Mixed-method evaluation of a culturally-adapted basic palliative care curriculum for practicing physicians in mainland China.

BACKGROUND: China's aging population will escalate palliative care (PC) needs in the next decade. Scalable and culturally-adapted training is necessary to equip practicing clinicians with essential PC skills. The objective of this study is to evaluate a culturally-adapted basic PC training course in mainland China. METHODS: A total of 29 practicing physicians from Zhejiang Province, China, were selected to participate in an in-person training program that spanned six days. We analyzed pre- and post-course quantitative surveys on knowledge, self-efficacy, and behavior using descriptive statist

Internalized Disability Stigma Among People Living With Chronic Traumatic Brain Injury.

OBJECTIVE: Qualitative evidence suggests that disability stigma is an important concept in the lived experiences of individuals with traumatic brain injury (TBI). However, there have been no studies in this population specifically evaluating internalized disability stigma. We sought to characterize variability in levels of internalized stigma and examine the applicability of the "why try" effect (reduced self-efficacy and life goal attainment as consequences of internalized stigma) to people living with chronic TBI. SETTING: Outpatient research laboratory. PARTICIPANTS: Eighty-four adults wi

Validation of self-reported male circumcision status and genital ulcer disease among Ugandan men.

OBJECTIVE: Voluntary medical male circumcision (MC) is a critical tool in combination HIV prevention programmes in Africa. Self-reported MC (SrMC) status is used in HIV epidemiological surveys to assess MC coverage but is subject to response bias with limited validation. This study evaluated the utility of SrMC status as a marker of MC as well as self-reported genital lesions for genital ulcer disease (GUD) among Ugandan men. METHODS: Male participants aged 18-49 years in the cross-sectional Sexually Transmitted Infection Prevalence study, conducted between May and October 2019, responded to

Familial implications of autoimmune disease: Recurrence risks of alopecia areata and associated conditions in first-degree relatives.

Alopecia areata (AA), a complex autoimmune hair loss condition, affects approximately 2.1% of the population. Individuals with AA have increased susceptibility to diseases such as atopy and autoimmune disorders, but little is known about first-degree relatives' risk to develop AA and associated conditions. Genetic counseling for multifactorial conditions, including autoimmune disease is complex, but potentially valuable. Anecdotally we know patients with AA ask medical providers about recurrence risk for family members as well as question whether they and their relatives are at risk for other

Communication changes with laryngectomy and impact on quality of life: a review.

PURPOSE: Patients with throat cancer at later stages often undergo total laryngectomy, a procedure that removes the larynx (voice box) and directly impacts the patient's ability to produce natural voice and communication. This narrative review aimed to explore how changes to communication following laryngectomy may impact quality of life (QoL) for patients. METHODS: Literature searches were conducted using CINAHL, MEDLINE and PsychInfo databases for studies published between 2007 and 2018. The search terms (and derivatives) of laryngectomy AND communication AND quality of life were used. A sy

Accelerated active case detection of visceral leishmaniasis patients in endemic villages of Bangladesh.

BACKGROUND: The visceral leishmaniasis (VL) elimination program in Bangladesh is in its attack phase. The primary goal of this phase is to decrease the burden of VL as much as possible. Active case detection (ACD) by the fever camp method and an approach using past VL cases in the last 6-12 months have been found useful for detection of VL patients in the community. We aimed to explore the yield of Accelerated Active Case Detection (AACD) of non-self reporting VL as well as the factors that are associated with non-self reporting to hospitals in endemic communities of Bangladesh. METHODS: Our

Assessment of oral self-care in patients with periodontitis: a pilot study in a dental school clinic in Japan.

BACKGROUND: Oral hygiene education is central to every stage of periodontal treatment. Successful management of periodontal disease depends on the patient's capacity for oral self-care. In the present study, the oral self-care and perceptions of patients attending a dental school clinic in Japan were assessed using a short questionnaire referring to existing oral health models. METHODS: A cross-sectional study design was used. The study population consisted of sixty-five patients (age range 23-77) with chronic periodontitis. The pre-tested 19-item questionnaire comprised 3 domains; 1) oral hy

The self-efficacy model of medication adherence in chronic mental illness.

AIM: In this position paper, the self-efficacy model of medication adherence in chronic mental illness is presented, and its application to antipsychotic medication adherence is considered. BACKGROUND: Poor adherence to antipsychotic medications is common in chronic mental illness. Major implications of this are relapse and re-hospitalisation. Several conceptual frameworks have been developed about adherence and, in some instances, have been incorporated in medication taking studies, but have resulted in inconsistent outcomes. METHOD: This paper draws on a review of literature from databases

The neuropeptide-Y Y5 receptor antagonist L-152,804 decreases alcohol self-administration in inbred alcohol-preferring (iP) rats.

Neuropeptide-Y (NPY) is the most abundant and widely distributed peptide in the mammalian central nervous system and increases feeding behavior through actions at the Y5 receptor subtype. Recent pharmacological evidence indicates that NPY activity at this receptor subtype can modulate ethanol reinforcement. The purpose of this study was to determine if NPY Y5 receptor antagonism reduces ethanol self-administration and reinforcement in a rodent genetic animal model of alcoholism. Selectively inbred alcohol-preferring (iP) rats were trained to voluntarily consume ethanol (10% vol/vol) versus H2O

Self-Collection for HPV Testing: Potential Issues Related to Performance Measures and Quality Improvement Among Federally Qualified Health Centers.

CONTEXT: Most incident cases of cervical cancer in the United States are attributable to inadequate screening. Federally qualified health centers (FQHCs) serve a large proportion of women who are low-income, have no insurance, and are underserved-risk factors for insufficient cervical cancer screening. FQHCs must maintain quality measures to preserve their accreditation, address financial reimbursements, and provide quality care. Implementation of human papillomavirus (HPV) self-collection can improve cervical cancer screening coverage within FQHCs. OBJECTIVES: To understand perspectives from

Nail disorders in pediatric patients: Part 2. Clinical overview, diagnosis and management of inflammatory, pigmentary, and neoplastic etiologies.

Nail disorders represent a relatively uncommon but diverse group of conditions seen in pediatric patients. Management varies largely by etiology. Some inflammatory nail disorders in children are self-limited, whereas others may lead to permanent nail loss if not recognized and treated early. In addition, certain nail disorders may present differently in children than adults and may suggest increased systemic disease burden. In Part 2 of this two-part continuing medical education series, we review inflammatory, pigmentary, and neoplastic nail disorders in children and propose clinical approach

Successful Empirical Treatment of a Severe Corneal Abscess After Delayed Consultation in a Low-Resource Setting: A Case Report.

Corneal abscesses remain an important cause of preventable monocular blindness, particularly where access to specialized eye care is limited. We report a 34-year-old man from the Democratic Republic of the Congo who presented with a severe corneal abscess after three weeks of symptoms and prior self-medication with unidentified topical eye drops. In the absence of microbiological investigations, intensive empirical medical treatment achieved infection control and partial visual recovery. This case highlights the importance of timely referral and clinically guided management when diagnostic res
