessful - A case report.

Submental intubation (SI) has been proposed as an alternative to nasoendotracheal intubation when oral endotracheal intubation is contraindicated. In patients who require intubation for maxillofacial reconstruction, this is an alternative to a traditional tracheostomy. The present case report presents an 18-year-old woman who suffered a comminuted mandibular fracture. Two days after her accident, she was taken to the operating room for open reduction with internal fixation of her mandible; however, the anesthesia staff was unable to nasally intubate the patient. A SI was performed. The procedu

Comparison of the efficacy of hook plate versus tension band wire in the treatment of unstable fractures of the distal clavicle.

Fifty-two patients with unstable fractures of distal clavicle treated by open reduction and internal fixation with hook plates or tension band wires were retrospectively reviewed. The 52 patients were divided into two groups based on the method of treatment. The hook plate (HP) group included 32 patients and the tension band wire (TBW) group included 20 patients. Both groups were similar in respect to injury mechanisms, compounding medical conditions, and shoulder score (p > 0.1). However, hook plating had a significantly lower rate of complication (p = 0.01) and symptomatic hardware (p = 0.00

Delayed appearance of diplopia due to orbital emphysema after repair of orbital fractures.

A 38-year-old caucasian male, after continuous nose blowing caused by common flu, developed left diplopia, especially in downward gaze, and complained of orbital dull pain during the preceding 48 hours. Clinical signs included left lower eyelid crepitant edema, mild exophthalmos and restriction of the eye movement. The patient had been operated on for orbital fractures 12 days previously. Open reduction had been performed, stabilization had been achieved with osteosynthesis miniplates, and the orbital floor defect was reconstructed with a piece of Liodura. Orbital computerized tomographic scan

[Bicondylar fractures].

Late results after operative treatment of 59 bicondylar fractures were excellent in 38% and fair or poor in 35%. Classic Y- and T-types are rare (30.5%), while comminution and soft tissue lesions (50.7%) are frequent and worsen the prognosis. In about three-fourths of fractures functional rehabilitation is possible after internal fixation according to AO principles. With advanced age and poor bone stock early functional nonoperative treatment is advisable, even though nonunion must be expected to follow in such patients. Closed pinning may be a reasonable alternative in similar situations, and

Lisfranc complex injuries management and treatment: current knowledge.

Lisfranc complex injuries are a spectrum of midfoot and tarsometatarsal (TMT) joint trauma, more frequent in men and in the third decade of life. Depending on the severity of the trauma can range from purely ligamentous injuries, in low-energy trauma, to bone fracture-dislocations in high-energy trauma. A quick and careful diagnosis is crucial to optimize management and treatment, reducing complications and improving functional outcomes in the middle and long-term. Up to 20% of Lisfranc fractures are unnoticed or diagnosed late, above all low-energy trauma, mistaken for simple midfoot sprains.

Percutaneous Reduction and Screw Fixation of Displaced Intra-articular Fractures of the Calcaneus.

BACKGROUND: Extensile open approaches to reduce and fix intra-articular calcaneal fractures are associated with high levels of wound complications. To avoid these complications, a technique of percutaneous reduction and fixation with screws alone was developed. This study assessed the clinical outcomes, radiographs, and postoperative CT scans after operative treatment with this technique. METHODS: 153 consecutive patients with 182 intra-articular calcaneal fractures were reviewed. All patients were assessed for early postoperative complications at 3 months from the injury. The clinical result

Balloon osteoplasty--a new technique for minimally invasive reduction and stabilisation of Hill-Sachs lesions of the humeral head: a cadaver study.

PURPOSE: Traumatic shoulder dislocation may be complicated by concomitant bony injury of the glenoid rim or the humeral head. In Hill-Sachs lesions, reconstruction techniques vary widely and range from open reduction to tendon transposition or humeral head derotation. These operations are extensive and have questionable outcomes. With the expertise from vertebral compression fracture reduction by kyphoplasty, we examined in a cadaver feasibility study whether reduction of the Hill-Sachs lesion via hydraulic lift might be an anatomical and minimally invasive treatment option. We postulated that

Functionality in patients over 60years of age with distal radius fracture treated with and without surgery: scoping review.

BACKGROUND: Distal radius fracture is the most common in women over 50years, accounting for up to 18% of all fractures in the elderly. The objective of this study is to compare the functional results of patients over 60years with distal radius fracture after conservative treatment versus surgical treatment. METHODS: A review of the literature of articles published between 2015 and 2022 related to the PICO question posed «Functionality in patients over 60years of age with distal radius fracture with conservative treatment compared with surgical treatment». The eligibility criteria were: popula

Effect of Physiotherapy on a Rare Case of Malunion of Femur and Patellar Fracture in a 43-Year-Old Male: A Case Report.

The hip is a ball-and-socket joint surrounded by strong and well-balanced muscles that allow for a wide range of motion in many physical planes. Iliofemoral, ischiofemoral, and pubofemoral are the three major ligaments of the hip joint that provide stability to the joint. Supracondylar femoral fractures are common in old age and can be caused in young people due to accidents or traumatic causes. These types of fractures are complicated to fix surgically due to different architectural designs. If not treated appropriately, these can cause malunion or non-union of the joint. The knee joint is a
