Treatment Discontinuation and Barriers and Facilitators to Retention Among Individuals Initiating Buprenorphine for Opioid Use Disorder: A Longitudinal Cohort Study.

OBJECTIVES: Early discontinuation of medication for opioid use disorder (MOUD) is associated with increased mortality and hinders remission. Combining data from electronic health records (EHRs) and patient questionnaires, we examined buprenorphine discontinuation and barriers and facilitators to retention in outpatient settings. METHODS: Adults initiating buprenorphine in a Pennsylvania outpatient program (2021-2023) completed 2 questionnaires: at medication initiation (baseline) and 6 months after initiation (follow-up). Among 1189 eligible patients, we evaluated buprenorphine discontinuatio

Topological Fuzzy Analytic Hierarchy Process (ToF-AHP) for geospatial assessment of malaria vulnerability in Nigeria.

Malaria remains a persistent public health challenge in Nigeria, where structural inequalities in socioeconomic, gender, and health service factors amplify transmission risk. This study develops a Topological Fuzzy Analytic Hierarchy Process (ToF-AHP) framework to assess malaria vulnerability from social and spatial perspectives. The approach integrates fuzzy pairwise comparisons with topological data analysis to capture higher-order structural patterns among vulnerability determinants. Five indicators, ITN usage, maternal education, socioeconomic status, antenatal care (ANC) non-attendance, a

Burden of disease and barriers to comprehensive care for rheumatic heart disease in South Africa: an updated systematic review protocol.

INTRODUCTION: Rheumatic heart disease (RHD) is responsible for a significant burden of cardiovascular morbidity and mortality, and remains the most common cause of acquired heart disease among children and young adults in low-income and middle-income countries. Additionally, the global COVID-19 pandemic has forced the emergency restructuring of many health systems, which has had a broad impact on health in general, including cardiovascular disease. Despite significant cost to the health system and estimates from 2015 indicating both high incidence and prevalence of RHD in South Africa, no cohe

Comparative Effectiveness of a Web-Based Patient Decision Aid for Therapeutic Options for Sickle Cell Disease: Randomized Controlled Trial.

BACKGROUND: Hydroxyurea, chronic blood transfusions, and bone marrow transplantation are efficacious, disease-modifying therapies for sickle cell disease but involve complex risk-benefit trade-offs and decisional dilemma compounded by the lack of comparative studies. A patient decision aid can inform patients about their treatment options, the associated risks and benefits, help them clarify their values, and allow them to participate in medical decision making. OBJECTIVE: The objective of this study was to develop a literacy-sensitive Web-based patient decision aid based on the Ottawa decisi

Design and Evaluation of a Source-Grounded Medical LLM for Clinical Decision Support and Patient Care in Trustworthy Diagnostic Systems.

Background/Objectives: Large language models (LLMs) are increasingly explored for clinical decision support and digital health applications. However, reliable diagnostic assistance remains challenging for low-resource medical languages such as Turkish due to limited source grounding, transparency, clinical safety, and localized medical knowledge. This study presents TurkishMedLLM, a source-grounded and safety-aware Turkish medical LLM designed for clinician-supervised diagnostic decision support, symptom interpretation, and patient care. Methods: The methodology integrates multi-source Turkish

ALternatives To prophylactic Antibiotics for the treatment of Recurrent urinary tract infection in women (ALTAR): study protocol for a multicentre, pragmatic, patient-randomised, non-inferiority trial.

BACKGROUND: At least half of all adult women will experience infective cystitis (urinary tract infection: UTI) at least once in their life and many suffer from repeated episodes. Recurrent urinary tract infection (rUTI) in adult women is usually treated with long-term, low-dose antibiotics and current national and international guidelines recommend this as the 'gold standard' preventative treatment. Although they are reasonably effective, long-term antibiotics can result in bacteria becoming resistant not only to the prescribed antibiotic but to other antimicrobial agents. The problem of antim

Effect of exercise versus cognitive behavioural therapy or no intervention on anxiety, depression, fitness and quality of life in adults with previous methamphetamine dependency: a systematic review.

BACKGROUND: Methamphetamine (MA) is a highly addictive psychostimulant used by approximately 52 million people worldwide. Chronic MA abuse leads to detrimental physiological and neurological changes, as well as increases in anxiety and depression, and decreases in overall fitness and quality of life. Exercise has been reported to possibly reverse physiological and neurological damage caused by previous MA use, and to reduce anxiety and depression in this population. The aim of this systematic review was to identify, clinically appraise and synthesise the available evidence for the effectivenes

Leveraging implementation science theories to develop and expand the use of a penicillin allergy de-labeling intervention.

BACKGROUND: Penicillin allergy is the most frequently reported drug allergy, yet most patients can tolerate the drug if challenged. Despite this discrepancy, large scale penicillin allergy de-labeling interventions have not been widely implemented in many health care systems. The application of a multi-method implementation science approach can provide key tools to study this evidence to practice gap and provide insight to successfully operationalize penicillin allergy evaluation in real-world clinical settings. METHODS: We followed a four-step process that leverages qualitative analysis to d
