Evaluating MRI response criteria in microsatellite instability-high rectal cancer treated with immune checkpoint inhibitors.

BACKGROUND: Immune checkpoint inhibitors (ICIs), particularly anti-programmed cell death protein 1 (PD-1) agents, have enabled consideration of non-operative management in patients with microsatellite instability-high (MSI-H) or mismatch repair-deficient (dMMR) rectal cancer. Standard magnetic resonance imaging (MRI) response criteria-developed for chemoradiotherapy-may not accurately reflect treatment effects with ICIs, however. PATIENTS AND METHODS: We retrospectively analyzed 14 patients with locally advanced MSI-H/dMMR rectal adenocarcinoma treated with neoadjuvant anti-PD-1 monotherapy.

TUMOR RESPONSE TO NEOADJUVANT LONG-COURSE CHEMORADIOTHERAPY DEPENDS ON THE RECTAL CANCER PATIENT GENDER.

Our study aimed to analyze the correlation of cancer-related parameters with tumor regression grade (TRG) and disease-free survival (DFS) by gender in 192 rectal cancer patients operated on after neoadjuvant long-course chemoradiotherapy (LCCRT). Preoperative diagnostics revealed no significant gender differences in any clinical parameters other than obesity (p=0.031). We found that slightly overweight men had a lower incidence of distant metastases (p=0.042). The post-LCCRT pathologic finding showed that women had more positive lymph nodes (ypN, p=0.002) while no other pathologic parameter di

Dosimetric impact of variable air cavity within PTV for rectum cancer.

PURPOSE: The aim of this study is to determine the impact of rectal air volume changes on treatment plan quality, and subsequently inform daily cone-beam computed tomography (CBCT) evaluation constraints, in terms of acceptable rectal air volume during treatment. METHODS: Twelve rectal cancer patients who exhibited rectal air within the PTV on their planning CT were selected. A study was conducted to evaluate the deterioration in plan quality due to expanding air volume. For each case, the air cavity volume was isotropically expanded in three dimensions using predefined margins of 3, 5, 7, an

A phase III randomised trial on the addition of a contact X-ray brachytherapy boost to standard neoadjuvant chemo-radiotherapy for organ preservation in early rectal adenocarcinoma: 5 year results of the OPERA trial.

BACKGROUND: The OPERA trial has shown that a contact X-ray brachytherapy 50 kV (CXB) boost with neoadjuvant chemoradiotherapy (NCRT) can increase organ preservation (OP) rate for early rectal adenocarcinoma (ADK) of low-mid rectum. We report the results after 5 years of follow-up. PATIENTS AND METHODS: OPERA was a multicentre, phase III trial that included operable patients (pts), with cT2-cT3b low-mid rectal ADK, tumours <5 cm, cN0 or cN1 <8 mm. All pts received external beam radiotherapy (EBRT): 45 Gy in 25 fractions with concurrent capecitabine. Pts were randomly assigned (1:1) to receive

Urinary and sexual dysfunction after rectal cancer surgery: A surgical challenge.

This manuscript focused on the surgical challenge of urinary and sexual dysfunction after rectal cancer surgery based on the interesting results demonstrated by the observational study of Chen et al, which was published in the World Journal of Gastrointestinal Surgery. Urinary dysfunction occurs in one-third of patients treated for rectal cancer. Surgical nerve damage is the main cause of urinary dysfunction. Radiotherapy seems to exacerbate sexual dysfunction. The role of Denonvilliers' fascia preservation vs resection when performing total mesorectal excision (TME), the impact of robotic and

Müllerian clear cell carcinoma arising from rectal endometriosis with pericolonic lymph node metastasis.

We present a case of clear cell carcinoma arising in endometriosis of the rectum that provides insights to the biological behavior of this extremely rare tumor. The patient presented with a large rectal mass, and the biopsy was suggestive of clear cell carcinoma of Müllerian origin based on the morphology and immunoprofile. The patient underwent a proctosigmoidectomy and hysterectomy with bilateral salpingo-oophorectomy with clear cell carcinoma involving the colon. Five pericolonic lymph nodes were positive for carcinoma. Endometriosis was identified in the colonic wall adjacent to the carcin

Using CT-Based Pelvimetry and Visceral Obesity Measurements to Predict Total Mesorectal Excision Quality for Patients Undergoing Rectal Cancer Surgery.

BACKGROUND: A complete total mesorectal excision is the criterion standard in curative rectal cancer surgery. Ensuring quality is challenging in a narrow pelvis, and obesity amplifies technical difficulties. Pelvimetry is the measurement of pelvic dimensions, but its role in gauging preoperatively the difficulty of proctectomy is largely unexplored. OBJECTIVE: To determine pelvic structural factors associated with incomplete total mesorectal excision after curative proctectomy and build a predictive model for total mesorectal excision quality. DESIGN: Retrospective cohort study. SETTING: A

Machine learning model for prediction of permanent stoma after anterior resection of rectal cancer: A multicenter study.

BACKGROUND: The conversion from a temporary to a permanent stoma (PS) following rectal cancer surgery significantly impacts the quality of life of patients. However, there is currently a lack of practical preoperative tools to predict PS formation. The purpose of this study is to establish a preoperative predictive model for PS using machine learning algorithms to guide clinical practice. METHODS: In this retrospective study, we analyzed clinical data from a total of 655 patients who underwent anterior resection for rectal cancer, with 552 patients from one medical center and 103 from another

Reducing Remifentanil Usage in Laparoscopic Rectal Cancer Surgery for Elderly Patients by Optimizing Dosing.

BACKGROUND: Colorectal cancer is a prevalent and serious health concern globally, particularly among the elderly population. Laparoscopic surgery is a commonly used approach for colorectal cancer treatment. However, the use of appropriate anesthesia and muscle relaxants is essential to ensure optimal surgical outcomes. Elderly patients undergoing surgery often have unique physiological characteristics and comorbidities, such as hypertension, diabetes, and coronary heart disease. These factors can affect treatment efficiency and patient outcomes. OBJECTIVE: This study aimed to investigate the

[Transanal Tumor Resection: Indication, Surgical Technique and Management of Complications].

Transanal resection procedures are special operations for the minimally invasive treatment of rectal tumours. Apart from benign tumours, this procedure is suitable for the excision of low-risk T1 rectal carcinomas, if these can be completely removed (R0 resection). With stringent patient selection, very good oncological results are achieved. Various international trials are currently evaluating whether local resection procedures are oncologically sufficient if there is a complete or near complete response after neoadjuvant radio-/chemotherapy. Numerous studies have shown that the functional re

Patient- and physician-reported radiation-induced toxicity of short-course radiotherapy with a prolonged interval to surgery for rectal cancer.

AIM: A prolonged interval (>4 weeks) between short-course radiotherapy (25 Gy in five fractions) (SCRT-delay) and total mesorectal excision for rectal cancer has been associated with a decreased postoperative complication rate and offers the possibility of organ preservation in the case of a complete tumour response. This prospective cohort study systematically evaluated patient-reported bowel dysfunction and physician-reported radiation-induced toxicity for 8 weeks following SCRT-delay. METHOD: Patients who were referred for SCRT-delay for intermediate risk, oligometastatic or locally advanc
