Efficacy and Safety of Split Peroneal Tendon Lateral Ankle Stabilization.

Chronic lateral ankle instability is a common condition. Split peroneal tendon lateral ankle stabilization, a modification of the Chrisman-Snook procedure, is biomechanically stable and often used for severe and/or recurrent chronic lateral ankle instability. The purpose of the present study was to evaluate the efficacy and safety of this technique. Specifically, the midterm recurrence of instability and postoperative complications, such as stiffness, neurologic pain, and wound healing complications, were evaluated. We evaluated 30 consecutive procedures with a minimal follow-up period of 1 ye

Tenodesis of the long head of biceps brachii in the painful shoulder: improving results in the long term.

Fifteen shoulders of 14 patients with a keyhole tenodesis of the long head of the biceps were reviewed at an average follow-up of 7 years (3 years, 1 month to 13 years, 2 months). In 13 cases additional shoulder disease was noted during the operation. Eight patients had undergone rotator cuff decompression before the reference biceps tenodesis was performed. Eight (53%) cases achieved an excellent result; one was rated as good, four were rated as fair, and two had failures. Seven shoulders had an improved result from short to long term, and only two deteriorated. An upward migration of the hum

Editorial Commentary: Utility of Bone Marrow Aspirate Concentrate in Orthopaedic Surgery Remains Questionable.

Despite multiple advancements in rotator cuff repair fixation and technique that have allowed for stronger and more anatomic constructs, failure remains commonplace. As is a common theme throughout orthopaedics, both biomechanics and biology play a role in successful healing. The new frontier in tendon-to-bone healing appears to be pairing biologic augmentation, such as bone marrow aspirate concentrate (BMAC), with previous biomechanical advancements. However, only 8 studies have specifically investigated the utility of BMAC in rotator cuff repair, and as of yet, there are minimal data to sugg

Free functional gracilis muscle opponensplasty for thenar reconstruction: Indications, technique, and long-term outcomes.

Thumb opposition is an essential component of human hand function, and loss of the thenar musculature creates substantial disability of the hand. Additionally, first web space contracture as a result of trauma is a substantial limitation in reconstruction of thumb opposition. Reconstruction of thenar function is most commonly performed via opponensplasty tendon transfers. Although beneficial, tendon transfers require cortical retraining and fall short of native function. These may be the only option in the case of median nerve palsy. However, at times, thenar function is lost owing to thenar m

Arthroscopic Resection of Symptomatic Tibial Tubercle Ossicles for Recalcitrant Osgood-Schlatter Disease Using a 2-Portal Technique.

Arthroscopic resection of symptomatic unfused tibial tubercle ossicles causing chronic anterior knee pain due to recalcitrant Osgood-Schlatter disease has been shown to be a reproducible arthroscopic technique. Although a number of other surgical techniques have been described, including ossicle excision and tubercle-plasty, drilling of the tibial tubercle, bone peg insertion to induce fusion, open excision of loose fragments, direct bursoscopic ossicle resection, and closing-wedge tubercle osteotomy, with varied outcomes, this technique offers a minimally invasive approach with low risk. Traumatic Hallux Varus in Association with Agenesis of the Fibular Sesamoid: A Case Study.

Hallux varus is most commonly seen iatrogenically following overaggressive lateral release, removal of the fibular sesamoid, or overaggressive removal of the medial eminence. There are several reported cases of traumatic hallux varus, although this is much less common. We present a case of traumatic hallux varus in a patient who was later found to have bilateral absence of her fibular sesamoids. We postulated that lack of her fibular sesamoid led to weakness of her lateral capsular ligaments, thereby making her more susceptible to this injury. We performed a repair using a split extensor hallu

Arthroscopic treatment of lateral ankle instability. Is there a safe zone? An anatomic study.

BACKGROUND: The purpose of this study is to describe the relative location of superficial anatomic landmarks and likely location of structures at risk in order to predict the proximity of the later and avoid their injury during the arthroscopic treatment of lateral ankle instability. METHODS: Fifteen cadaver ankles were dissected. Based on superficial anatomic landmarks, the location and distances to the structures at risk (extensor tendons, peroneus tertius, peroneal tendons, main branch or intermediate branch of the superficial peroneal nerve, and the sural nerve) were measured. RESULTS: T

Assessment of the structures at risk during wrist arthroscopy: a cadaveric study and systematic review.

UNLABELLED: We assessed the proximity of neurological structures to arthroscopic portals in a cadaveric study and through a systematic review. Arthroscopy was performed on ten cadaveric wrists. Subsequently the specimens were dissected to isolate the superficial branch of the radial nerve, the dorsal branch of the ulnar nerve, the posterior interosseous nerve and the extensor tendons. We measured the distances from the nerves to common portals. For the systematic review Pubmed and EMBASE were searched on the 31 May 2014 for cadaveric studies reporting the proximity of neurological structures t

The ligament augmentation device in hamstring grafts for reconstruction of the anterior cruciate ligament.

Twenty-eight patients who had reconstructions of the anterior cruciate ligament with a ligament augmentation device in the semitendinosus and gracilis tendons were compared with 32 patients who had reconstructions with semitendinosus and gracilis tendons alone. Preoperatively, there was no significant difference between the 2 groups with respect to age, sex, Tegner activity level, and time from injury to operation. Mean postoperative followup was 31.4 months for the augmentation group and 32.3 months for the nonaugmentation group. At postoperative evaluation, there was no difference in the 2 g

Anatomy of the axillary nerve and its relation to inferior capsular shift.

Axillary nerve injury is a recognized complication of the capsular slide procedure for multidirectional instability of the shoulder. Axillary nerve dissection followed by an anterior or posterior capsular shift procedure was carried out on 12 autopsy subjects to observe: (1) the normal relationships of the nerve; (2) its proximity to structures dissected in the procedure; and (3) the effects upon it of both anterior and posterior capsular shift procedures. The axillary nerve arises immediately posterior to the coracoid process and conjoint tendon. It crosses the inferolateral border of the sub

Chronic Acromioclavicular Joint Instability: Outcomes after Stabilization without Tendon Graft Augmentation.

INTRODUCTION: Chronic acromioclavicular joint (ACJ) instability only rarely causes significant symptoms. In the uncommon cases with substantial pain and impairment, surgical treatment may become necessary. Many surgeons recommend tendon-graft augmentation to reconstruct the coracoclavicular (CC) ligaments. However, it remains unclear whether graft use is mandatory in all chronic cases. This study evaluates clinical and radiographic outcomes after open reduction and stabilization of chronic ACJ dislocations without grafting. MATERIALS AND METHODS: Sixteen consecutive patients with chronic, sym

Umbilical cord-derived mitochondrial transplantation restores tendon structure and function in a preclinical model of achilles tendinopathy.

BACKGROUND: Achilles tendinopathy is a degenerative musculoskeletal disorder for which disease-modifying therapies remain limited, largely due to the inability of current interventions to directly restore cellular bioenergetic function. Emerging evidence has identified mitochondrial dysfunction as a central contributor to tendon degeneration, highlighting mitochondria as a potential therapeutic target. Here, we evaluated umbilical cord-derived mitochondria (UC-MT) as a dose-defined, cell-free therapeutic strategy for tendinopathy. METHODS: UC-MT were isolated from human umbilical cord-derived

Magnetic Resonance Imaging With a Novel Hip Flexion Scanning Position for Diagnosing Proximal Hamstring Tendinopathy.

BACKGROUND: Making a diagnosis of proximal hamstring tendinopathy (PHT) may be challenging, as patients with correlating clinical symptoms may have normal or minimal findings on magnetic resonance imaging (MRI) scans. PURPOSE/HYPOTHESIS: The purpose of this study was to assess the effect of a novel hip flexion (HF) scanning position on the MRI diagnosis of PHT. It was hypothesized that the HF position, which simulates the symptom-provoking sitting position, would reveal PHT pathology more accurately than the standard scanning position. STUDY DESIGN: Cohort study (diagnosis); Level of evidenc

Isolated Rupture of the Short Head of the Biceps at the Proximal Musculotendinous Junction: A Report of Two Cases.

CASE: Two men aged 19 and 21 years sustained wakeboarding injuries where the towrope was forced against the anterior region of the arm. Physical examination revealed bruising and tenderness over the biceps with weakness in elbow flexion. Magnetic resonance imaging demonstrated complete musculotendinous rupture of their short head of biceps. Surgical repair followed by cast immobilization led to an excellent outcome in both cases. CONCLUSIONS: This is a unique series of complete musculotendinous rupture of the short head of the biceps occurring in wakeboarding accidents, with the mechanism of

Characteristics of acute groin injuries in the hip flexor muscles - a detailed MRI study in athletes.

Hip flexor injuries account for one-third of acute groin injuries; however, little is known about specific injury characteristics. The aims of this study were to describe acute hip flexor injuries using magnetic resonance imaging (MRI) in athletes with acute groin pain and to compare specific muscle injuries with reported injury situations. Male athletes with acute groin pain were prospectively and consecutively included during three sports seasons. MRI was performed within 7 days of injury using a standardized protocol and a reliable assessment approach. All athletes with an MRI confirmed acu

Throwing injuries of the upper extremity.

The overhead throwing motion is a complex sequence of maneuvers that requires coordinated muscle activity in the upper and lower extremities. The shoulder and elbow are subject to multidirectional forces and are particularly vulnerable to injury during specific phases of the overhead throwing motion. Ligamentous, tendinous, neural, and osseous pathology that may occur in the shoulder or elbow of an overhead-throwing athlete will be discussed, with an emphasis on the role of MR imaging and MR arthrography.

Surgical decompression of impingement in the weightbearing shoulder.

Shoulder pain is a common affliction, posing particular limitations on the spinal cord injured person. Abnormalities of the rotator cuff mechanism are a common cause of shoulder injury in the general population and it has recently been observed that a large percentage of persons with paraplegia suffer from chronic shoulder pain. This report describes six cases of impingement syndrome in four spinal cord injured persons who depend on upper extremity function for the execution of activities of daily living and mobility. All had failed prolonged trials of conservative therapy and subsequently und
