Recovery of airway protection compared with ventilation in humans after paralysis with curare.

d-Tubocurarine (dTc) was administered intravenously to six healthy unanesthetized volunteers to assess the sensitivity to neuromuscular blockade of those muscles involved in protecting the airway against obstruction and/or aspiration relative to the muscles of inspiration. Each subject was given an intravenous bolus of dTc followed by an infusion to allow three different levels of inspiratory muscle weakness as measured by maximum inspiratory pressure (MIP). Levels of MIP were control (-90 cm H2O), -60, -40, and -20 cm H2O. Vital capacity (VC), hand grip strength (HGS), and end-tidal CO2 (PETC

[A case of jugular foramen meningioma in a child].

The common tumors originating in the jugular foramen are chemodectoma and schwannoma. Jugular foramen meningioma is extremely rare. Review of the literature revealed only seven reported cases of this tumor. The authors present a child case of jugular foramen meningioma with intra and extracranial extension. A 9-year-old boy was admitted to the Department of Neurosurgery, Hiroshima University School of Medicine on March 20, 1985. Since the age of 3, the patient had hoarseness and was found to tilt his neck when he shouted. Since the age of 6, he was found to nod when he swallowed. At the age of

G syndrome: an unusual family.

The G syndrome is a multiple congenital anomaly (MCA) syndrome of hypertelorism, hypospadias, stridor, and swallowing difficulties. Cleft lip and palate, cardiac defects, cranial asymmetry, and bowel obstruction are occasional manifestations. Family data suggest autosomal dominant inheritance. Males appear to have more serious manifestations, whereas most females with the G syndrome have a benign course and are ascertained through affected male relatives. In the family reported here, the proposita was first seen at age 7 weeks because of swallowing difficulties, stridor, and unusual facial app

Panendoscopy for second primaries in head and neck cancer.

Four hundred patients with biopsy-proven squamous cell carcinoma of the upper aerodigestive tract were studied for evidence of a synchronous second primary tumor. The protocol of study included in most cases direct laryngoscopy, metal tube bronchoscopy, and esophagoscopy. Chest x-ray studies were performed on all patients and many had barium swallow x-ray studies. Asymptomatic second head and neck primary tumors were found only in the vallecula and esophagus among this series of patients. Metachronous second primary tumors were presented as early as one month after the original endoscopy and w

The OMA-Project-Orofacial Myofunctional Behaviour in Adults: Effectiveness of Orofacial Myofunctional Therapy Protocols in Healthy Young Adults With Orofacial Myofunctional Disorders.

OBJECTIVES: This study aimed to assess the short- and long-term effectiveness of two intensive Orofacial Myofunctional Therapy (OMT) protocols on orofacial myofunctional status, orofacial strength and Oral Health Related Quality of Life (OHRQoL). One protocol focused exclusively on breathing patterns and orofacial posture (Breathing and Posture Only group; OMTBPO), while the other also incorporated orofacial muscle strengthening and swallowing patterns (Breathing, Posture, Strength and Swallowing group; OMTBPSS). Additionally, adherence to home-based exercises and satisfaction with OMT interve

Patient-Reported Outcomes of Functional Orthodontic Treatment for Skeletal Class II and III Malocclusions in Growing Patients: A Systematic Review.

Functional orthopedic appliances are widely used to manage skeletal Class II and Class III malocclusions in growing patients due to their well-established dentoskeletal effects. However, their impact on patient-reported outcome measures (PROMs) remains insufficiently investigated. PROMs, including pain, functional impairment, swallowing difficulties, and oral health-related quality of life (OHRQoL), are essential components of patient-centered care and are increasingly emphasized in contemporary orthodontic research. This systematic review aimed to critically evaluate the available evidence re

The Effect of Repetitive Transcranial Magnetic Stimulation With Different Stimulation Methods on Post-Stroke Dysphagia: A Network Meta-Analysis.

BACKGROUND: Multiple studies have substantiated that repetitive transcranial magnetic stimulation (rTMS) is effective in improving the swallowing function of patients with post-stroke dysphagia (PSD). Nevertheless, the choice of stimulation methods diverges, underpinned by different recovery theories. Among the distinct stimulation approaches currently in use, which one yields the most optimal therapeutic outcomes remains unexplored. OBJECTIVE: To evaluate the rehabilitation efficacy of different rTMS methods on the swallowing function of PSD patients through network meta-analysis and traditi

Unilateral Ptosis and Bulbar Symptoms as the Initial Presentation of Late-Onset Acetylcholine Receptor Antibody-Positive Myasthenia Gravis Mimicking Acute Ischemic Stroke in an 82-Year-Old Man.

Myasthenia gravis is an autoimmune disorder of the neuromuscular junction. In older adults, it may present with bulbar and ocular symptoms that resemble acute stroke. Ocular myasthenia is typically asymmetric and often bilateral; persistent unilateral ptosis in this context can be misleading. An 82-year-old man with type 2 diabetes mellitus, chronic kidney disease, cataracts, and macular degeneration presented with a two-week history of fatigable dysphagia, slurred speech, and new-onset unilateral left ptosis. Symptoms were worse towards the end of the day. He denied diplopia and limb weakness

Surgical management of jugular foramen meningiomas: a function-prioritized perioperative workflow.

OBJECTIVE: Jugular foramen meningiomas are challenging because of their deep, neurovascularly crowded location and multicompartment extension; hyperostosis and rigid dural attachment further narrow the corridor and increase the risk of lower cranial nerve morbidity, causing dysphagia and airway complications that may rarely require tracheostomy. This study aimed to describe a contemporary function-first workflow integrating compartment-based anatomy, venous sinus status, preoperative embolization, and continuous vagus nerve monitoring and its relation to clinically actionable recovery endpoint

Intensive multidisciplinary feeding intervention to address malnutrition and saliva pooling in two autistic adolescents with avoidant/restrictive food intake disorder.

Avoidant-restrictive food intake disorder (ARFID) in autistic youth may present with heterogeneous symptoms. The present case series illustrates the treatment of two autistic adolescents with fear-based ARFID who both displayed significant regressions in feeding that led to precipitous weight loss, malnutrition, and nasogastric tube placement. Both patients refused all oral intake and had stopped swallowing saliva. Developmental and social barriers likely delayed access to care. This case series illustrates the multiple, intersecting challenges imposed by medical and developmental conditions a
