IMPORTANCE: The success of parathyroidectomy depends on accurate intraoperative localization and identification of all diseased glands in parathyroid exploration based on surgeon expertise to prevent persistent hyperparathyroidism. Near-infrared autofluorescence (NIRAF) imaging has recently emerged as a promising adjunctive intraoperative tool for localizing parathyroid glands; however, its potential utility in the assessment of parathyroid glands has yet to be established.
OBJECTIVE: To analyze the differences in NIRAF signatures of parathyroid glands in single vs multiple glands in primary

BACKGROUND: Caesarean section is a common surgery, with almost 23 million procedures performed globally each year. Postpartum haemorrhage, in association with caesarean section surgery, is a leading global cause of maternal morbidity and mortality. Perioperative iron deficiency anaemia is a risk factor for intraoperative bleeding. Therefore, anaemia is an important and modifiable risk factor for bleeding during caesarean section surgery. Recent recommendations advise that all preoperative patients with anaemia (defined as haemoglobin concentration (Hb) < 130 g/L), regardless of sex, be assesse

BACKGROUND: Palestine refugees from Syria (PRS) are among the most vulnerable refugee groups for adverse health outcomes and require assistance from humanitarian agencies. As the armed conflict in Syria has continued, most Palestinians have been displaced from that country to neighbouring countries, where they have experienced difficulties in accessing essential services. More than 17 000 PRS are in Jordan as of 2018 and have received assistance from the United Nations Relief and Works Agency for Palestine Refugees in the Near East (UNRWA), including free primary care and subsidised secondary

BACKGROUND: Acute illness requiring hospitalization frequently is a sentinel event leading to long-term disability in older people. Prolonged bed rest increases the risk of developing cognitive impairment and dementia in acutely hospitalized older adults. Exercise protocols applied during acute hospitalization can prevent functional decline in older patients, but exercise benefits on specific cognitive domains have not been previously investigated. We aimed to assess the effects of a multicomponent exercise intervention for cognitive function in older adults during acute hospitalization.

BACKGROUND & AIMS: There are few data available on the real-life pharmacokinetic and pharmacodynamics features of vedolizumab, a monoclonal antibody against integrin α4β7. We performed a prospective study of patients with inflammatory bowel diseases (IBDs) treated with vedolizumab to determine serum drug concentrations, formation of antivedolizumab antibodies (AVAs), and integrin α4β7 saturation.
METHODS: We performed a prospective study of 106 patients with IBD (67 with Crohn's disease and 39 with ulcerative colitis) treated with vedolizumab from September 2014 through March 2017 at 2 tertia

INTRODUCTION: Opioid-based analgesic therapy represents a cornerstone of pain management after surgery. The recent rise in opioid sales and opioid overdoses suggests it is important to maximize the safety of opioid prescribing after surgery. Given that patients may live with other family members in the home, safe storage and appropriate disposal of excess opioids after hospital discharge are necessary to prevent unintended secondary exposures. Identifying characteristics of patients who are likely to be prescribed excess opioids after surgery may enable more targeted prescription practices and

Background: Intraoperative vascular injuries during elective hip and knee arthroplasty are uncommon but limb-threatening complications. Real-world evidence on emergency on-call vascular management in this setting remains limited. We aimed to identify the intraoperative ischemia time threshold associated with progression to a more severe ischemic presentation (Rutherford IIb) at vascular consultation, in order to support early multidisciplinary activation and prevent irreversible ischemic limb damage. As a secondary aim, we described the clinical spectrum, treatment strategies, and 30-day outco

BACKGROUND: Carbapenemase-Producing Enterobacteriaceae (CPE) are known to be on the rise globally and are a major concern. Rapid screening of at-risk patients and implementation of infection control measures can prevent colonization from becoming a source of infection.
OBJECTIVE: We investigated the incidence of carbapenem-resistant Enterobacteriaceae (CRE) colonization, CRE-producing Enterobacteriaceae species, carbapenemase gene types and prevalence in patients subjected to CRE screening tests in rectal swab samples using the CHROMAgar culture method and real-time Polymerase Chain Reaction

BACKGROUND: Cancer screening nonadherence persists among adults who are deaf, deafblind, and hard of hearing (DDBHH). These barriers span individual, clinician, and health care system levels, contributing to difficulties understanding cancer information, accessing screening services, and following treatment directives. Critical communication barriers include ineffective patient-physician communication, limited access to American Sign Language (ASL) cancer information, misconceptions about medical procedures, insurance navigation difficulties, and intersectional barriers for multiply marginaliz

BACKGROUND: Critical care nurses are vulnerable to depression, which not only lead to poor well-being and increased turnover intention, but also affect their working performances and organizational productivity as well. Work related factors are important drivers of depressive symptoms. However, the non-liner and multi-directional relationships between job demands-resources and depressive symptoms in critical care nurses has not been adequately analyzed. Understanding these relationships would be helpful for reducing depression, increasing nurses' well-being and retain healthcare forces.

Background Diabetic foot ulcers (DFUs) represent a severe complication of diabetes, contributing to increased morbidity, escalating healthcare expenses, and a heightened risk of limb amputation. Early detection is crucial for preventing ulcer progression. Biomarkers offer a non-invasive approach to identifying at-risk individuals. This study evaluates emerging serum biomarkers for early DFU detection in the South Indian population, focusing on inflammation, oxidative stress, and endothelial dysfunction markers. Materials and methods A cross-sectional study was carried out at our tertiary care

BACKGROUND: Intermediate-high risk acute pulmonary embolism (PE) carries a significant long-term burden of post-thrombotic syndrome (PTS) and chronic thromboembolic pulmonary hypertension (CTEPH). Standard anticoagulation does not actively resolve obstructive thrombi, and systemic thrombolysis is limited by high bleeding risk. While percutaneous mechanical thrombectomy (PMT) offers rapid thrombus removal, robust multicenter evidence regarding its efficacy in preventing long-term complications is scarce, leading to its current class IIb recommendation in major international guidelines (2022 ESC

BACKGROUND: There are concerns that biocide skin and mucous membrane decolonisation, which is widely used to prevent health-care-associated infections in intensive care units (ICUs), might select for multidrug-resistant pathogens. We aimed to evaluate the effects of de-escalating from universal to targeted skin and nasal decolonisation on Staphylococcus epidermidis bloodstream infections (SE-BSI).
METHODS: We did a retrospective, before-after-control-impact time-series analysis and longitudinal genotypic study in two ICUs with divergent decolonisation practice in tertiary care hospitals of ad

BACKGROUND: Surgeon-administered transversus abdominis plane block is a contemporary approach to providing postoperative analgesia, and this approach is performed by transperitoneally administering local anesthetic in the plane between the internal oblique and transversus abdominis muscles to target the sensory nerves of the anterolateral abdominal wall. Although this technique is used in many centers, it has not been studied prospectively in patients undergoing a midline laparotomy.
OBJECTIVE: This study aimed to evaluate whether surgeon-administered transversus abdominis plane block reduces

STUDY QUESTION: Does treatment selection for cervical lesions affect the outcome of IVF/ICSI?
SUMMARY ANSWER: There was no difference in pregnancy outcome between treated and untreated groups, or between different types of IVF/ICSI treatment.
WHAT IS KNOWN ALREADY: Human papillomavirus (HPV) infection and HPV-induced cervical lesions are associated with decreased fertility, and cervical intraepithelial neoplasia (CIN) treatment may increase the risk of adverse pregnancy outcomes.
STUDY DESIGN, SIZE, DURATION: Between 2018 and 2020, 190 women with infertility who had abnormal HPV screening o
