A Meta-Analysis of Palatal Repair Timing.

Developing midface hypoplasia is common after palatoplasty and has been hypothesized to be influenced by the timing of hard palate repair. This meta-analysis assesses the risk of developing midface hypoplasia based on age at hard palate repair. A Pubmed PRISMA systematic review and meta-analysis was completed for literature focused on palatoplasty and midface hypoplasia published between 1970 and 2019. Cephalometric data were extracted and categorized by age at hard palate repair: <6, 7 to 12, 13 to 18, 19 to 24, and 25 to 83 months. Analysis of these groups and a control were compared using i

Outcome of penile lengthening of adolescents and young adults born with bladder exstrophy and epispadias.

BACKGROUND: The decreased penile length in patients born with BE results partly from pubic symphysis diastasis and the separation of the corporal bodies, which causes a shortened penis as the corporal length is lost in traversing the distance between the pubic rami. However, in some cases there is an intrinsic penile abnormality and dorsal chordee. Furthermore, multiple surgeries has in some cases, resulted in cutaneous and subcutaneous scarring, which contributed to the problem of the short phallus and dorsal tethering to the abdominal wall (figure). OBJECTIVE: Herein we evaluated the outcom

Stiffness reduction of the rib cage to perform a minimally invasive pectus excavatum repair: biomechanical evaluation.

OBJECTIVES: To study the influence of sternal transection and costal chondrotomies on the stiffness and stresses in the rib cage of adult patients undergoing Nuss pectus excavatum procedure. METHODS: Four pectus excavatum models with different Haller indexes were created by parameterizing a 3D model of a rib cage obtained based on a computed tomography scan of a patient with no pectus deformity. Using the finite element method, insertion of intrathoracic bars into all models was simulated in 3 conditions, namely, non-intervened, transverse sternal section and costal chondrotomies. Stiffness,

Titanium Fan Plate Reconstruction for Lateral Orbital Traumatic Defects.

Fractures of the orbit often require operative repair with materials ranging from autologous tissue to titanium implants. Reconstructive techniques of the lateral orbital rim and wall offer a unique challenge due to the structural and aesthetic demands with regard to its natural contour. The authors present a case in which a patient sustained a ballistic injury involving the lateral orbital rim and wall requiring reconstruction. The authors employed the use of a single titanium orbital mesh fan plate (DePuy Synthes, Warsaw IN) in an injury-specific and cost-effective technique to rapidly resto

Modified Lengthening Temporalis Myoplasty Using an Intraoral Approach.

BACKGROUND: Lengthening temporalis myoplasty (LTM) is a unique and definite facial reanimation procedure that involves inserting the whole temporal tendon directly into the nasolabial fold. However, the nasolabial fold incision remains one of the difficulties of this procedure, particularly in young or female patient. To avoid the need for this incision, the authors developed an intraoral approach to manipulate the temporal muscle tendon toward the nasolabial fold. in this study, we describe the details of this procedure. METHOD: The authors performed their intraoral approach procedure in 5 p

Reconstruction of soft tissue defects around the Achilles region with distally based extended peroneal artery perforator flap.

INTRODUCTION: Achilles tendon rupture and soft tissue infections with wound dehiscence and tendon exposure following the tendon repair are not infrequent. Various procedures have been described for the reconstruction of soft tissue defects at the Achilles tendon region, yet there is lack of consensus on the ideal method. In this article we report our experience using the distally based peroneal artery perforator flap in reconstruction of combined defects of the Achilles tendon and overlying soft tissue. METHODS: 7 patients with Achilles tendon injury and full-thickness soft tissue defects ove

Evaluating the use of anterolateral thigh flaps to prevent reconstruction plate exposure in patients with oral cancer.

OBJECTIVE: Plate exposure remains one of the most serious complications of oromandibular reconstruction. The purpose of this study was to investigate the effect of wrapping reconstruction plates with anterolateral thigh (ALT) free flaps as a way to prevent plate exposure. STUDY DESIGN: A total of 91 patients with composite oromandibular defects who underwent surgical reconstruction using a reconstruction plate in conjunction with an ALT musculocutaneous flap were recruited. The participants were divided into 2 groups, based on whether the plate was wrapped with or covered by the ALT flap. RE

Lumbar Perforator Flaps for Coverage of Extensive Defects With Osteomyelitis.

INTRODUCTION: Lumbar osteomyelitis is a rare, although serious condition if not appropriately treated, potentially leading to neurologic complications, such as radiculopathy. Traditionally, studies have suggested the preferred use of muscle or myocutaneous flaps to prevent recurrent infections. However, late evidence suggests that radical debridement and dead space obliteration are more important than the type of flap. The lumbar perforator flap is a reliable local option with low donor site morbidity. It is a powerful tool for local trunk reconstruction, but its use in case of osteomyelitis i

Unicortical sternal graft reconstruction for anterior sternoclavicular joint instability.

Sternoclavicular joint (SCJ) injury represents 3%-5% of all shoulder girdle injuries, yet can produce significant pain and disability. While conservative management improves symptoms in most cases, surgical intervention may be indicated for patients with symptoms recalcitrant to nonoperative treatment. A wide range of surgical stabilisation techniques is documented in the literature; however, the scarcity of SCJ pathology has hindered development of a 'gold standard'.We present a minimalistic medial clavicle osteoplasty and SCJ reconstruction using semitendinosus autograft anchored with unicor

Posterior auricular artery helix root free flap-part II: clinical application.

The surgical repair of full-thickness defects involving the alae nasi is complex. Pedicle flaps such as frontal and nasolabial flaps can be used, but require several interventions with different techniques. In contrast, free flaps from the foot of the ear helix allow the three layers of the nasal wings to be reconstructed in a single operation. Nevertheless, in the classical approach, the vascular pedicle is short. Although some authors have proposed raising the flap in a retrograde manner, this still yields a relatively short pedicle with narrow vessels. In the companion paper, we demonstrate

Management of Complicated Nasal Defects.

The nose is the most common site for nonmelanoma skin cancer. Fortunately, most skin cancer lesions on the nose are discovered early and their timely diagnosis and excision result in superficial defects that can be reconstructed with minimal impact on the aesthetics and function. Reconstruction of full-thickness defects of the nose remains a challenging endeavor for reconstructive surgeons. An organized and systematic approach is necessary to streamline the analysis, planning, and reconstruction complex nasal defects to consistently achieve optimal results. [Lipotransfer after free-flap tongue reconstruction can improve dysphagia and articulation disorders: a case report].

Atrophy of tongue after free-flap reconstruction often leads to functional problems like dysphagia and articulation disorders. Lipotransfer might resolve this issue. We describe a case of repeated lipotransfer into an atrophied reconstructed tongue.A 66-year-old patient with a squamous cell carcinoma of the tongue underwent hemiglossectomy and reconstruction of the tongue by a free radial forearm flap. After atrophy of the reconstructed tissue, repeated sessions of lipotransfer were undertaken to augment the atrophic tissue. Three sessions of lipotransfer resulted in satisfactory augmentation
