Urethral tumor involvement was examined in 910 patients treated for bladder cancer at a single institution over a period of 25 years. The overall incidence in 2,052 primary and recurrent bladder-tumor events was 6.1%. Risk factors for urethral tumor occurrence are tumors at the bladder neck and recurrent multifocal tumors. Carcinoma in situ (CIS) of the bladder not involving the bladder neck and muscle-invasive tumors with or without lymph-node involvement are not significantly correlated with urethral cancer. The recent introduction of endourology and accumulated experience has led to the development of concepts regarding the effective role of conservative surgical treatment in cancer of the upper urinary tract. The Authors report their experience in 59 patients with cancer of the upper urinary tract; 36 (61.2%) underwent radical surgery whereas 23 (38.9%) were treated using conservative surgical therapy. The results of this retrospective study confirm the role of conservative surgical therapy in cases at an initial stage of cancer development, together with an attentive endourological follow-up.

Between 1967 and 1986, 319 patients, judged unsuitable for cystectomy, were scheduled to receive curative radiation treatment for transitional cell cancer of the urinary bladder. Crude and corrected 5-year survival for all stages were 18% and 28%, respectively. Corrected 5-year survival by stage was: T1-57%, T2-31%, T3-16% and T4-6%. The sexual concerns of patients with cancer may be overlooked during the acute stage of illness. Both the general stress of hospitalization and the specific threat of surgery can affect sexuality. The application of the Argon laser in urology has proved to be effective in resecting recurrent, exophytic urothelial tumors of the bladder up to the size of a raspberry. In cases of wide spread bladder tumors we only perform a radiation of the resected area as local recurrence prophylaxis following transurethral resection. The urethroscopic Argon laser irradiation makes laser urethrotomy and evaporisation of urethral strictures possible. Furthermore, a curative and conservative treatment of urethral tumors is possible in combination with chemotherapy. The same applies for the penis carcinoma.

A man with crossed, non-fused renal ectopia had carcinoma of the distal ureter, leading to hydro-ureter, hydronephrosis and urinary infection. Crossed ectopic kidneys are more disease-prone than are normal kidneys. Complicating disorders are discussed, the value of thorough urologic investigation is stressed and the incidence of tumour of the upper urinary tract in crossed renal ectopia without fusion is reviewed.
