Digital vaginal and ultrasound assessments and the likelihood of intrapartum cesarean delivery: a prognostic factor study.

PURPOSE: Ultrasound offers an objective method of assessing the progress of labour. We sought to examine whether repeat clinical and ultrasound assessments in labour were prognostic for mode of delivery. MATERIALS AND METHODS: Term nulliparous women underwent ultrasound of fetal head position, head descent measured as head-perineum distance, caput succedaneum, moulding, umbilical and middle cerebral artery Doppler and assessment of cervical dilatation from digital vaginal examination. Univariable and multivariable logistic regression analyses were used to investigate a prognostic model for in

Multimodal Ultrasound in Evaluating Radiotherapy Efficacy for Vaginal Squamous Cell Carcinoma: A Case Report.

BACKGROUND: Vaginal malignancy is rare and carries a poor prognosis. MRI is the reference standard for staging and follow-up, but cost and availability are limiting. We describe the use of high-frequency transrectal ultrasound (HF-TRUS) for pre-treatment mapping and early response assessment of primary vaginal cancer. CASE PRESENTATION: A 65-year-old woman presented with post-menopausal bleeding for 20 days. Histology of the right vaginal fornix revealed high-grade squamous intraepithelial lesion (HSIL). Clinical stage cT1N0M0 poorly differentiated carcinoma was confirmed, and curative radiot

Sexual transmission of hepatitis E virus via vaginal and rectal routes in a rabbit model.

Hepatitis E virus (HEV) is a major cause of acute hepatitis worldwide. While faecal-oral transmission is well-established, the potential for sexual transmission remains unclear. This study aimed to investigate whether HEV can be transmitted through sexual routes using a rabbit model. A total of 80 rabbits were inoculated intravaginally or intrarectally with HEV, compared to intravenous challenge. To assess hormonal influence, rabbits were pre-treated with medroxyprogesterone acetate (MPA) or pregnant mare serum gonadotrophin (PMSG) prior to vaginal inoculation. Also, 12 pregnant rabbits were i

Comprehensive Diagnosis of Abnormal Vaginal Discharge Using qPCR-Based Microbial Dysbiosis Indices.

Background/Objectives: Abnormal vaginal discharge (AVD) is a common complaint among women of reproductive age, often involving multiple, overlapping etiologies, most commonly bacterial vaginosis (BV), vulvovaginal candidiasis (VVC), aerobic vaginitis (AV), and sexually transmitted infections (STIs). We aimed to evaluate a syndromic diagnostic approach by developing qPCR-derived dysbiosis indices for BV, VVC, and AV, subsequently comparing their performance against established reference methods and clinician-assigned diagnoses. Methods: Vaginal swabs were collected in a case-control design from…

Neuraminidase-Associated Vaginal Dysbiosis Correlates with HPV Infection Across Oncogenic Risk Subtypes.

BACKGROUND: Human papillomavirus (HPV) infection is a major cause of cervical cancer, and increasing evidence suggests that cervicovaginal microbial dysbiosis may influence HPV susceptibility. This study evaluated the association between microbial enzymatic markers and HPV infection across different risk subtypes. METHODS: A cross-sectional analysis was conducted in 43,426 women undergoing HPV screening. HPV status and subtype classification (high-, possible-, and low-risk) were determined using standardized genotyping. Clinical and laboratory parameters including age, white blood cells (WBC)

An Observational Study of Vaginal Antibacterial Activity and Outcomes after Antibiotic Prescription for Bacterial Vaginosis.

BACKGROUND: Bacterial vaginosis (BV) is the most common cause of vaginal discharge syndrome and usually treated with metronidazole. BV frequently recurs. Published studies have not linked measured vaginal antibiotic activity (VAA) or antibiotic concentrations to clinical or microbiological BV outcomes. The objective is to determine if VAA in vaginal fluid after diagnosis of BV is associated with persistent/recurrent BV or changes in concentrations of vaginal BV-associated bacteria (BVAB). METHODS: VAA was assessed using a bioassay to measure inhibition of growth of BVAB in culture. Persistent

First-void urine and cervicovaginal brushes for vaginal microbiome profiling: a proof-of-concept study.

UNLABELLED: The human vaginal microbiome is a cornerstone of female reproductive health, influencing susceptibility to vaginal conditions, pregnancy complications, and urogenital infections. Understanding this microbiome is critical for advancing preventive, diagnostic, and therapeutic interventions, yet its intimate and dynamic nature poses challenges for accurate, representative, and more inclusive sampling methods. This proof-of-concept study evaluated the performance of first-void urine (FVU) samples and cervicovaginal brushes for vaginal microbiome profiling relative to vaginal swabs. FVU

Prebiotic-matrix protective stabilization enables long-term probiotic storage for vaginal intervention.

Probiotic therapy is an emerging non-antibiotic strategy for bacterial vaginosis (BV). However, the susceptibility of probiotics to environmental factors leads to poor storage stability, and the complex, multi-step processes involved in current protection strategies increase production costs, thereby limiting their further clinical application. Here, we report a single-pot co-lyophilization formulation (termed prebiotic-matrix protective stabilization, PMPS), in which probiotics are uniformly dispersed within a protective prebiotic matrix. The formulation was characterized for microstructure,

Hidden endometrial cancer due to vaginal obliteration associated with lichen sclerosus/lichen planus.

An elderly postmenopausal woman with untreated vulvar lichen sclerosus and lichen planus (LP) overlap presented with abdominal pain, constipation and urinary symptoms. Severe vulvovaginal scarring caused complete vaginal obliteration, preventing conventional speculum examination and transvaginal ultrasonography. Endorectal and abdominal ultrasonography revealed a large cystic pelvic mass. Urgent laparotomy identified extensive haematocolpos and uterine distension due to complete outflow obstruction. The patient underwent a hysterectomy, bilateral salpingo-oophorectomy and subtotal colpectomy.

Bleeding control and posterior vaginal fornix approach for cesarean hysterectomy in placenta accreta spectrum.

OBJECTIVE: Placenta accreta spectrum (PAS) is a leading cause of massive obstetric hemorrhage, particularly when the placenta is removed or unintentionally disrupted during cesarean hysterectomy. In this video, we demonstrate an alternative surgical approach via the posterior vaginal fornix for performing cesarean hysterectomy in patients with PAS. METHODS: A cesarean hysterectomy for a case of placenta percreta was performed using the posterior vaginal fornix approach. This technique focuses on avoiding bleeding due to placental disruption and preventing urinary bladder injury. RESULTS: We

Radiological Insights Into Uterine Arteriovenous Malformation Causing Profuse Vaginal Bleeding: A Rare Case.

Uterine arteriovenous malformation (AVM) is a rare cause of severe genital tract bleeding. It can be classified as either congenital or acquired. Congenital form occurs due to aberrant arteriovenous vascular connections during embryonic development, whereas acquired form has been reported following previous uterine trauma or can be seen in association with gestational trophoblastic disease (GTD) and endometrial adenocarcinoma. This report describes a case of a 26-year-old female, who presented with heavy vaginal bleeding and pain in the lower abdomen. An ultrasound of the abdomen and pelvis, f

Association between changes in genital immune markers and vaginal microbiome transitions in bacterial vaginosis.

Bacterial vaginosis (BV), characterized by an imbalance in the vaginal microbiota, is a prevalent condition among women of reproductive age and a risk factor for human immunodeficiency virus, sexually transmitted infections, and preterm birth. BV is generally considered to induce mucosal inflammation, but the specific pathways and cell types involved are not well characterized. This prospective study aimed to assess associations between microbial changes and mucosal immune responses in BV patients. Therefore, samples from 20 premenopausal women with BV and treated with metronidazole were analy
