Clay shoveler's fracture is a stable spinous process fracture commonly occurring at the seventh cervical vertebrae. The name originated in Australia during the 1930s, after many reports of similar injuries among clay workers who suffered fractures due to a sudden flexion force on their neck and back muscles, hence the name "clay shoveler's fracture." This fracture may occur as a consequence of a direct blow/trauma, a contraction of the shoulder muscles, and sudden extreme movements of the neck influenced by additional factors presented throughout the article. Clay shoveler's fracture mechanism

We describe an arthroscopic rein-type capsular suture that approximates the triangular fibrocartilage complex to the anatomical footprint, and report the results at a minimum 12 month follow-up. The procedure involves two 3-0 polydioxanone horizontal mattress sutures inserted 1.5 cm proximal to the 6-R and 6-U portals to obtain purchase on the dorsal and anterior radioulnar ligaments, respectively. The two sutures work as a rein to approximate the triangular fibrocartilage complex to the fovea. Ninety patients with Type IB triangular fibrocartilage complex injuries were included retrospectivel

With advancements in arthroscopic techniques and instrumentation, hip arthroscopy has become an increasingly used technique to treat soft-tissue and osseous pathologies about the hip. Patient predisposition to labral and capsular injuries can present as femoroacetabular impingement or hip dysplasia, sometimes in combination. Capsular management continues to be a topic of debate, with capsular repair becoming the standard of care in most cases. Furthermore, in cases of borderline dysplasia and microinstability, considerations for not only capsular repair but with plication has shown significant

Scapulothoracic dissociation is the traumatic separation of the scapula from the thoracic wall. It is the outcome of rare, high-energy trauma to the shoulder girdle and may include vascular and neurologic damage of the upper extremity. Of the few cases presented in the literature, the majority refer to polytrauma patients. We present a rare variant of scapulothoracic dissociation combined with comminuted fractures of both the scapula and the clavicle associated with brachial plexus palsy. There were also vertebral fractures of the spinous processes of C6, T4, T5, and T6, of the transverse proc

Paralympic wheelchair curling is an adapted version of Olympic curling played by individuals with spinal cord injuries, cerebral palsy, multiple sclerosis, and lower extremity amputations. To the best of the authors' knowledge, there has been no experimental or computational research published regarding the biomechanics of wheelchair curling. Accordingly, the objective of the present research was to quantify the angular joint kinematics and dynamics of a Paralympic wheelchair curler throughout the delivery. The angular joint kinematics of the upper extremity were experimentally measured using

We conducted a study to determine the overall incidence and long-term clinical and functional outcomes of patients with malunion after nonoperative management of humeral shaft fractures. Fifteen patients with radiographic malunion (>20° angulation or shortening of :ge;2.5 cm) were identified, and their medical records retrospectively reviewed for information about their injuries and treatment. Long-term outcomes were assessed with a self-reported questionnaire, the DASH (Disabilities of the Arm, Shoulder, and Hand) form, and physical examination. Of 91 study-eligible patients, 15 (16%) had mal

Forequarter amputation is a radical surgical procedure initially described for the treatment of traumatic injuries in 1908. This procedure has been used more recently in the treatment of soft tissue tumors. This report describes the experience in the Division of Surgical Oncology at the University of Illinois over a 20-year period. Between 1970 and 1991, 10 patients underwent forequarter amputations for malignant disease. Nine of these patients had soft tissue tumors and one a malignant melanoma. Four patients underwent amputation as primary treatment of their tumor, and six underwent the proc

Two hundred fifty-six vacuum extractions and 300 randomly chosen forceps deliveries were analyzed retrospectively. Vacuum extraction use increased from 0.3 to 3.1%, while forceps use declined from 10.1 to 4.9% over a five-year period. No differences were found in indications for vacuum extraction and forceps, but the preapplication position differed (occiput posterior or transverse in 81.2% in the vacuum group and 27% in forceps patients). Preapplication station also differed, with 59.8% of vacuum extraction at +1 or higher stations, compared with 9% of forceps. Under these conditions we found

Using a 1982 to 1983 regional network data bank of 33,545 delivered infants, a study was conducted comparing 574 macrosomic infants weighing greater than 4500 g to a control group of 18,739 infants whose birth weights were 2500 to 3499 g. Macrosomic infants occurred in 1.7% of the deliveries. Women delivering macrosomic infants were significantly older, of higher parity, more obese (greater than 90 kg), and more frequently diabetic and postmature (longer than 42 weeks) than the controls. The women having macrosomic infants had a higher frequency of cesarean deliveries. The macrosomic infants w

All 132 participants in a 500 mile, 8 day bicycle tour were surveyed by questionnaire to characterize the demographics and bicycling experience of the riders, and to determine the frequency and severity of nontraumatic injuries they experienced. Riders who developed significant symptoms were interviewed and/or examined. Eighty-six percent of ride participants responded to the survey. The average age of the riders was 41.4 years (+/- 11.7 years). They rode an average of 95.8 miles per week on a routine basis, but the majority were new to long distance touring. Most were healthy, but 5% had seri

Iatrogenic nerve injury in head and neck surgery remains a substantial yet potentially preventable source of morbidity across endocrine, otolaryngologic, and related procedures. This systematic review synthesized evidence to identify the nerves most frequently affected, delineate operative mechanisms of injury, describe the clinical course, and evaluate preventive strategies. Traction on visually intact nerves emerged as the predominant mechanism, with additional contributions from thermal injury, compression, ischemia, and entrapment by suture or clip. The recurrent laryngeal, facial, trigemi

Rotator cuff injury is a disease in which the muscle and tendon that constitute the rotator cuff are torn causing shoulder pain and limited function. Osteoporosis (OP) is a systemic metabolic bone disease characterized by decreased bone mass, destruction of bone microstructure, decreased bone strength, and increased bone fragility. Both are common musculoskeletal diseases that occur in middle-aged and elderly people, and their prevalence gradually increases with age. Clinically, rotator cuff injury and OP comorbidity are very common, especially in terms of bone metabolism. In recent years, pla

Despite medical advances, degloving injury remains one of the most difficult traumatic injuries to treat. The conventional method for treating degloving injury of the hand is reconstruction with a groin flap. However, few reports have described the mid- or long-term functional and aesthetic outcomes after a hand reconstruction with a groin flap. This case report describes a 68-year-old woman with no specific medical history who presented with a severe degloving injury of the right hand, caused by a roller machine. The area of skin loss was covered with a pedicled groin flap that was separated

Posterior sternoclavicular joint (SCJ) dislocation is a rare but potentially life-threatening injury due to its proximity to critical mediastinal structures. Early diagnosis and prompt management are essential to prevent severe complications such as vascular or respiratory compromise. We report a case of a 23-year-old male who presented to our emergency department five days after a high-energy motor vehicle accident with isolated, closed posterior dislocation of the SCJ. The patient complained of left shoulder pain, mild dysphagia, and intermittent tingling in the left upper limb. Imaging reve

INTRODUCTION: No detailed study dealing with an injury to the AC joint in combination with scapular fractures has been published to date. The aim of this study is to describe pathoanatomy of these injuries.
METHODS: In a series of 519 scapular fractures in adult patients from the period of 20022020 we identified a total of 20 (3.9%) cases associated with AC dislocation. The group comprised 17 men and 3 women with the mean age of 49 years (range, 2178). Radiographs of the shoulder joint followed by CT examination, including 3D reconstructions, were performed in all patients. This documentation

INTRODUCTION & IMPORTANCE: Elbow dislocations are rare among the pediatric population as their ligaments are stronger than bones. Anterior dislocations of the elbow are even rarer entity usually caused by anteriorly directed force on the hyper-flexed elbow. We present a case of a rotational force on an extended elbow causing anterior dislocation of the shoulder in an 11-year-old boy.
CASE PRESENTATION: An 11-year-old boy presented to the Accident & Trauma unit with pain & swelling of the right elbow following a twisting injury by a spinning washing machine. The child was in pain.

This review presents the available data on the diagnosis of obstetrical anal sphincter injury by postnatal ultrasound imaging. There is increasing evidence that anal sphincter tears are often missed after childbirth and, even when diagnosed, often suboptimally repaired, with a high rate of residual defects after reconstruction. Even after postpartum diagnosis and primary repair, 25% to 50% of patients will have persistent anal incontinence. As clinical diagnosis may fail in the detection and classification of obstetrical anal sphincter injury, the use of imaging has been proposed to improve th
