Decreased CSF histamine in narcolepsy with and without low CSF hypocretin-1 in comparison to healthy controls.

STUDY OBJECTIVE: To examine whether cerebrospinal fluid (CSF) histamine contents are altered in human narcolepsy and whether these alterations are specific to hypocretin deficiency, as defined by low CSF hypocretin-1. METHODS: Patients meeting the ICSD-2 criteria for narcolepsy with and without cataplexy and who had CSF hypocretin-1 results available were selected from the Stanford Narcolepsy Database on the basis of CSF availability and adequate age and sex matching across 3 groups: narcolepsy with low CSF hypocretin-1 (n=34, 100% with cataplexy), narcolepsy without low CSF hypocretin-1 (n=2

Reversal of sleep-disordered breathing with opioid withdrawal.

Obstructive sleep apnea, central sleep apnea, sleep related hypoventilation, Biot's or ataxic breathing, and cluster breathing are some of the commonly described sleep disorders in patients who are on long-term opioids. Continuous positive airway pressure that is commonly used to treat obstructive sleep apnea may not be effective in treating sleep-disordered breathing in long-term opioid users, and an adaptive servoventilator (ASV) may be needed. We present a 30-year-old woman with excessive daytime sleepiness and sleep-disordered breathing for the past 4 years. Medical history was complicated

Polygraphy of sleep at altitudes between 5300 m and 7500 m during an expedition to Mt. Everest (MedEx 2006).

OBJECTIVES: Sleep at extreme altitudes is characterized by the repetitive occurrence of central apneas that in some cases may lead to a marked decrease in arterial oxygen saturation. During the Ludwig Maximilians University Expedition to Mt. Everest (MedEx 2006), nocturnal polygraphic recordings were made at different altitudes and included the first recordings ever made at 7500 m, which were completed on 8 separate occasions during the expedition. METHODS: The study was performed on the author (K.M., 58 years, 181 cm, 75 kg), who is an experienced high-altitude mountaineer. The standard poly

CPAP inhibits non-nutritive swallowing through stimulation of bronchopulmonary receptors.

While swallowing and respiratory problems are among the most frequent disorders encountered in neonates, the interrelationships between both functions are not completely understood. This is especially true for non-nutritive swallowing (NNS), which fulfills the important function of clearing upper airways from both local secretions and liquids refluxed from the stomach. Recently, we showed that nasal CPAP inhibits NNS during quiet sleep in the newborn lamb (Samson, St-Hilaire, Nsegbe, Reix, Moreau-Bussière and Praud 2005). The present study was aimed at testing the hypothesis that NNS inhibitio

Challenging sleep homeostasis in narcolepsy-cataplexy: implications for non-REM and REM sleep regulation.

STUDY OBJECTIVES: We recently proposed insufficient non-rapid eye movement sleep (NREMS) intensity to contribute to disturbed nocturnal sleep in patients with narcolepsy-cataplexy (NC). To test this hypothesis, we investigated the effect of physiologically intensified NREMS in recovery sleep following sleep deprivation. DESIGN: Nocturnal baseline and recovery sleep architecture, and the sleep electroencephalogram (EEG) before and after 40 hours of sustained wakefulness were compared between 6 drug-free patients with NC (age range: 19-37 years) and 6 individually matched, healthy control subje

Aspects of parasomnias in childhood and adolescence.

The many parasomnias now officially described often occur in young patients. They are at risk of being confused with each other if their characteristic features are not well known and if they are not carefully assessed, mainly clinically. Accurate diagnosis is essential for choice of treatment, which varies considerably for different parasomnias. These points are illustrated mainly by reference to arousal disorders (including sleepwalking and sleep terrors) and the other parasomnias, such as nightmares and sleep-related epilepsies, from which they must be distinguished.

Compensatory rebound of body movements during sleep, after asphyxia in neonatal rats.

PURPOSE: The usefulness of body movements that occur during sleep when assessing perinatal asphyxia and predicting its long-term consequences is contradictory. This study investigated whether neonatal rats manifest these movements in compensatory rebound after asphyxia, and if these alterations play an important role in its pathogenesis. METHODS: Eight neonatal rats (aged 6-48 h) were implanted with small EMG and EKG electrodes and sleep movements were recorded over a 30-minute control period. Recordings were continued during asphyxia caused by the enclosure of the animal in a polyvinyl sheet

Surges of arterial pressure during REM sleep in spontaneously hypertensive rats.

STUDY OBJECTIVES: Rapid-eye-movement sleep (REM sleep) physiologically entails arterial pressure surges. Pressure surges may lead to acute cardiovascular events in risk conditions such as arterial hypertension. We investigated whether arterial hypertension alters the rate of occurrence and the characteristics of the pressure surges during REM sleep. DESIGN: Spontaneously hypertensive rats (SHR) were compared with Wistar-Kyoto normotensive controls (WKY) and a group of SHR, in which hypertension was prevented by long-term enalapril treatment (ena-SHR). SETTING: N/A. SUBJECTS: Seven male rats

Abnormal sleep-wake cycles in patients with tuberculous meningitis: a case-control study.

BACKGROUND: Patients with tuberculous meningitis (TBM) have been frequently observed to have excessive sleep during the day and frequent awa … on of sleep-wake circadian cycles. This needs to be further characterized through studies involving polysomnography. There is a need to address these specific sleep difficulties to improve the quality of life of the patient as well as the care-giver.

Melatonin excretion levels and polysomnographic sleep parameters in healthy subjects and patients with sleep-related disturbances.

BACKGROUND: The hormone melatonin plays a key role in the proper functioning of the circadian timing system (CTS). Exogenous melatonin has been shown to be beneficial in cases of CTS dysfunction and sleep disturbances. The aim of our study was to relate 24-h melatonin excretion to objective sleep measures. METHODS: A total of 67 individuals were included in the study: 29 healthy subjects (16 women, 13 men; mean age 62.4 y, range 24-86) and 38 outpatients with neuropsychiatric sleep-related disturbances (25 women, 13 men; mean age 46.5 y, range 21-69). Over two consecutive nights in the sleep

Exhaled breath markers in patients with obstructive sleep apnoea.

The objectives of the present study were to assess the level of exhaled breath markers indicating airway inflammation and oxidative stress in patients with obstructive sleep apnoea syndrome (OSAS) in comparison with non-apnoeic (obese and non-obese) subjects and investigate whether therapy with continuous positive airway pressure (CPAP) can modify them. The design was a retrospective observational study, set in Evgeneidio Hospital. Twenty-six OSAS patients and nine obese and 10 non-obese non-apnoeic subjects participated in this study. We measured nasal nitric oxide (nNO), exhaled nitric oxide

Relationship of periodic leg movements and severity of restless legs syndrome: a study in unmedicated and medicated patients.

OBJECTIVE: To evaluate the relationship of the severity of restless legs syndrome (RLS) as assessed by a subjective, patient-rated scale (International RLS Study Group Rating Scale, IRLS), and of periodic leg movements in sleep (PLMS) as an objective parameter, in two different patient populations. METHODS: Data of 200 unmedicated patients with idiopathic RLS were evaluated. Group 1 (n=100) consisted of selected patients participating in the Pergolide European Australian RLS (PEARLS) study. Group 2 (n=100) represented an outpatient RLS population investigated in a Sleep Disorders Center. Addi

Insufficient non-REM sleep intensity in narcolepsy-cataplexy.

STUDY OBJECTIVES: To compare electroencephalogram (EEG) dynamics during nocturnal sleep in patients with narcolepsy-cataplexy and healthy controls. Fragmented nocturnal sleep is a prominent feature and contributes to excessive daytime sleepiness in narcolepsy-cataplexy. Only 3 studies have addressed changes in homeostatic sleep regulation as a possible mechanism underlying nocturnal sleep fragmentation in narcolepsy-cataplexy. DESIGN, SETTING AND PARTICIPANTS: Baseline sleep of 11 drug-naive patients with narcolepsy-cataplexy (19-37 years) and 11 matched controls (18-41 years) was polysomnogr

The visual scoring of sleep and arousal in infants and children.

Age is probably the single most crucial factor determining how humans sleep. Age and level of vigilance significantly influence the electroencephalogram (EEG) and the polysomnogram (PSG). The Pediatric Task Force provide an evidence-based review of the age-related development of the polysomnographic features of sleep in neonates, infants, and children, assessing the reliability and validity of these features, and assessing alternative methods of measurement. We used this annotated supporting text to develop rules for scoring sleep and arousals in infants and children. A pediatric EEG or PSG ca
