Clinical impact of patients with ruptured esophageal varices with death outcome.

BACKGROUND/AIMS: High hemostasis rates for ruptured esophageal varices have been acquired, but many patients die of liver failure afterwards. The aim of the current study is to find clinical information about the patients with ruptured esophageal varices who died during an early period after acquiring a hemostasis. METHODOLOGY: Forty-five patients with ruptured esophageal varices, including 17 who died during two months after acquiring a hemostasis by an initial endoscopic therapy, were investigated in the current study. The relationship between the clinical data and the prognosis of the pati…

Endovascular repair of a ruptured chronic type B aortic dissection.

Aneurysm formation is a common sequel of chronic type B aortic dissection. Ruptured false lumen aneurysms have traditionally been treated with open repair. These procedures are associated with high morbidity and mortality rates. We report the first successful endovascular repair of a ruptured chronic type B aortic dissection in a patient who had been turned down for elective surgery. The endovascular management of chronic dissection with rupture is difficult and may necessitate stenting of both entry and reentry points to induce false channel thrombosis. The long-term efficacy of this techniqu…

Ossification of a ruptured achilles tendon: a case report in a diabetic patient.

Ossification of the Achilles tendon is a rare condition to be distinguished from the more frequently occurring tendon calcification. Achilles tendon ossification is more common in males and is usually associated with prior surgery or trauma to the tendon. A case history of a ruptured ossified Achilles tendon in an elderly diabetic female is presented. Although Achilles tendon rupture with subsequent ossification and fracture of the ossified Achilles tendon have been reported individually, there is evidence to suggest that this patient may have both ruptured and fractured an ossified Achilles t…

Rupture of flexor tendons after arthrodesis of the basal joint of the thumb.

We report a case of rupture of the flexor tendons to the index finger after arthrodesis of the basal joint of the thumb. The tendons ruptured as a result of the Kirschner wires having penetrated in the carpal tunnel. This unusual complication was treated by tendon graft of the palmaris longus tendon.

Behçet's disease: endovascular management of a ruptured peripheral arterial aneurysm.

Traditionally, bypass grafts are at a high risk for thrombosis or anastomotic degeneration in patients with Behçet's disease. We report the successful deployment of a vein-covered stent across the neck of a ruptured peripheral arterial aneurysm, via a remote site access, with intermediate-term follow-up. Covered stents may represent an attractive alternative to open surgical bypass for the management of aneurysms in patients with Behçet's disease.

Six cases of ruptured aneurysm, treated with cellulose acetate polymer.

We review 6 cases of ruptured aneurysm which were elderly, poor-grade SAH or requiring high-risk surgery treated with Cellulose Acetate Polymer (CAP). A 76-year-old female with A-com aneurysm was comatose. Heart failure was her complication (case 1). An 81-year-old female with A-com aneurysm was semicomatose (case 2). An 89-year-old female with MCA aneurysm was semicomatose and had right hemiplegia (case 3). An 88-year-old female with MCA aneurysm was comatose (case 4). A 55-year-old male with IC-PC aneurysm had a small intracerebral hematoma and was semicomatose (case 5). A 52-year-old female…

[Clinical analysis of 68 patients with ruptured sinus of Valsalva aneurysm].

OBJECTIVE: To evaluate the clinical characteristics, diagnosis, treatment and prognosis of ruptured aneurysm of the sinus of Valsalva (ASV). METHODS: Sixty-eight cases of ruptured ASV treated in our hospital from 1986 to 1998 were retrospectively analyzed. RESULTS: The ratio of male to female was 1.8:1 in 68 patients (44 males, 24 females). The mean age was (29.5 +/- 10.7) years (8-60 years). The proportion of ruptured aneurysms originating from the right coronary and noncoronary sinus was 79.4% and 20.6% respectively. Sixty-seven point six percent of ASV ruptured into right ventricle and 32…

Placenta percreta with spontaneous rupture of an unscarred uterus in the second trimester.

BACKGROUND: Rupture of a pregnant uterus occurs most often in a scarred uterus, and spontaneous rupture of a non-scarred uterus in the early second trimester is rare. CASE: A woman with two previous normal vaginal deliveries and no prior trauma to the uterus presented at 16 weeks' gestation with an acute abdomen due to intraperitoneal hemorrhage. A large rupture of the fundus of the uterus was found. A supracervical hysterectomy was carried out, with subsequent good recovery. The specimen showed placenta percreta. CONCLUSION: Spontaneous rupture of an unscarred uterus, due to placenta percre…

Indirect MR arthrography of the shoulder in detection of rotator cuff ruptures.

The aim of this study was to compare the efficacy of indirect MR arthrography images obtained following intravenous contrast injection and conventional MR imaging in the diagnosis of rotator cuff tears. Twenty-four patients with clinically suspected rotator cuff disease were examined. Conventional MR images and post-contrast indirect MR arthrography images were obtained. All images were evaluated in a blinded fashion by two musculoskeletal radiologist. Results were than analyzed depending on surgical output. The correlation coefficient (Spearman rank correlation test) and the kappa values for

Risk factors for third degree perineal ruptures during delivery.

OBJECTIVE: To determine risk factors for the occurrence of third degree perineal tears during vaginal delivery. DESIGN: A population-based observational study. POPULATION: All 284,783 vaginal deliveries in 1994 and 1995 recorded in the Dutch National Obstetric Database were included in the study. METHODS: Third degree perineal rupture was defined as any rupture involving the anal sphincter muscles. Logistic regression analysis was used to assess risk factors. MAIN OUTCOME MEASURES: An overall rate of third degree perineal ruptures of 1.94% was found. High fetal birthweight, long duration o…

Ruptured internal iliac artery aneurysm mimicking a hip fracture.

Internal iliac artery aneurysms are difficult to detect due to their wide range of clinical presentations. The mortality rate from ruptured internal aortic aneurysms is particularly high in part because of delayed diagnosis. This report reviews the management of a patient with a ruptured internal iliac artery aneurysm which presented mimicking a hip fracture.

Rupture of the rectosigmoid colon with evisceration of the small bowel through the anus.

Spontaneous rupture of the rectosigmoid colon and herniation of the small intestine through the rupture site and eventual evisceration through the anus is a very rare event. In the literature, only 42 cases have been reported. The majority of them occurred in patients with rectal prolapse and one case was reported in association with a third-degree uterine prolapse. We experienced an 81-year-old female patient with rectal prolapse and second-degree uterine prolapse complicated by spontaneous perforation of the rectosigmoid colon and anal evisceration of the small intestine. Segmental resection…
